The Timing of Total Hip Arthroplasty After Intraarticular Hip Injection Affects Postoperative Infection Risk.

BACKGROUND: The data regarding any association between preoperative intraarticular steroid injection and risk ofperiprosthetic joint infection (PJI) after total hip arthroplasty (THA) are conflicting. The goal of the present study is to evaluate the association of preoperative intraarticular hip injection before THA on the incidence of postoperative PJI. METHODS: A national database was queried for patients who underwent THA and those patients who underwent prior ipsilateral hip injection. Three cohorts were created: THA within 3 months of ipsilateral hip injection (n = 829), THA between 3 an…

Hip capsulolabral spacer placement for the treatment of severe capsulolabral adhesions after hip arthroscopy.

Recently, there has been a rapid increase in the number of hip arthroscopies performed. The increase in the number of primary surgeries has been associated with a similar increase in the number of revision procedures. The most frequent indications for revision hip arthroscopy are residual bony deformity (impingement), persistent labral pathology, and intra-articular adhesions. Our current understanding of capsulolabral adhesions is limited. Although adhesions between the capsule and labrum are common after hip arthroscopy, generally, they are mild and asymptomatic. However, in severe cases the…

MRI evaluation of hip containment and congruency after closed reduction in congenital hip dislocation.

BACKGROUND: Developmental dysplasia of the hip is the most common congenital skeletal disease. In its most severe form--dislocation--the treatment is directed at reducing the hip and establishing normal congruency between the femoral head and the acetabulum. Closed reduction with casting is a common primary treatment, where reduction is confirmed by magnetic resonance imaging (MRI). OBJECTIVE: This study analyses anatomical aspects depicted on MRI after closed reduction to identify disparities in the growth behaviour of dislocated hips. MATERIALS AND METHODS: In 38 patients MRI after closed …

Primary total hip arthroplasty with a spongy metal surface acetabular component for hip dysplasia.

We evaluated the clinical and radiographic results of primary total hip arthroplasty for 81 dysplastic hips (71 patients) using cementless Spongiosa Metal II cups (ESKA Implants, Lübeck, Germany). The mean follow-up period was 6.4 years (minimum 5 years), and the preoperative mean Japanese Orthopaedic Association hip score had improved from 45.2 to 87.4 points at the latest follow-up. The radiographic outcome was no aseptic loosening in all 81 hips. The hip center was located significantly more superior than in the contralateral normal hip in 45 patients, but the difference was less than 10 mm…

Recovery expectations of hip resurfacing compared to total hip arthroplasty: a matched pairs study.

OBJECTIVE: Expectations of higher activity levels associated with hip resurfacing arthroplasty (HRA) may be driving better outcomes in this group compared to total hip arthroplasty (THA). Previous studies evaluated patient expectations before consulting with the surgeon, although these expectations were likely unrealistic and would change after the consultation. We compared HRA and THA patient expectations after consultation with the surgeon. METHODS: In a prospective registry setting, patients awaiting HRA were matched to THA patients by age, sex, and a preoperative Lower Extremity Activity …

Total hip arthroplasty after prior surgical treatment of hip fracture is it always challenging?

Salvage total hip arthroplasty (THA) presents a viable solution for failed open reduction internal fixation. This study compares salvage THA in patients with prior femoral neck fractures vs patients with prior intertrochanteric fractures. One hundred fifty-four hips in 152 patients underwent conversion from open reduction internal fixation to THA. Eighty-three patients had previous femoral neck fractures, and 69 patients (71 hips) had prior intertrochanteric fractures. Salvage THA in patients with prior intertrochanteric fractures presented a more technically demanding procedure with longer op…

Comparison of hip reduction using magnetic resonance imaging or computed tomography in hip dysplasia.

BACKGROUND: The purpose of this paper was to compare the use of computed tomography (CT) versus magnetic resonance imaging (MRI) to evaluate hip reduction in patients with dysplasia of the hip. METHODS: A retrospective review of postoperative pelvic CT and MRI in patients <13 months of age with hip dysplasia was performed. Scanner time, anesthesia requirement, cost, and radiation dosage were recorded. Hips were classified as dislocated, subluxated, or reduced. Sensitivity and specificity of CT and MRI were calculated. The outcomes of the subluxated hips were followed. RESULTS: Thirty-two CT …

Correct positioning for hip radiographs allows reliable measurement of hip displacement in cerebral palsy.

