Delayed treatment of unstable proximal interphalangeal joint fracture-dislocations with a dynamic external fixator.

BACKGROUND: Fracture-dislocations of the proximal interphalangeal joint (PIPJ) remain a challenging problem to treat. Although there are a number of papers describing the use of dynamic external fixators and force couples for treatment of unstable PIPJ fracture-dislocations acutely, the literature is scarce on delayed treatment of PIPJ fracture-dislocations, where malunion of the articular surface may theoretically compromise postoperative range of motion (ROM) at the PIPJ. The purpose of this study was to evaluate the effectiveness of dynamic distraction external fixation (DDEF) for the delay…

[Treatment of hindfoot instability in Charcot foot using a hybrid technique of internal and external fixation].

OBJECTIVE: The main problem of patients with Charcot foot is their inability to off-load. Therefore the risk of internal fixation failure is increased, especially in hindfoot instability (Sanders type IV) with osteonecrosis of the talus. Combination of internal and additional external fixation guarantees the reconstruction and improves surgical outcome. The main objective of this surgery is to obtain a resilient, plantigrade foot that is shoeable in custom-made orthopedic shoes. INDICATIONS: Charcot foot with instable collapse of the hindfoot with or without fragment dislocation, with or with

Trichophyton rubrum osteomyelitis after calcaneus external fixation pin stabilization of a pilon fracture.

Fungal organisms are an uncommon cause of osteomyelitis, and no dermatophyte osteomyelitis infections have been reported in published studies. We present the case of Trichophyton rubrum osteomyelitis of the calcaneus. Our patient initially presented with a pilon fracture requiring temporary external fixation while awaiting definitive fixation. From our review of the published data, the present case is the first of this type of fungal osteomyelitis to be reported. The patient was evaluated for a left neck mass during his hospitalization that was later found to be consistent with salivary duct c

Distraction osteogenesis for complex foot deformities: Gigli saw midfoot osteotomy with external fixation.

Open midfoot wedge osteotomy correction can cause neurovascular compromise, requires extensive exposure, sacrifices normal joints, and shortens the foot. We used a minimally invasive technique to treat complex foot deformities by combining percutaneous Gigli saw midfoot osteotomy, circular external fixation, and acute, gradual, or gradual with acute manipulation correction. The medical records of 23 patients (26 feet) with complex foot deformities (congenital, 18 feet; neuromuscular, 4 feet; post-traumatic, 3 feet; malunion, 1 foot) who had undergone treatment within an 18-year period (1990 th

Posterior approach for subtalar joint distraction arthrodesis by compact external fixation: a technique guide.

Subtalar joint arthrodesis is a commonly used surgical procedure for the management of rearfoot pathologic features. We present a technique guide for a posterior incisional approach to subtalar joint arthrodesis for correction of a calcaneal deformity secondary to calcaneal fracture malunion. This technique uses a monolateral external fixation device for controlled distraction and intercallary allograft placement. In contrast to a standard lateral approach, this incision provides better visualization of the joint space, and the use of a distractor enhances access to the joint surfaces for fusi

Subtrochanteric valgus osteotomy with monolateral external fixator in hips for patients with severe cerebral palsy.

Subtrochanteric valgus osteotomy has been used for painful hip joint dislocation in patients with severe cerebral palsy. The goal of this study was to evaluate 11 patients (17 hips) with severe cerebral palsy who had chronically dislocated and painful hips treated with subtrochanteric valgus osteotomy using a monolateral external fixator. A retrospective review was performed of 11 patients (average age, 17.8 years) with severe quadriplegic cerebral palsy with flexion-adduction contractures due to chronically dislocated and painful hips. A subtrochanteric valgus osteotomy with a monolateral fix

Strength evaluation of lateral calcaneal wall with circular external fixation application in cadaveric specimens.

Circular external fixation is a useful treatment option for the correction of complex trauma, extremity deformity, osteomyelitis, and reconstruction of the foot and ankle. The goal of the present study was to determine the degrees of bolt rotation required to create enough wire tension to cause structural failure of the lateral calcaneal wall when stressed with both olive and smooth wires in a cadaveric model. Ten fresh, thawed, below-the-knee specimens were tested at the San Diego Cadaveric Academic Research Symposium. The mean bolt rotation required to pull an olive wire through the lateral

Use of temporary external fixation to generate a "customized" osteotomy of proximal femur in pediatric patients.

Proximal femoral osteotomies are performed commonly by pediatric orthopedic surgeons. Traditionally, utilization of a blade plate has been the standard method of realignment and fixation of the osteotomized fragments. In conjunction with use of temporary Schanz screws to generate the most appropriate realignment, newer implants with locking fixation capabilities can be used to stabilize essentially "customized" osteotomies of the proximal femur.

The stress response to bilateral femoral fractures: a comparison of primary intramedullary nailing and external fixation.

OBJECTIVES: Concern has recently been expressed with regard to the physiologic effects of primary intramedullary femoral nailing in seriously injured patients. "Damage control orthopaedics" techniques have been proposed, which comprise principally the use of primary external fixation. The aim of this study was to compare the physiologic effects of external femoral fixation with those of intramedullary stabilization over the first 24 hours after femoral fracture using an established large animal (ovine) trauma model. METHODS: Under terminal anesthesia, bilateral high-energy femoral fractures a

Treatment of bicondylar tibia plateau fractures using locked plating versus external fixation.

Bicondylar tibial plateau fractures can be difficult to treat due to the extent of articular cartilage, metaphyseal bone, and soft tissue injury. The purpose of this study was to compare the outcomes of open reduction and locked plating vs fine-wire external fixation of 58 consecutive bicondylar tibial plateau fractures at a level I trauma center. All bicondylar tibial plateau fractures were classified as Schatzker V/VI or AO/OTA type 41C. Twenty-eight patients in one group were treated using a locked plating system, and 30 patients in another group were treated with a hybrid or circular exter

Distal tibial reconstruction with use of a circular external fixator and an intramedullary nail. Surgical technique.

BACKGROUND: Distal tibial reconstruction with use of an external fixator when there is bone loss, limb-length discrepancy, and/or ankle instability is associated with many problems. The technique of limb-lengthening, ankle arthrodesis, and segmental transfer over an intramedullary nail has been introduced to overcome these problems. The present study investigates this combined technique. METHODS: Between 2002 and 2005, six patients, who ranged from seventeen to seventy years old, underwent distal tibial reconstruction and ankle arthrodesis with use of a circular external fixator and an intram

[Original vacuum dressing for the treatment of open femur fracture with gangrene immobilized by external fixation].

We report a case of gangrene, which developed following an open fracture of the femur immobilized with an external fixator in a 45-year-old patient. A conventional vacuum dressing (VAC Therapy) could not be applied with the external fixator in place. An original vacuum dressing was thus fashioned after surgical debridement. Scabs were covered with calcium alginate. The lower limb was enveloped in sterile dressings and vacuum was achieved by suction with gastric tubes under adhesive films. The dressing was redone every 48 h in a surgical setting. Antibiotic prophylaxis enabled cure of the infec

Open intercondylar fractures of the distal humerus: Management using a mini-external fixator construct.

Intercondylar fractures of the distal humerus are a challenging entity to treat. Open reduction-internal fixation with plating is well accepted as the standard treatment for these fractures, because it allows early mobilization and prevents joint stiffness. However, this technique may be associated with inferior postoperative results and a higher risk of infection in open fractures. External fixation appears to be an interesting option in such cases, where the results of open reduction-internal fixation with plating may be suboptimal. We present a bilateral uniplanar mini-external fixator cons
