Measurement of torsion angles of long finger bones using computed tomography.

OBJECTIVE: Rotational dislocation at the fracture site is a complication of long finger bone fractures of the metacarpals and phalanges. To evaluate such deformities, we performed CT of the articular surfaces of these bones to demonstrate the torsion angles. DESIGN: We evaluated 10 pairs of cadaver hands. These were placed flat, with the bones of interest perpendicular to the gantry to acquire axial images. The torsion of the long bone axes was defined as the angle between a tangent positioned parallel to the proximal articular surface and a tangent parallel to the distal articular surface of

Comparison of the fixation ability between lag screw and bone plate for oblique metacarpal shaft fracture.

BACKGROUND: For oblique metacarpal shaft fracture, if anatomical reduction is achieved through conservative cast immobilization rather than stable fixation, bone malrotation can easily occur, resulting in severe loss in hand prehensile function. However, whether bone plate fixation or only lag screw fixation is more preferable remains unclear. Few studies have evaluated whether screw fixation can provide biomechanical fixation strength similar to bone plate fixation. OBJECTIVE: We assessed the difference in fixation strength between fixtation with two lag screws and bone plate for oblique met

Radiologic evidence of symmetric and polyarticular monosodium urate crystal deposition in gout - A cluster pattern analysis of dual-energy CT.

OBJECTIVES: To determine the clustering patterns of monosodium urate (MSU) crystal deposition and bone erosions among patients with gout requiring urate-lowering therapy (ULT) using dual-energy CT (DECT). METHODS: DECT scans of bilateral hands/wrists, feet/ankles, and knees were obtained on 153 patients with gout on allopurinol ≥300 mg daily for ≥3 months. Two radiologists assessed the images at pre-specified sites (15 in the hands/wrists, 12 in the feet/ankles, 4 in the knees). Clustering patterns of MSU crystal deposition and bone erosions were evaluated. RESULTS: Among 153 patients with g

The Versatile Vascularized Second Metacarpal-Base Bone Graft.

Vascularized bone grafts of the wrist and hand help to achieve bone union in various clinical settings such as bone fusion or nonunion treatment. Among the multiple possible donor sites, the base of the second metacarpal is convenient because of its relatively straightforward harvesting, lack of impairment of the hand vascularization, sufficient bone supply for small joint fusion and phalanx or metacarpal nonunion management. The flap is versatile as it can reach any carpal and metacarpal bone as well as the proximal phalanx of all fingers. The arc of rotation of the flap depends on which side

The influence of parity on quantitative ultrasound evaluation of the calcaneus and hand phalanges in Polish postmenopausal women.

The aim of the retrospective study was to assess the influence of parity on the skeletal status in 760 postmenopausal women. Women with factors affecting their bone status, or those treated for osteoporosis, were not included. Quantitative ultrasound measurements were performed at the heel (Achilles, n = 465) or at hand phalanges (DBM Sonic 1200, n = 295). Regarding hand phalanges, Z score values were significantly lower in women with three, four, and five or six deliveries in comparison to nonparous women and in women with four deliveries versus women with one or two deliveries. Calcaneus ult

Brachydactyly in 14 genetically characterized pseudohypoparathyroidism type Ia patients.

Brachydactyly, classically described as shortening of III, IV, and V metacarpals and I distal phalanx, is the typical and most specific sign … ween hand bone segments. Between subjects, shortening ranged from -2 to -10.4 SDS and involved 1-19 hand bones (5.3-100%). Between segments, III-IV metacarpals were the most compromised (-10.4 and -10.0 SDS, respectively); V metacarpals and I-IV distal phalanges were the most frequently shortened (85.7%). Overall, bone length median values revealed shortenin …

[Transtrapezoid carpal dislocation: a case report].

Fracture dislocations of the STT joint are extremely rare. We present the case of a 50-year-old worker, who sustained a crush injury of his left hand with a dorsal fracture dislocation of the trapezoid bone and the associated second metacarpal bone at the STT, and a concomitant compartment syndrome of the hand. The fracture was reduced immediately and compartment release was performed, followed by percutaneous stabilisation using three K-wires. Material removal after six weeks was followed by intensive physical therapy. At the final follow-up examination after three months, we observed a good

Osteolytic lesions of hand and feet: a seven-year experience from a tertiary referral centre of North India.

There are 106 bones in hands and feet but their lesions are not commonly reported. This was a retrospective study of all osteolytic lesions involving bones of the hands or feet presenting to the only tertiary referral centre of the north Indian state of Uttarakhand during the 7-year period from January 2006 to December 2012. A compilation of the various demographic, clinical, radiological and histopathological findings was made. Of the 52 lesions encountered in the 7-year record, 75% were asymptomatic. 20 (38.4%) were benign tumours, 20 (38.4%) tumour-like lesions, 9 (17.3%) inflammatory and p

[The hand in hematologic diseases].

Radiography of the hand often reflects the picture of generalized diseases, affecting both the muscolo-skeletal system and the others. Some of the most common hematologic disorders may be detected in roentgenograms of the hand, especially in the anemias, but also in plasma cell dyscrasias and proliferative malignant diseases (i.e., lymphomas and leukemias). On the basis of their experience, the authors have reviewed and discussed the radiographic "pattern" of the hand in several hematologic conditions (i.e., anemias; thalassemias; sickle-cell disease; lymphomas; multiple myeloma; etc.), and th

Vascularized Bone Flaps for Carpal Reconstruction.

