Taking care of systemic sclerosis patients during COVID-19 pandemic: rethink the clinical activity.

COVID-19 outbreak has quickly spread worldwide, causing a high pressure on the health-care system. In Italy, from March 8, 2020, all the deferrable clinical activities have been suspended to increase the health care offer for COVID-19 patients. The hospital organization has been modified also in order to assure non-COVID-19 patients assistance. The Scleroderma Unit of ASST Pini-CTO Hospital, in Milan, in the region mostly hit by SARS-CoV-2 in Italy, follows more than 600 patients affected by systemic sclerosis (SSc). Patients with SSc need a close follow-up with a regular screening of organ in

Digital Health and Community-Based Care for Diabetic Foot Prevention and Management in Resource-Limited Settings: A Narrative Review.

Diabetic foot disease is a major cause of preventable disability, hospitalization, and lower-extremity amputation worldwide, with a disproportionate burden in resource-limited settings. Although major amputation rates have declined in high-income countries through multidisciplinary care, many resource-limited settings still face delayed presentation, fragmented services, limited specialist access, and weak financial protection. Previous reviews have often examined diabetic foot care, digital health, or community interventions separately, with limited attention to how these approaches can be in

Community-based management of gastrointestinal symptoms at end-of-life in scleroderma using subcutaneous medication: a case report.

BACKGROUND: Systemic sclerosis (SSc) is a rare autoimmune connective tissue disorder often associated with severe gastrointestinal symptoms. At the end of life, managing these symptoms in the community can be challenging, particularly when patients are unable to take oral medications and wish to avoid hospital admission. This case describes the use of subcutaneous omeprazole in a community setting to manage refractory upper gastrointestinal symptoms in a patient with SSc during the final days of life. CASE DESCRIPTION: A 51-year-old patient with advanced SSc experienced significant upper gast

Analysis of low resource setting referral pathways to improve coordination and evidence-based services for maternal and child health in Ethiopia.

BACKGROUND: In low-resource settings, patient referral to a hospital is an essential part of the primary health care system. However, there is a paucity of study to explore the challenges and quality of referral coordination and communication. OBJECTIVE: The purpose of this research was to analyze the existing paper-based referral registration logbook for maternal and child health in general and women of reproductive age in particular, to improve referral coordination and evidence-based services in Low-Resource Settings. METHODS: This study analyzed the existing paper-based referral registra

Implementation of a Technology-Based Mobile Obstetric Referral Emergency System (MORES): Qualitative Assessment of Health Workers in Rural Liberia.

BACKGROUND: Maternal mortality remains a persistent challenge in low- and middle-income countries, where evidence-based interventions of obstetric triage and prehospital communication remain sparse. There is limited implementation evidence for technology-based approaches to improve obstetric care in such contexts. Liberia struggles with maternal mortality, particularly in rural areas where deaths are attributable to delays from absent triage and interfacility communication. We implemented a Mobile Obstetric Referral Emergency System (MORES) in rural Bong County to improve prehospital transfer,

Reducing HIV- and TB-Stigma among healthcare co-workers in South Africa: Results of a cluster randomised trial.

RATIONALE: The HIV and TB co-epidemic has a severe impact on the South African healthcare workforce and health system. HIV- and TB- stigma directed from healthcare workers (HCWs) towards colleagues not only has a negative impact on the mental health and well-being of the HCWs, but has been identified as a barrier to their own health-seeking behaviour. It also increases the strain on the health system due to absenteeism. OBJECTIVE: This cluster-randomised trial tested an intervention to reduce HIV- and TB-stigma among HCWs. The intervention, based on the theory of Diffusion of Innovations cons

Service readiness of Public and private health facilities for roll-out of hepatitis C screening and management: a cross-sectional study.

BACKGROUND: Direct-acting antiviral (DAA) therapy has transformed hepatitis C management by enabling cost-effective outpatient treatment. Pakistan, with one of the world’s highest hepatitis C burdens, has set ambitious policy targets for the rollout of DAA therapy supported by rapid screening and confirmatory testing. This study assessed the service readiness of public and private healthcare facilities to inform future scale-up of hepatitis C services in Pakistan. METHODS: Between June 2021 and January 2022, a purposive sample of 39 facilities was selected from 14 high-burden districts in cons

High HIV and active tuberculosis prevalence and increased mortality risk in adults with symptoms of TB: a systematic review and meta-analyses.

INTRODUCTION: HIV and tuberculosis (TB) remain leading causes of preventable death in low- and middle-income countries (LMICs). The World Health Organization (WHO) recommends HIV testing for all individuals with TB symptoms, but implementation has been suboptimal. We conducted a systematic literature review and meta-analyses to estimate HIV and TB prevalence, and short-term (two to six months) mortality, among adults with TB symptoms at community- and facility level. METHODS: We searched Embase, Global Health and MEDLINE databases, and reviewed conference abstracts for studies reporting simul

Esophageal lung misdiagnosed as tracheoesophageal fistula: A case report.

