Effect of physiologic and pharmacologic adrenergic stimulation on heart rate variability.

OBJECTIVES: The aim of this study was to evaluate and compare the effects of physiologic and pharmacologic sympathetic stimulation on time and frequency domain indexes of heart rate variability. BACKGROUND: Measurements of heart rate variability have been used as indexes of sympathetic tone. To date, the effects of circulating catecholamines on heart rate variability have not been evaluated. METHODS: Fourteen normal subjects (eight men, six women, mean [+/- SD] age 28.5 +/- 4.8 years) were evaluated. Five-minute electrocardiographic recordings were obtained in triplicate after physiologic an…

Autonomic regulation of heart rate and blood pressure in hemorrhaged toads.

Mean arterial blood pressure (P(a)) and heart rate (HR) were studied in normal conscious cane toads (Bufo marinus) or in autonomically blocked animals treated variously with bretylium, atropine, phentolamine, and propranolol. Partial or total autonomic blockade had no effect on resting values of the variables tested, with the exception that atropine treatment of bretylium-treated toads caused an increase of heart rate by 5.3 +/- 1.3 beats/min. In graded hemorrhage, aliquots of blood (1.68 ml/100 g body wt) were removed at 60-min intervals. There was extensive plasma recruitment, and blood coul…

Enhanced heart rate response to haemorrhage by ileus in the pig.

Effects of ileus on cardiovascular and hormonal responses to haemorrhage were evaluated in 10 anaesthetized pigs. Ileus was induced and the bleeding sequence repeated on the following day. Before ileus, a resting heart rate (HR) of 105 (range 83-140) beats min-1 remained unchanged until haemorrhage amounted to 15% of the blood volume. With a blood loss of 28% HR increased to a maximum of 162 (126-245) beats min-1. In contrast, ileus increased HR upon initiation of haemorrhage from a similar resting value to a maximum of 200 (152-250) beats min-1 (P < 0.0005). There were neither statistical dif…

Characterization of the neonatal heart rate baroreflex during and after ECMO.

Prolonged continuous blood pressure (BP) and heart rate (HR) recordings from neonates of 35 to 42 weeks gestation were studied during and after ECMO. Data segments with significant deviation of BP were extracted for further analysis. The simultaneous changes in BP and HR were compared and the slope of the regression determined baroreflex sensitivity (BRS). Of 464 BP deviations, 98% produced curves with a negative slope consistent with the presence of a baroreflex. The average BRS was -1.0 +/- 0.8 bpm/mmHg (mean +/- S.D.) and curves were steeper during rising BP than falling BP (-1.1 +/- 0.9 be…

Xamoterol in hypertrophic cardiomyopathy: effects on diastolic function and heart rate.

The aims of this study were to determine the effects of the partial beta 1-adrenergic agonist, xamoterol, on diastolic function and ambulatory heart rate in hypertrophic cardiomyopathy. Eleven patients with non-obstructive hypertrophic cardiomyopathy were studied with cross-sectional and Doppler echocardiography and 24-h Holter monitoring before and after a single intravenous dose of xamoterol. Resting heart rate (mean +/- SD) increased from 76 +/- 16 before, to 83 +/- 15 beats/min 15 min after xamoterol, p = 0.03. In the 4-h period after xamoterol, maximum heart rate was reduced (127 +/- 21 t…

Adenosine use in pregnancy: lack of effect on fetal heart rate.

The treatment for supraventricular tachycardia in pregnancy is somewhat controversial. Although a variety of medications have been used to terminate this rhythm during pregnancy, all have actual or theoretical drawbacks. Adenosine is a relatively new medication with an extremely short half-life and is effective in the treatment of supraventricular tachycardia. We report a case in which this medication was used successfully during pregnancy. In addition, we found that adenosine had no effect on fetal heart rate in this case.

Postterm pregnancy: computer analysis of the antepartum fetal heart rate patterns.

The purpose of this study was to establish reference ranges for numerical fetal heart rate (FHR) data in postterm pregnancy and to compare them with the patterns of fetuses under undisturbed condition at term. FHR was analysed on-line by Sonicaid Computer System 8000. A statistically significant decrease in the number of accelerations and decrease of variation in postterm pregnancy was observed. The duration of high variation (high episodes) in the 42nd week of gestation was statistically lower than in the pregnancy at term. These observations should be taken into account by clinicians in the …

Effect of coronary arterial occlusion on vagal control of heart rate.

