General anesthesia during dental treatment in a pediatric patient with Fontan circulation using a minimally invasive cardiac output monitoring device.

This case report presents the hemodynamic management of a 3-year-old patient with Fontan circulation who underwent dental treatment under general anesthesia using minimally invasive cardiac output monitoring devices. To avoid the use of monitoring methods more invasive than the dental procedure itself, cardiac output was continuously assessed using FloTrac™ and BioZ.com™ systems. The bispectral index was maintained between 40 and 67 during anesthesia. Although the patient's body surface area (BSA) was 0.5 m2, which is below the validated threshold for both devices, monitoring was cautiously co

Review of pediatric combined heart-liver transplantation: A roadmap to success.

BACKGROUND: Combined heart-liver transplantation (CHLT) is a promising technique to address end stage organ failure in patients with concomitant heart failure and chronic liver disease. While most experience with CHLT has involved adult patients, the expanding population of children born with univentricular congenital heart disease who underwent the Fontan procedure and develop Fontan-associated liver disease (FALD) has emerged as a growing indication for pediatric CHLT. METHODS: Currently, CHLT is performed at a select subset of experienced transplant centers, especially in the pediatric pop…

Bidirectional cavopulmonary shunt with additional pulmonary blood flow.

There are some controversies regarding the usefulness of leaving additional pulmonary blood flow when establishing a bidirectional cavopulmonary shunt. From April 2002 to September 2008, 13 patients (mean age, 24 +/- 16 months) underwent a bidirectional cavopulmonary shunt procedure with fine adjustment of additional pulmonary blood flow, as an intermediate step before the Fontan operation. There were no hospital deaths. Modified Blalock-Taussig shunts were left during the bidirectional cavopulmonary shunt operation in 7 patients, and pulmonary bands were tightened in 4. The main pulmonary art

Total cavopulmonary connection for surgical treatment of complex congenital heart disease.

Diversion of vena caval flow directly into the pulmonary circulation--total cavopulmonary connection--has been tried at our hospital for two years as an alternative to the modified Fontan procedure for surgical treatment of complex congenital heart disease other than tricuspid atresia in 26 cases with six operative mortalities. The causes of death were low cardiac output in four, uncontrollable paroxysmal supraventricular tachycardia in one and airway obstruction in one. Except for one late death which occurred three months after surgery due to sepsis, the 19 patients who survived the operatio…

Malignant wide complex tachycardia after adenosine administration to a postoperative pediatric patient with congenital heart disease.

Adenosine has become the treatment of choice for paroxysmal supraventricular tachycardia because of its safety and efficacy. There have been no reports of malignant arrhythmias occurring after adenosine administration. This case report presents the occurrence of a malignant wide complex tachycardia after intravenous adenosine administration in a 10-year-old boy 2 days after a Fontan procedure. Thus the administration of adenosine in a critically ill postoperative patient can have morbidity or potential mortality and must be monitored closely.

Five-year outcomes in a paediatric and young adult Fontan cohort: the relevance of atrial function and body mass index.

INTRODUCTION: The Fontan procedure enables survival in individuals with univentricular physiology but is associated with progressive circulatory failure. Identifying predictors of adverse outcomes is essential to improve long-term management. This study evaluated five-year outcomes and baseline predictors of major events in a previously characterised paediatric and young adult Fontan cohort. METHODS: This retrospective longitudinal study included 51 patients (median age 18 years, interquartile range 11) who underwent comprehensive evaluation between 2018 and 2019, including echocardiography w…

[Closure of extrocardiac Fontan fenestration by using the Amplatzer duct occluder].

INTRODUCTION: A direct or tubular communication between the systemic venous system and the systemic atrium, generally called fenestration, is surgically created to improve the postoperative period of patients undergoing total cavopulmonary connection. However, a fenestration prompts a potentially deleterious right to left shunt, and is generally closed after the postoperative period. Direct fenestrations can be closed using coils, or devices designed for atrial septal defect closure. However, no devices have been designed for closure of extracardiac fenestrations. We report our experience conc

Management of Congenital Heart Disease: State of the Art-Part II-Cyanotic Heart Defects.

