In seven children, five to nine years old, with a flail shoulder and a functional hand and elbow, arthrodesis of the shoulder in 80 to 90 degrees of abduction was performed by denuding the humeral head and glenoid of cartilage and fixing the humerus to the glenoid with Steinmann pins first inserted in the humerus in a proximal-distal direction and then driven in the reverse direction into the glenoid. Postoperatively each limb was immobilized in a spica cast, the pins were removed after a month, and the cast was removed after ten to twelve weeks when union was evident. All seven fusions united…

The medicinal treatment of osteonecrosis is described as part of the conservative treatment for atraumatic femoral head necrosis of the hip … t Ahlbäck's disease, although the results are sometimes contradictory. Other locations, such as the humeral head, talus, scaphoid, lunate, proximal tibia, and metatarsal head, are sometimes successfully treated with bisphosphonates. Although the results seem promising, the groups are far too small to derive treatment recommendations in this regard.

Many patients with Human Immunodeficiency Virus (HIV) Infection now live long due to the availability of antiretroviral drugs. This has resulted in many hitherto unknown complications being reported. We report a 56-year-old trader who presented with a seven-month history of pain in both shoulders and hips. Laboratory investigations and x-rays confirmed oesteonecrosis of both humeral heads and femoral heads in HIV/AIDS. She is presently on antiretroviral drugs and conservative management of her multiple joint disorders.

Osteonecrosis is a common sequela of long-term steroid therapy. This paper presents the only case of multifocal osteonecrosis to occur after a short-term course of methylprednisolone for treatment of septic shock. Two years after receiving a two-week course of corticosteroid treatment, the patient developed pain in his right and left shoulders and left hip. Roentgenograms and magnetic resonance imaging revealed changes consistent with osteonecrosis of both humeral heads and the femoral head. The patient failed to respond to conservative treatment and eventually was treated with bipolar hip rep…

Twenty-seven patients (30 shoulders) presenting with an unreduced posterior dislocation or fracture-dislocation were operated on and followed up for at least 2 years. The lesions were classified in 4 types, after Randelli: posterior dislocation with cephalic fracture inferior to 50 per cent (type I = 10 cases) or superior to 50 per cent (type II = 8 cases) and posterior fracture-dislocations with two fragments (type III = 6 cases) or several fragments (type IV = 6 cases). The first patients of the series underwent a resection of the humeral head (5 cases) or an open reduction (7 cases). During…

Osteo-articular complications of sickle cell anaemia in adult are represented by bone marrow infarcts, osteomyelitis and osteonecrosis of femoral and humeral heads. The frequency and/or the severity of these complications provide a great functional disability in young patients whose life expectancy is growing. A well adapted treatment would may limit the sequels.

FINDINGS: To satisfy the mechanical equilibrium, a deficient subscapularis induced a decrease of the force of the infraspinatus muscle. This force decrease was balanced by an increase of the supraspinatus and middle delt …

Proximal migration of the humeral head is thought to indicate fatty infiltration of the rotator cuff muscles or rotator cuff tears. We sought to evaluate the influence of these rotator cuff abnormalities on the subacromial space. Using anteroposterior radiographs, ultrasound, and computed tomography, we analyzed 54 shoulders in 29 patients with rheumatoid arthritis. The upward migration index was defined as proximal migration of the humeral head relative to its size. The mean muscle density from computed tomography images was used to indicate fatty infiltration. Fatty infiltration of the infra…

Twenty-four recurrent dislocations of the shoulder with an onset after the age of 40 years have been operated on using the Trillat technique. The results have been studied with a mean follow-up of 10 years. This type of lesion, which is not rare, forming five per cent of cases in the authors series between 1953 and 1982, deserves separate consideration because of the frequency of associated rupture of the rotator cuff which seems to arise as a consequence of the first dislocation. There were 75 per cent of indirect tuberosity signs and 20 per cent of elevations of the humeral head before opera…

OBJECTIVES: Field-based planning for regional nodal breast radiotherapy (RT) used to be standard practice. This study evaluated a field-based posterior axillary boost (PAB) and two forward-planned intensity-modulated RT (IMRT) techniques, aiming to replace the first.
METHODS: Supraclavicular and axillary nodes, humeral head, brachial plexus, thyroid, and oesophagus were retrospectively delineated on 12 CT scans. Three plans, prescribed to 40.05 Gy, were produced for each patient. Breast plans consisted of field-in-field IMRT tangential fields in all three techniques. Nodal plans consisted of …

BACKGROUND: The glenoid track concept is used to determine preoperatively whether a Hill-Sachs defect is engaging or not. Currently, the glenoid track concept relies on measurements of bony structures as well as on the confines and elasticity of the rotator cuff as a reference point, which varies extensively among individuals and therefore limits the reliability and accuracy of this concept.
PURPOSE: To evaluate the reliability of the global track concept, which determines the angular distance of the Hill-Sachs defect from the center of the articular surface of the humeral head as a new refer…

BACKGROUND: This study aimed to present a safe zone for distal pin insertion for external fixation using magnetic resonance imaging (MRI) images.
METHODS: All patients who took at least one upper arm MRI from June 2003 to July 2021 were searched via a clinical data warehouse. For measuring the humerus length, proximal and distal landmarks were set as the highest protruding point of the humeral head and lowermost margin of ossified bone of the lateral condyle, respectively. For children or adolescents with incomplete ossification, the uppermost and lowermost ossified margin of the ossification…