Diminished variation in heart rate as a sign of impaired vagal control is common in coronary arterial disease. To evaluate the effect of short-term myocardial ischaemia induced by coronary arterial occlusion during therapeutic percutaneous transluminal coronary angioplasty we measured the variation in heart rate during controlled deep breathing in 50 patients before and during arterial occlusion. Variation in heart rate diminished from 11.1 +/- 4.5 to 9.5 +/- 5.1 beats/min (P less than 0.01) during occlusion. No change occurred in heart rate, blood pressure or levels of noradrenaline and adren…

Time of night effects on heart rate variation in normal neonates.

Circadian patterns have been observed in infants as early as the first few postnatal days. We hypothesized that, in each sleep-waking state, heart rate variation in several distinct frequency bands would show consistent variations across a night in newborn infants. Twelve-hour night-time recordings of EEG, ECG, EOG, digastric EMG, respiratory movements, and CO2 were obtained from 25 normal full-term infants at 2-7 days postnatal age. The extents of three types of heart rate variation were determined for all epochs identified as quiet sleep, rapid eye movement (REM) sleep, and waking during eac…

Differential effects of right unilateral and bilateral ECT on heart rate.

In a series of 11 melancholic patients who received both right unilateral and bilateral ECT during a single course of treatment, mean and minimum postictal heart rates were greater after bilateral ECT. Greater catecholamine release during bilateral ECT is the most likely explanation for these effects. These differences complement other evidence that bilateral ECT has a more pronounced physiological impact than unilateral ECT.

Autonomic control of heart rate during dynamic exercise in human hyperthyroidism.

1. The relative contribution of the sympathetic and parasympathetic components of the autonomic nervous system to the regulation of the chronotropic response to dynamic exercise was evaluated indirectly in nine patients with thyrotoxicosis and in seven normal volunteers. All subjects were women, with equivalent ages in both groups. Six of the nine patients with hyperthyroidism were reevaluated after clinical compensation of the disease with propylthiouracil. 2. Heart rate responses were evaluated during discontinuous dynamic effort maintained for 4 min on a bicycle ergometer at levels of 5, 15…

Ventricular rhythm with intermediate rate in the neonate without heart disease.

Nonsustained, sustained, or incessant AVR, a form of VRIR, although infrequent, can present in the neonatal period. It must be considered in the differential diagnosis of wide QRS tachycardia. It seems to be different from VI, and it is asymptomatic and self-limited. It has good long-term prognosis and requires no treatment. These patients should not undergo invasive diagnostic evaluation unless symptoms develop.

Determinants of fetal heart rate response to vibroacoustic stimulation in labor.

To determine whether the fetal heart accelerates in response to a sound stimulus in labor, 40 women at various stages of labor were chosen at random to receive either a vibroacoustic stimulus or sham stimulus over the fetal head. Subsequent fetal heart rate (FHR) accelerations occurred to a significantly greater extent in study patients. One hundred thirty-two high- and low-risk patients were studied to determine correlations between the acceleration response and other maternal and fetal variables. There was a statistically significant negative correlation between the heart rate response to st…

Systolic time interval. Heart rate relationship during atrial or ventricular pacing.

In order to evaluate the relative importance of heart rate (HR) and A-V synchrony during pacing, 16 patients with VVI pacemakers were compared with 8 patients with AAI pacemakers using systolic time intervals. The patients were examined at rest at increasing HR with increments of 10 beats/min until a maximum of 150 beats/min was reached or for AAI pacing until 2 degrees A-V block occurred. The slopes of the linear regression equations for PEP versus HR and PEP/LVET versus HR were used. To compensate for differences in baseline values, delta PEP% versus delta HR% and delta PEP/LVET% versus delt…

The effect of maternal hypocalcemia on fetal heart rate baseline variability.

Marked decrease in fetal heart rate variability during labor was observed in a term fetus, borne by a mother with severe hypocalcemia due to idiopathic hypoparathyroidism and nutritional vitamin D deficiency. During the period of decreased baseline variability, fetal scalp pH was normal. A marked increase in fetal heart rate variability was observed within 5 minutes of intravenous administration of 1 gram calcium gluconate to the parturient. It seems that the recognition of the effect of maternal hypocalcemia on baseline fetal heart rate variability is important for accurate interpretation of …

Tonic heart rate level, social class and antisocial behaviour in adolescents.

On the basis of previous empirical research it was hypothesised that (a) antisocial behaviour in adolescence would be characterised by lower tonic heart rate levels and (b) any such relationship would be particularly borne out in the higher social classes where the 'social push' towards antisociality may be relatively weaker. These predictions were tested by relating tonic heart rate levels in a sample of 15 year old male schoolchildren to self-report and teacher ratings of antisocial behaviour/undersocialization. An 'antisocial' group was found to have significantly lower heart rate levels th…