In this review management of the most common cyanotic congenital heart defects (CHDs) was discussed; the management of acyanotic CHD was reviewed in Part I of this series. While the need for intervention in acyanotic CHD is by and large determined by the severity of the lesion, most cyanotic CHDs require intervention, mostly by surgery. Different types of tetralogy of Fallot require different types of total surgical corrective procedures, and some may require initial palliation, mainly by modified Blalock-Taussig shunts. Babies with transposition of the great arteries with an intact ventricula

Comparison of Norwood shunt types: do the outcomes differ 6 years later?

BACKGROUND: A modification to the Norwood procedure involving a right ventricle-to-pulmonary artery (RV-PA) shunt may improve early postoperative outcomes. Concerns remain about the effect of the right ventriculotomy required with this shunt on long-term ventricular function. METHODS: Between January 2000 and April 2005, 76 patients underwent the Norwood procedure, 35 with a modified Blalock-Taussig shunt (mBTS) and 41 with a RV-PA shunt. Patients were monitored until death or September 1, 2009, with an average follow-up of 6.8 years. Cardiac catheterization, echocardiograms, perioperative Fo

The influence of pulmonary artery banding on outcome after the Fontan operation.

Thirty-eight patients were selected from a total of 120 patients who underwent the Fontan operation between 1974 and 1988. They were classified into two groups. Group 1 consisted of 18 patients with previous pulmonary artery banding at a mean age of 7 months (2 days to 59 months), and group 2 comprised 20 patients with native pulmonary stenosis. In group 1, 10 children had tricuspid atresia (seven with normally connected and three with transposed great arteries), six had double-inlet ventricle, and two had complex heart malformations. Group 2 consisted of 12 patients with tricuspid atresia and

Anesthetic Management of a 32-Year-Old Woman Undergoing Laparoscopic Gynecological Surgery With Prior Fontan Repair for a Complex Congenital Heart Defect.

BACKGROUND Fontan repair in complex congenital heart conditions involves creating a total cavopulmonary connection, redirecting systemic venous blood to the pulmonary circulation, and bypassing cardiac systemic blood flow. Anesthetic management for a patient with a Fontan circulation requires ensuring adequate preload and avoiding circulatory complications. This report describes the anesthetic management of a 32-year-old woman undergoing laparoscopic surgery with a history of Fontan repair of a complex congenital heart defect. Prediction of heart failure and death in an adult population of Fontan patients.

BACKGROUND: Late Fontan survivors are at high risk to experience heart failure and death. Therefore, the current study sought to investigate the role of non-invasive diagnostics as prognostic markers for failure of the systemic ventricle following Fontan procedure. METHODS: This monocentric, longitudinal observational study included 60 patients with a median age of 24.5 (19-29) years, who were subjected to cardiac magnetic resonance imaging, echocardiography, cardiopulmonary exercise testing, and blood analysis. The primary endpoint of this study was decompensated heart failure with symptoms …

Immunophenotyping and protein profiling of Fontan-associated plastic bronchitis airway casts.

RATIONALE: Plastic bronchitis (PB) is a rare and deadly condition that is characterized by the formation of airway casts. It most frequently occurs in children with underlying congenital heart disease that has been surgically palliated by the Fontan procedure. The Fontan circulation results in above-normal central venous pressure, and it has been hypothesized that the formation of airway casts is due to lymph leak. Knowledge of plastic bronchitis pathogenesis is poor and stems mostly from published case reports. OBJECTIVES: To garner information about cast pathogenesis by characterizing infla…

The effect of additional pulmonary blood flow on timing of the total cavopulmonary connection.

BACKGROUND: The staged Fontan procedure is used to palliate functionally univentricular hearts. The effect of additional pulmonary blood flow combined with a bidirectional cavopulmonary shunt in these patients remains a controversial subject. METHODS: This retrospective study included all 82 patients with a unilateral or bilateral bidirectional cavopulmonary shunt at our institution between April 1990 and July 2010. Patients with hypoplastic left heart syndrome were excluded. Two groups, based on the presence (n=57) or absence (n=25) of additional pulmonary blood flow after the bidirectional …
