Early intestinal barrier changes in A53T transgenic Parkinson's disease mice.

Gut dysfunction commonly precedes motor symptoms in Parkinson's disease (PD), but the mechanistic sequence of gut versus brain pathology remains unclear. This work aimed to define the timing of intestinal barrier dysfunction relative to central nervous system (CNS) changes in the A53T α-synuclein transgenic mouse model of PD. Functional and molecular assessments of the gastrointestinal tract (ileum and colon) were conducted at 12 and 36 weeks. We measured in vivo and ex vivo intestinal permeability, nutrient absorption, histomorphology, goblet cell density, and expression of MUC2 and Claudin-1

Self-Nanoemulsifying Drug Delivery Systems for Cardiovascular Therapeutics: Formulation Strategies, Pharmacokinetic Enhancement and Precision-Oriented Translation.

Cardiovascular drugs face many barriers to oral delivery, and self-nanoemulsifying drug delivery systems (SNEDDS) are becoming a promising nano-enabled drug delivery vehicle to address these challenges and aid precision cardiovascular delivery. The poor aqueous solubility and/or intestinal permeability, combined with high intestinal and hepatic clearance and, in some cases, intestinal P-glycoprotein (P-gp) efflux, leads to variable systemic exposure and inconsistent therapeutic activity of many antihypertensive drugs, antianginal drugs, and cardioprotective agents. Under gastrointestinal dilut

Pharmacokinetics of asciminib in a patient with chronic myeloid leukemia undergoing maintenance hemodialysis.

Asciminib (ASC) is a novel BCR::ABL1 tyrosine kinase inhibitor that binds to the ABL myristoyl pocket. Although dose adjustments for renal i … sma concentrations. These findings suggest that clinicians should prioritize monitoring for delayed gastrointestinal absorption and potentially reduced bioavailability in patients undergoing maintenance hemodialysis, rather than focusing on drug removal by dialysis.

Preparation, Characterization, and In Vivo Evaluation of an Oral Triptolide Nanomatrix System for Rheumatoid Arthritis Therapy.

Background: Triptolide (TP), a principal bioactive component of Tripterygium wilfordii, exhibits potent anti-inflammatory activity. However, its application is still limited due to its poor solubility and systemic toxicity, primarily caused by uncontrolled absorption after oral administration. Our previously established oral nanomatrix system, composed mainly of commercially available nanoporous Sylysia and Eudragit®, can not only enhance the in vitro dissolution of poorly water-soluble drugs, but also modulate their absorption sites in gastrointestinal tract. Methods: We prepared a TP nanomat

Characterization of neonatal and infant enterostomy fluids.

The composition of gastrointestinal (GI) fluids is crucial for the dissolution, solubilization, and absorption of orally administered drugs. Disease- or age-related changes in GI fluid composition could significantly affect the pharmacokinetics of oral drugs. However, limited studies have been conducted on the characteristics of GI fluids in neonates and infants due to practical and ethical challenges. The current study collected enterostomy fluids from 21 neonate and infant patients over an extended period of time and from different regions of the small intestine and colon. The fluids were ch

Cancer-related hypercalcemia and potential treatments.

Cancer-related hypercalcemia is a common finding typically seen in patients with advanced cancer and occurs in about 20 to 30 percent of cases. The most common cause of hypercalcemia in hospitalized patients is hypercalcemia due to malignancy. This clinical problem is seen in patients with both solid tumors and patients with hematologic malignancies. Hypercalcemia is associated with a poor prognosis in oncology patients. This pathologic condition can occur due to many different mechanisms but is usually caused by abnormal calcium use resulting from bone resorption, intestinal absorption, or ren

Zwitterion-functionalized mesoporous silica nanoparticles for enhancing oral delivery of protein drugs by overcoming multiple gastrointestinal barriers.

Oral delivery of protein or peptide drugs confronts several barriers, the intestinal epithelium and the mucus barrier on the gastrointestinal tract is deemed to be the toughest obstacles. However, overcoming these two obstacles requires contradictory surface properties of a nanocarrier. In the present work, mesoporous silica nanoparticles (MSNs) were modified with deoxycholic acid (DC) and coated with sulfobetaine 12 (SB12) for the first time to achieve both improved mucus permeation and transepithelial absorption. MSNs modified with stearic acid and coated with dilauroylphosphatidylcholine (D

Complexes of myo-inositol-hexakisphosphate (InsP6) with zinc or lanthanum to enhance excretion of radioactive strontium from the body.