An esophageal lung is a subtype of communicating bronchopulmonary foregut malformation (CBPFM) where the lung, often right side, communicates with the esophagus, which causes hypoplastic and consolidated diseased lung, and is usually diagnosed late in its clinical course. Clinical suspicion based on patient history, signs, and symptoms should lead to this opinion. A chest CT scan combined with esophagography is highly recommended for suspicious cases. Here, we describe the case of a 3-month-old female infant who was referred to our hospital because of respiratory distress. The diagnosis of the

Community care for people who are homeless and mentally ill.

This qualitative longitudinal study documents the experiences of 60 people who are homeless and mentally ill from their state mental hospital discharge through their first two years in community housing. The study explores the personal, cultural, and environmental contexts of life for adults who are homeless and mentally ill and examines the interaction between an individual's needs and community resources. The research identifies forces that perpetuate homelessness and traces the struggles that people who are homeless and mentally ill encounter during the transition from the streets to stable

The mentally ill in jails and prisons: towards an integrated model of prevention.

Jails and prisons have become a final destination for persons with severe mental illness in America. Addiction, homelessness, and fragmentation of services have contributed to the problem, and have underscored the need for new models of service delivery. Project Link is a university-led consortium of five community agencies in Monroe County, New York that spans healthcare, social service and criminal justice systems. The program features a mobile treatment team with a forensic psychiatrist, a dual diagnosis treatment residence, and culturally competent staff. This paper discusses the importanc

Rapid Sentinel Surveillance for COVID-19 - Santa Clara County, California, March 2020.

On February 27, 2020, the Santa Clara County Public Health Department (SCCPHD) identified its first case of coronavirus disease 2019 (COVID-19) associated with probable community transmission (i.e., infection among persons without a known exposure by travel or close contact with a patient with confirmed COVID-19). At the time the investigation began, testing guidance recommended focusing on persons with clinical findings of lower respiratory illness and travel to an affected area or an epidemiologic link to a laboratory-confirmed COVID-19 case, or on persons hospitalized for severe respiratory

From Tragedy To Opportunity: Hospital-based Violence Intervention Programs May Address Root-Cause Health Disparities for Violent Traumatic Injury Patients.

Violence is a growing public health issue that disproportionately affects low-income and racialized communities across North America. While trauma centers appropriately respond to acute violent injuries, many patients are discharged back into the same environments that put them at risk. Hospital-based Violence Intervention Programs (HVIPs) offer opportunities for trauma care to address the upstream root causes of violence. These programs engage patients during critical "teachable moments," connecting them to staff with lived experience, along with social workers and community partners, providi

Healthcare-Associated Legionnaires' Disease, Europe, 2008-2017.

Healthcare-associated Legionnaires' disease (HCA LD) can cause nosocomial outbreaks with high death rates. We compared community-acquired LD cases with HCA LD cases in Europe during 2008-2017 using data from The European Surveillance System. A total of 29 countries reported 40,411 community-acquired and 4,315 HCA LD cases. Of the HCA LD cases, 2,937 (68.1%) were hospital-acquired and 1,378 (31.9%) were linked to other healthcare facilities. The odds of having HCA LD were higher for women, children and persons <20 years of age, and persons >60 years of age. Out of the cases caused by Legionella

Observational study of telephone consults by stroke experts supporting community tissue plasminogen activator delivery.

OBJECTIVES: Barriers to intravenous (IV) tissue plasminogen activator (tPA) use in ischemic stroke include limited treatment experience of community physicians. Models of acute stroke care have been designed to address these limitations by providing community support. These include support by telephone or televideo, with or without subsequent transport to tertiary care centers. The authors describe the frequency, characteristics, and effect of community phone consultations to a 24/7 stroke "hotline" staffed by stroke physicians at an academic stroke center using such a model. METHODS: Twelve

Racial and ethnic disparities in emergency department-initiated buprenorphine across five health care systems.

BACKGROUND: Opioid overdose deaths have disproportionately impacted Black and Hispanic populations, in part due to disparities in treatment access. Emergency departments (EDs) serve as a resource for patients with opioid use disorder (OUD), many of whom have difficulty accessing outpatient addiction programs. However, inequities in ED treatment for OUD remain poorly understood. METHODS: This secondary analysis examined racial and ethnic differences in buprenorphine access using data from EMBED, a study of 21 EDs across five health care systems evaluating a clinical decision support system for

Impact of Interhospital Transfer on Outcomes in Acute Pancreatitis: Implications for Healthcare Quality.

Background/Objectives: Effective management of acute pancreatitis (AP) hinges on prompt volume resuscitation and is adversely affected by delays in diagnosis. Given diverse clinical settings (tertiary care vs. community hospitals), further investigation is needed to understand the impact of the initial setting to which patients presented on clinical outcomes and quality of care. This study aimed to compare outcomes and quality indicators between AP patients who first presented to the emergency department (ED) of a tertiary care center and AP patients transferred from community hospitals.
