Clinical Assessment of Prefrontal Lobe Functions.

PURPOSE OF REVIEW: Whereas it was previously thought that there was a single overarching frontal lobe syndrome, it is now clear that several distinct cognitive and behavioral processes are mediated by the frontal lobes. This article reviews these processes and the underlying neuroanatomy and provides an approach to the assessment of prefrontal lobe functions at the bedside. RECENT FINDINGS: Cognitive and behavioral frontal lobe functions are mediated by the prefrontal regions rather than the frontal lobes as a whole. At least five separate prefrontal functions have been defined: energization,…

Frontal stroke syndromes.

The frontal lobe is the largest lobe of the brain, and it is thus commonly involved in stroke. Moreover, almost one in five strokes is limited to the prerolandic areas. This high frequency of anatomical involvement is in sharp contrast with the apparent rarity of clinical frontal dysfunction in stroke. It is remarkable that frontal behavioral syndromes have been rather uncommonly reported in patients with stroke as compared to patients with other diseases, such as brain tumor. This fact is paradoxical, because an acute process (stroke) is expected to yield more clinical dysfunction than a more…

Reversible hyposmia caused by intracranial tumour.

Two patients with hyposmia caused by an intracranial tumour recovered olfactory functions after craniotomy. The first case was a 68-year-old male with a tumour metastasized from the lung to the right frontal lobe. The second case was a 75-year-old male with meningioma of the right frontal lobe. Results of T & T olfactometry and venous olfaction tests also indicated suspected central hyposmia. Magnetic resonance imaging (MRI) indicated compression of the frontal lobe by intracranial tumour. Pressure on the olfactory centre located in the frontal lobe produced hyposmia. Decompression of the fron…

Frontal glioblastoma multiforme may be biologically distinct from non-frontal and multilobar tumors.

Glioblastoma multiforme (GBM) is the most common primary brain tumor in adults and carries a grim prognosis. Lobar GBM, notably those localized to the frontal lobe, are generally more amenable to complete surgical resection, and may carry a better prognosis. The biology of differently localized GBM has been reported scarcely in terms of prognostic markers, including isocitrate dehydrogenase 1 (IDH1) mutation and O(6)-methylguanine-methyltransferase (MGMT) methylation. To our knowledge, there has been no evaluation in the literature of different proliferation indexes in different GBM locations …

Attention-deficit hyperactivity disorder (ADHD) and frontal-motor cortex disconnection.

Attention-Deficit Hyperactivity Disorder (ADHD) is a very common pediatric-neuropsychiatric entity of still unknown etiology, and is thus a topic of controversy. A neurological concept is presented in order to attain a better conceptualization of ADHD. This concept focuses on the frontal lobe as the inhibitor of excessive motor activity. This inhibitory function is presumed to be most active during childhood. The concept of a disturbed frontal-motor cortex connection was first developed in view of a completely different and rare disorder of childhood: Rett Syndrome (RS) with smallness of the f…

The association between White matter microstructure alterations detected by Diffusional kurtosis imaging in Neural circuit and post-stroke depression.

AIM: In order to study the mechanism of post-stroke depression (PSD), we used diffusion kurtosis imaging (DKI) to describe the changes in wh … he severity of PSD. CONCLUSION: Our research indicated that the damage to WM microstructure in the frontal lobe, temporal lobe and the genu of corpus callosum may be related toPSD. DKI explores the microstructural changes of WM in PSD patients and may be an auxiliary diagnostic method for PSD.

Morphological Description of Frontal EEG Interictal and Ictal Discharges in an Adult Cohort of 175 Patients.

Clinical and electroencephalogram (EEG) features in frontal lobe epilepsy (FLE) vary considerably among patients, making the diagnosis a challenge. The objective of this study was to describe interictal and ictal EEG activity, identifying variables that could help to differentiate and diagnose frontal lobe epilepsy cases. A prospective cross-sectional study from patients with frontal interictal epileptiform discharges (IED) referred to the Vall d'Hebron University Hospital (Barcelona, Spain) after a clinical event compatible with epileptic seizures was designed. The interictal and ictal activi…

Diffusion Kurtosis Imaging Detects Microstructural Changes in the Brain after Acute Alcohol Intoxication in Rats.