90Sr, which was released into the atmosphere and the ocean following the Chernobyl and Fukushima Daiichi nuclear power plant disasters, is an important nuclear fission element. Compounds that inhibit the absorption of 90Sr into the bloodstream and enhance its elimination can be beneficial in decreasing the absorbed radiation dose in people exposed to 90Sr. Recently, we prepared complexes of myo-inositol-hexakisphosphate (InsP6) with zinc or lanthanum as decorporation agents. These complexes, called Zn-InsP6 and La-InsP6 respectively, are insoluble in water and can potentially chelate additiona

Crushed dolutegravir/abacavir/lamivudine given via nasogastric tube in gastric outlet obstruction caused by cancer resulted in rapid viral load suppression.

Alternative modes of antiretroviral administration are sought for people with impaired intestinal passage and/or absorption. We present a case of late HIV diagnosis (CD4+ count 160 cells/µL) with gastric outlet obstruction due to stomach adenocarcinoma. Co-morbidities included oesophageal candidiasis, Helicobacter pylori-positive duodenal ulcers and cytomegalovirus duodenitis. The gastric outlet obstruction required total parenteral nutrition and parenteral medication during four weeks of diagnostic work-up leading to pyloric resection. Crushed dolutegravir, abacavir and lamivudine were admini

Bioinspired butyrate-functionalized nanovehicles for targeted oral delivery of biomacromolecular drugs.

Ligand-functionalization can increase the affinity of nanoparticles (NPs) with targeted cells. However, one major defect of ligands still exists in oral administration: limited receptor recognition. The hindrance of mucus network and deactivation of enzymes severely challenge the targeting efficiency of macromolecular ligands. Inspired by "molecular exchange" between intestinal microbiota and host cells, we anchored microbiota metabolite butyrate on classical "mucus-inert" polyethylene glycol (PEG) NPs. Butyrate has unique advantages of low molecule weight, high hydrophilicity and chemical sta

Effects of iron on the pharmacokinetics of paracetamol in saliva.

Paracetamol has been reported to chelate with iron. It was found that no in vitro reaction between ferrous ion and paracetamol. Other studies found that there is an aerobic (in the gastrointestinal tract) oxidation of ferrous ion to ferric ion caused in iron-paracetamol in vivo reactions. The objective of this study was to determine if iron interacts with paracetamol and reduces paracetamol absorption. A randomized, double-blind, cross-over study design was used to assess the in vivo interaction of paracetamol and ferrous ion. Paracetamol (1.0 g) was co-ignested alone or with (300 mg) ferrous

Thiamine pyrophosphate biosynthesis and transport in the nematode Caenorhabditis elegans.

Thiamine (vitamin B1) is required in the diet of animals, and thiamine deficiency leads to diseases such as beri-beri and the Wernicke-Korsakoff syndrome. Dietary thiamine (vitamin B1) consists mainly of thiamine pyrophosphate (TPP), which is transformed into thiamine by gastrointestinal phosphatases before absorption. It is believed that TPP itself cannot be transported across plasma membranes in significant amounts. We have identified a partial loss-of-function mutation in the Caenorhabditis elegans gene (tpk-1) that encodes thiamine pyrophosphokinase, which forms TPP from thiamine at the ex

Physiological importance of aquaporins: lessons from knockout mice.

The phenotype analysis of transgenic mice deficient in specific aquaporin water channels has provided new insights into the role of aquaporins in organ physiology. AQP1-deficient mice are polyuric and are unable to concentrate their urine in response to water deprivation or vasopressin administration. AQP1 deletion reduces osmotic water permeability in the proximal tubule, thin descending limb of Henle and vasa recta, resulting in defective proximal tubule fluid absorption and medullary countercurrent exchange. Mice lacking AQP3, a basolateral membrane water channel expressed mainly in the cor

Effect of experimental spinal cord injury on salicylate bioavailability after oral aspirin administration.

The purpose of the present work was to study whether spinal cord injury (SCI) alters salicylate bioavailability after oral aspirin administration. Female Sprague-Dawley rats were subjected to SCI at the T8 level by two procedures, contusion by the weight-drop method and severance by knife, and received a single oral aspirin dose (15 mg/kg) 24 h after injury. Blood samples were drawn and aspirin (ASA) and salicylic acid (SA) concentrations in whole blood were determined at selected times over a period of 240 min. Both SCI procedures produced similar alterations on salicylate bioavailability. AS

Personalised antiplatelet therapy during elective percutaneous coronary intervention in patients with chronic coronary syndrome: An international journal of cardiology expert consensus statement.

Oral P2Y12 inhibitors represent the mainstay therapy for the prevention of thrombotic complications in patients undergoing percutaneous coronary intervention (PCI) and coronary stent implantation. However, the onset of antiplatelet action of the oral P2Y12 inhibitors is affected by their need to be absorbed in the gastrointestinal tract before becoming systemically available. Following oral intake of P2Y12 inhibitors, the timeframe required for gastrointestinal absorption leads to a periprocedural period of less consistent platelet inhibition at the time of PCI during which patients are potent…