Within the field of hand and upper extremity surgery, reconstruction of the bony carpus remains a perplexing task and is a field undergoing rapid evolution. Among the eight bones of the carpus, the scaphoid and lunate are most frequently affected by traumatic and avascular processes which render their articular surfaces degenerated and painful. These conditions include scaphoid waist fracture, scaphoid proximal pole fracture, and Kienböck's disease of the lunate. While traditional salvage operations with limited functional outcomes have historically been employed for management of these unsolv

[Three-dimensional planing and correction of osteotomies in the forearm and the hand].

New 3-dimensional supported technologies considerably improved accuracy of corrective osteotomies and allowed alignments of cuts not possible before. The introduction of simply applicable software made it possible also for not in informatics educated persons to segment CT data, generate 3D models of bones and compare them with the mirrored opposite side. Only this approach allows an exact analysis of deformation, the creation of an operation plan and the virtual performance of the procedure in 3D. The production of individualized drilling and sawing guides which fit defined and unique to the b

Perforator-based forearm island flap.

Reconstruction of soft tissue defects of the hand with exposed tendons, nerves, joints and bones could be quite a challenge for the surgeon. Only adequate coverage can save their functions. In our case we present a patient with adduction contracture of the right thumb after II degree burns, treated with perforator-based forearm flap. This flap relies only on perforating branches (5 to 11 in number) arising from the radial artery and located proximal to the radial styloid (2 to 7 cm). The main advantages of this method are that it is a single-stage operative technique, it is applicable in emerg

'Punch' actinomycosis causing osteomyelitis of the hand.

Osseous actinomycosis usually results from direct invasion of bone from adjacent soft-tissue infection. Involvement of the jaw and vertebral column has been frequently reported, but involvement of the bones of the wrist or hand is rare. A patient with osteomyelitis of the distal right first metacarpal bone due to Actinomyces israelii following a punch injury during fisticuffs is described. Review of similar cases revealed a striking association of this type of infection with punch injuries; hence the term, punch actinomycosis. Analysis of the pus in these infections typically reveals sulfur gr

[Informational value of weak-radiation immersion radiography of the hand in hormonal and metabolic osteopathies].

The early detection of changes in the bones of the hand in hormonal and metabolic osteopathies is possible with the aid of soft tissue immersion radiographic technique combined with "microradioscopy". The informational value of this special method of investigation will be demonstrated by several examples from the 286 examinations of accentuated osteoporotic and osteomalacic and hyperparathyroidism changes of the skeleton. Structural changes in the normally mineralized and poorly mineralized bone as well as the neighbouring soft tissue (joint capsule, tendons, subdermal fatty tissue) and the sk

Rheumatoid Hand and Other Hand-deforming Rheumatic Conditions.

The hand and wrist are commonly involved in rheumatic conditions, particularly rheumatoid arthritis and other systemic connective tissue diseases. With spondyloarthritis, hand and wrist involvement frequently occurs in psoriatic arthritis but generally does not occur in the remaining subtypes. The hand and wrist may also be affected in various metabolic and endocrine diseases, but these lie beyond the scope of this review.Radiographs may demonstrate the presence of joint space narrowing, bone loss, cysts and erosions, malalignments, and osteolysis. They may also show regions of soft tissue swe

Microcrack surface density in the human otic capsule: An unbiased stereological quantification.

Bone is continuously remodeled to repair and strengthen degenerative bone with accumulating dead osteocytes and microfractures. Inner ear osteoprotegerin (OPG)-mediated inhibition of otic capsular bone remodeling causes excessive perilabyrinthine bone degeneration. Consequently, microcracks accumulate around the inner ear. Microcracks cause osteocyte apoptosis and may disrupt the canalicular network connecting osteocytes. Despite their linear microscopic appearance, microcracks are three-dimensional disruption planes and represent surface areas inside a tissue space. With an elevated microcrac

Cement spacers with antibiotics for the treatment of posttraumatic infected nonunions and bone defects of the upper extremity.

Treatment of patients with posttraumatic infected nonunions or highly contaminated open fractures with segmental bone loss of the long bones of the upper extremity is demanding. The use of a 2-stage reconstruction technique, being the first stage characterized by thorough debridement, copious lavage, soft tissue coverage, and placement of a cement spacer with antibiotics at the infected site, and the second stage by cement spacer removal, internal fixation, and placement of bone graft with local antibiotics, is presented. We carried out this technique in 20 cases, in 12 cases the cement was mo

Asymmetrical Damage of the Wrist Joint Induces Lateralized Cortical Bone Loss in the Metacarpal Diaphysis in Patients with Rheumatoid Arthritis.

Background/Objectives: Osteoporosis is common in rheumatoid arthritis (RA), occurring either systemically or locally around inflamed joints. Decreased metacarpal bone density is a known marker of RA progression and hand function impairment. Although RA is generally characterized by symmetrical arthritis, some patients exhibit asymmetrical joint involvement. This study investigates the frequency of unilateral metacarpal bone density reduction in RA patients and aims to identify associated factors. Methods: This study included 143 RA patients (107 females, mean age 62.4 yrs., mean disease durati
