We report on two cases of simultaneous asymmetrical bilateral hip dislocation. Both patients were involved in car accidents. The first case is a 23-year-old man who had a bilateral hip dislocation, anterior on the right side and posterior on the left associated with bilateral femoral head fracture. The second case presented the same dislocations of the hips associated with acetabular fracture on the right side. Closed reduction of the hips was performed in both cases. In the first case the femoral head fragments was subsequently removed. In the second case internal fixation of the acetabular f

The median nerve entrapment after elbow-dislocation is rare. We report two cases of fracture dislocation elbow completed by entrapment of median nerve occurred after reduction in two children. For both cases, the diagnosis of entrapment was made some months later after injury. Median nerve graft was performed after nerve resection. The nerve recovery was satisfactory at the final follow-up. This nerve complication should be suspected systematically before and after reduction of all dislocation of the elbow in the children. The indication of nerve release or nerve graft depends on the entrapmen…

BACKGROUND: Fracture dislocations of the knee involving the proximal tibia are difficult to treat because of the operative approach that maybe required and the instability of the construct. The optimal fixation method of these fractures remains controversial, improper fixations usually lead to an unsatisfactory prognosis. The purpose of this study was to compare the mechanical strength of four different fixation constructs for this kind of fracture.
METHODS: Twenty-eight Synbone simulated fracture models were created, reduced, and instrumented with one of four constructs: medial single plate,…

BACKGROUND: Traumatic cervical spinal cord injury with subaxial fracture and dislocation not only indicates a highly unstable spine but can also induce life-threatening complications. This makes first aid critically important before any definitive operative procedure is undertaken. The present study analyzes the various first aid measures and operative procedures for such injury.
MATERIALS AND METHODS: Two hundred and ninety-five patients suffered from cervical spinal cord injury with fracture and dislocation. The average period between injury and admission was 4.5 days (range 5 h-12 weeks).

Birth-related injuries in an uncommon location may be missed initially and may result in significant morbidity. We report for the first time a case of a Monteggia fracture dislocation in a neonate resulting from birth trauma. The delayed presentation at 7 days made open reduction necessary in this otherwise nonoperatively managed injury because of the accelerated rate of bony union in infants. All high-risk births, including 2nd-born twins, should be thoroughly screened to avoid initial misdiagnosis.

Injury cause, treatment, and long-term results [American Orthopaedic Foot and Ankle Society (AOFAS) Midfoot Score, Hannover Scoring System, Hannover Outcome Questionnaire] of patients with Chopart joint dislocations or fracture-dislocations were evaluated. Between 1972 and 1997, 100 patients with 110 Chopart joint dislocations were treated in the authors' institution. Pure Chopart joint dislocations were observed in 28 (25%) feet, fracture-dislocations in 60 (55%) feet, and combined Chopart-Lisfranc joint fracture-dislocations in 22 (20%) feet. The primary treatment was operative in 91 (83%) f

BACKGROUND: Unstable posterior pelvic ring fractures and dislocations are uncommon but potentially life-threatening injuries in children. Early definitive management reduces risk of immediate complications as well as chronic pain and gait dysfunction. Conventional operative therapy carries substantial risk of extensive blood loss and iatrogenic neurological and vascular injury. Minimally invasive image-guided intervention may further reduce immediate risk and improve long-term outcome.
OBJECTIVE: To describe CT-guided closed reduction and internal fixation (CRIF) and review outcomes of unstab

Transverse fracture-dislocations of the sacrum are rare. Associated lesions of the lumbosacral spine as well as neurological injuries are common. Conventional radiographs of the pelvis often fail to clearly visualize the fracture. Delayed diagnosis increases the risk of progressive neurological disfunction. True lateral sacral views and CT-scans with 3-dimensional reconstructions are very helpful in establishing the full extent of the injury. These examinations should be considered in all patients with a history of high energy trauma and clinical signs indicating lumbosacral injury, such as se

Isolated fractures of the capitate are uncommon. We report a rare case of isolated fracture of the capitate with dorsal dislocation of the proximal pole. After open reduction and K-wire fixation the fracture united, and a full range of wrist motion was achieved. No signs of avascular necrosis were observed after 3 years.

STUDY DESIGN: This is a retrospective single-patient case report with a literature review.
OBJECTIVE: To report on an uncommon injury: traumatic fracture-dislocation of the lumbosacral junction with delayed displacement.
SUMMARY OF BACKGROUND DATA: Only 31 cases of traumatic fracture-dislocation of the lumbosacral junction were found in the English literature. Only three previous reports refer to this injury occurring with a posterior displacement. This is the first report of this type of injury displacing in a delayed manner.
METHODS: Chart review.
RESULTS: Open reduction, Arbeitsgemeinsc

We describe 11 cases of posterior fracture-dislocation of the shoulder that required open reduction and fixation. Difficulties with access through anterior approaches led us to use the superior subacromial approach. This is an extension of the approach often used to expose the rotator cuff; the joint is opened by splitting the supraspinatus tendon 5 mm behind the cuff interval. The glenoid, proximal humerus and any fracture fragments can be seen from above, allowing reduction of the dislocation and osteosynthesis to be performed with minimal risk of damage to the humeral head and its blood sup

This report describes an extensively comminuted fracture of the sacrum and the L-5 vertebral body with complete anterior dislocation of the lumbar spine on the sacrum that occurred in a 19-year-old man ejected from a motor vehicle. Lumbosacral dissociation was characterized by computed tomography with three-dimensional and multiplanar reconstruction, and definitively managed by posterior instrumentation using locking hook spinal rods attached to the pelvis with hockey stick attachments by the Galveston technique. Implications of this case and a review of the literature are presented.

This report describes an unusual injury--complete acromioclavicular dislocation with midshaft clavicular fracture. The treatment decisions involved how best to treat a floating section of bone largely stripped of its soft tissue attachments. By a limited exposure, the acromioclavicular joint was reduced and pinned. The clavicle was then treated closed, obviating the need for additional soft tissue stripping and loss of blood supply. The successful outcome suggests this approach is effective in cases of this type.

Acute traumatic dislocation of the patella may be associated with osteochondral fractures. Clinical examination invariably shows a tense effusion. A detailed radiographic examination including antero-posterior, lateral and skyline views of the patella is usually necessary to establish an exact diagnosis. Once diagnosis is made open reduction and fixation of the osteochondral fracture should be carried out if it is possible. Out of 78 patients of our own with patella dislocation 24 cases suffered an osteochondral fracture. In ten cases refixation of the osteochondral fragment was achieved. Intr

A rare case of a posterior fracture-dislocation of the hip joint with an ipsilateral subcapital fracture in a young woman is presented. The isolated femoral head was displaced in the subcutaneous area in the buttock. A possible explanatory mechanism of dual injury is that the posterior fracture-dislocation occurred first and then a lateral blow to the buttock caused the subcapital fracture. The missing fragment of the femoral head, in two parts, was located and replaced with a prosthesis before the hip was reduced. An extensive search in the literature failed to reveal a similar case.