The aim of this study was to test the technical feasibility of diffusion kurtosis imaging (DKI) in the brain after acute alcohol intoxication. Diffusion tensor imaging (DTI) and DKI during 7.0 T MRI were performed in the frontal lobe and thalamus before and 30 min, 2 h, and 6 h after ethyl alcohol administration. Compared with controls, mean kurtosis values of the frontal lobe and thalamus first decreased by 44% and 38% within 30 min (p < 0.01 all) and then increased by 14% and 46% at 2 h (frontal lobe, p > 0.05; thalamus, p < 0.01) and by 29% and 68% at 6 h (frontal lobe, p < 0.05; thalamus, …

[An autopsied case of multiple system atrophy with remarkable cerebral atrophy].

In this paper, we report the pathological findings of a multiple system atrophy (MSA) patient with remarkable frontal lobe atrophy. The patient was a 65-year-old woman with a 13-year history of untreatable parkinsonism, dysautonomia and progressive motor aphasia. The cranial imaging study disclosed progressive atrophy of the frontal lobe, striatum and pontocerebellar system. She died of pneumonia. The brain weight was 810 g, and there was remarkable atrophy of the cerebrum predominantly in the frontal lobe, striatum, pons and cerebellum. The microscopic examination revealed a preserved cortica…

Experimental social training methods.

Impairments of social behavior constitute common symptoms of frontal lobe dysfunction and are frequent consequences of damage to the frontal lobe. In this chapter we define and describe social behavioral deficits that include mentalizing (e.g., theory of mind, empathy), social self-regulation, social self-awareness, and social problem solving, and discuss how intervention research might address these deficits. Three stages of neurologic illness are emphasized: the early recovery stage after frontal lobe damage, chronic recovery phases of recovery from frontal lobe damage, and progressive decli…

Sleep-related hyperkinetic seizures: always a frontal onset?

Hyperkinetic seizures are considered a typical manifestation of nocturnal frontal lobe epilepsy (NFLE). Patients with temporal lobe epilepsy with mainly sleep-related seizures have been described; however they commonly lack hyperkinetic activity and seizure frequency is low. We retrospectively analysed our population of 442 consecutive patients surgically treated between January 1996 and January 2004. Among these there were 25 patients with sleep-related hyperkinetic epileptic seizures, with a frontal lobe onset in 18 cases and a temporal lobe onset in 7. Patients with sleep-related hyperkinet…

Impaired grammar with normal fluency and phonology. Implications for Broca's aphasia.

Extensive testing of grammatical function, including assessment of spontaneous language, inflectional morphology, ability to produce grammatical structures, syntactic comprehension and grammatical judgement, was carried out in 2 patients with large dominant frontal lobe lesions, including but not confined to, the third frontal convolution. Both patients were fluent and had normal articulation and phonological production and neither was agrammatic, suggesting that even very large frontal lesions do not produce Broca's aphasia and that language cortex proper is confined to the postcentral perisy…

Epileptic seizures with reversible lesions in bilateral frontoparietal lobes: A case report and literature review.

Magnetic resonance imaging (MRI) is recommended for patients with epileptic seizures to rule out an underlying focal lesion. However, the radiological characteristics of epilepsy are not well elucidated. Transient periictal MRI abnormality (TPMA) refers to reversible MRI signal changes observed in epileptic patients. A 32-year-old man presented with a 2-week history of epileptic seizures, which initially manifested as focal aware seizures and progressed to a generalized tonic-clonic seizure on the third day. Electroencephalography showed sharp waves, sharp and slow wave complexes, and irregula

An analysis of neurocognitive dysfunction in brain tumors.

BACKGROUND: Neurocognitive dysfunction is an important issue in patients with frontal lobe lesions. These patients who may be in good neurological status may succumb to neurocognitive dysfunction, affecting their daily living and hampering the quality of life. This study aims to correlate pre- and post-operative neurocognitive dysfunction in patients with frontal lobe lesions. MATERIALS AND METHODS: A prospective analysis of 50 patients of newly-diagnosed frontal lobe tumors of any grade deemed suitable for surgical resection was carried out. All patients underwent neurocognitive testing usin…

Multilevel atlas comparisons reveal divergent evolution of the primate brain.

Whether the size of the prefrontal cortex (PFC) in humans is disproportionate when compared to other species is a persistent debate in evolu … nt in catarrhini when compared to other species through bootstrap analysis. Within the catarrhini's frontal lobe, the prefrontal cortex was the principal driver of frontal expansion. Across all species, expansion of the frontal lobe appeared to be systematically linked to the parietal lobe. Our findings suggest that the human frontal and parietal lobes are not …
