Susceptibility to moxifloxacin, penicillin and erythromycin was determined for 592 invasive Streptococcus pneumoniae collected from 20 English hospitals participating in the European Antimicrobial Resistance Surveillance System (EARSS) during 2003. Resistance to moxifloxacin, penicillin and erythromycin was observed in 0.8%, 5.4% and 13% of the isolates, respectively. These results show that the large majority of pneumococci were susceptible to moxifloxacin in 2003, the year when it was licensed for clinical use in the UK.

Following a case of diphtheria, 131 contacts were studied for throat and nose carriage. The carriage of C. diphtheriae was found to be 19.8%, 65.3% of them were toxin producing by counter-immunoelectrophoresis (CIEP). The carriers were treated with erythromycin for 7 days. Repeat swabs found them to be negative for C. diphtheriae except in four who had erythromycin resistant and penicillin sensitive strains. Penicillin treatment eliminated the organisms.

The susceptibility of 175 clinical isolates of Neisseria gonorrhoeae to penicillin G, ampicillin, erythromycin, tetracycline and spectinomycin was determined. Almost one-fifth of strains were relatively resistant to penicillin G. Relative resistance to penicillin G was significantly correlated with resistance to ampicillin, tetracycline and erythromycin, but not with resistance to spectinomycin. No penicillinase-producing or spectinomycin-resistant strains were encountered in this study. The relevance of the findings to the management of gonococcal urethritis is discussed.

We have tested the in vitro activities of streptomycin, rifampin, tetracyclines, trimethoprim-sulfamethoxazole, erythromycin, four new macrolides (roxithromycin, azithromycin, clarithromycin, and dirithromycin), and rifapentine against 62 strains of Brucella spp. Azithromycin and clarithromycin were, respectively, eight- and twofold more active than erythromycins (MIC for 90% of strains = 2, 8, and 16 micrograms/ml, respectively). The activity of rifapentine was similar to that of rifampin (MIC for 90% of strains = 1 microgram/ml).

In an 8-year-old boy, the Stevens-Johnson syndrome (SJS) was associated with protracted intrahepatic cholestasis and ultimately fatal respiratory disease. No precipitating factors of SJS were identified, but a Mycoplasma pneumoniae infection was suspected. The appearance of jaundice on the seventh day of illness was preceded by the prescription of erythromycin ethyl succinate, but intrahepatic cholestasis has never before been associated with this derivative of erythromycin. It is suggested that there may exist a subtype of SIS with severe hepato-pulmonary pathology.