AIM: The pelvic radiograph in children with cerebral palsy (CP) can inform the degree of hip displacement by calculation of the migration percentage. However, concerns have arisen about the reliability of this measurement. The present study examined the reliability of radiographic assessment of displacement and the importance of positioning and reporting experience. METHOD: Two pelvic radiographs, taken at least an hour apart, were performed in 20 children (total 40 hips) in the standard position by a trained paediatric radiographer. Children (13 males, seven females) were aged 30 months to 1…

Gait analysis in adults with severe hip dysplasia before and after total hip arthroplasty.

Anatomical reconstruction of high riding hips by total hip arthroplasty (THA) and subtrochanteric shortening osteotomy aims to normalise gait pattern and improve functional hip scores. We present the medium-term clinical results of a group of patients with high riding dislocated hips in whom a cementless THA and subtrochanteric shortening osteotomy had been performed. We compared them with their preoperative status, with patients who had undergone a cementless THA for primary osteoarthritis, and also with a group of healthy gender and age-matched controls. Prospective computerized, three-dimen…

[Primary abductor hip contracture as diagnostic, prognostic and therapeutic problem in child hip pathology].

Coxa obliqua represents a special functional entity in the pathology of the child hip. Authors have confirmed the results of S.L. Weissman and B. Strinovic which claimed that the abductor contracture of the hip was a primary congenital condition that developed as a result of intrauterine malposition, leading later to the contralateral adductor contracture. Critical period for the development of complications was between 6 and 8 month after birth, adductor contracture might keep persisting together with the development of acetabular dysplasia, and later on with ipsilateral subluxation. This mal…

Higher peak contact hip stress predetermines the side of hip involved in idiopathic osteoarthritis.

BACKGROUND: Biomechanical parameters of the hip have been suggested to have an important influence on the development of osteoarthritis. We aimed to find out whether higher stress is generated in a hip that subsequently results in earlier hip arthroplasty compared to the contralateral hip in the same subject. METHODS: Standard anterior-posterior pelvic radiographs with no or subtle radiological signs of hip osteoarthritis, of 59 female patients, who underwent hip arthroplasty for primary osteoarthritis years later, were selected from the archives. For each subject peak contact hip stress of t…

Weight bearing after hip fracture: a prospective series of 596 geriatric hip fracture patients.

Five hundred ninety-six patients age > or = 65 with femoral neck or intertrochanteric fractures were allowed immediate unrestricted weight bearing after surgery and were prospectively followed. Follow-up data and hospital records were examined to identify those patients who required additional hip surgery owing to failure of fixation, nonunion, osteonecrosis, or prosthetic dislocation. Four hundred seventy-three patients were available for 1-year minimum follow-up; 16 patients (3.4%) required additional hip surgery. The revision surgery rate after intertrochanteric fracture due to loss of fixa…

The pelvic femoral angle in flexed-seated position analysis predicts impingement risk of total hip arthroplasty: Defining hip joint hypermobility and true hip hyperflexion.

BACKGROUND: Although total hip arthroplasty (THA) is highly successful, standard cup orientation does not prevent all impingement-related complications. Functional planning integrates spinopelvic motion, but flexed-seated definitions of hip hypermobility remain unclear. We aimed to define clinically relevant flexed-seated femur-pelvis motion thresholds and test their association with simulated impingement and anteversion safe-zone constraints. HYPOTHESIS: The existence of a pathological hip mobility threshold, independent of other risk factors for impingement with standard cup positioning, wo…

Hip fracture predicts subsequent hip fracture: a retrospective observational study to support a call to early hip fracture prevention efforts in post-fracture patients.

UNLABELLED: In this real-world retrospective cohort, subsequent hip fracture occurred in one in four patients with any initial fracture, most often after hip fracture, on average within 1.5 years. These data support the need for early post-fracture interventions to help reduce imminent hip fracture risk and high societal and humanistic costs. PURPOSE: This large retrospective cohort study aimed to provide hip fracture data, in the context of other fractures, to help inform efforts related to hip fracture prevention focusing on post-fracture patients. METHODS: A cohort of 115,776 patients (72…
