Chronic contained rupture of an abdominal aortic aneurysm mimicking a retroperitoneal mass with hydronephrosis.

Chronic contained rupture (CCR) of an abdominal aortic aneurysm rarely causes hydronephrosis and may mimic retroperitoneal malignancy. We report the case of a 66-year-old woman presenting with hydronephrosis caused by a CCR-related hematoma and an accessory renal artery originating from the terminal aorta. Biopsies excluded malignancy. Because endovascular repair carried a risk of type II endoleak and renal ischemia, open aneurysm repair with direct arterial reimplantation was performed. This approach relieved ureteral compression, achieved definitive aneurysm exclusion, and preserved renal pe…

Primary mucinous adenocarcinoma of the renal pelvis associated with obstructive urolithiasis and giant hydronephrosis.

Primary mucinous adenocarcinoma of the renal pelvis is an exceptionally rare malignancy that often mimics chronic stone disease or infected hydronephrosis. We report a 55-year-old man presenting with bilateral flank pain, bilateral nephrolithiasis, and giant right hydronephrosis. Ultrasound and contrast-enhanced computed tomography suggested obstructive infected hydronephrosis caused by a large ureteropelvic junction stone. During surgery, abundant gelatinous mucin and thickened friable renal pelvic mucosa raised suspicion for malignancy. Biopsy and final histopathology confirmed moderately di…

Managing Pyeloureteral Atresia in Pediatrics: A Case of Right-Sided Hydronephrosis and Surgical Intervention.

BACKGROUND Pyeloureteral atresia is a rare congenital anomaly characterized by complete obstruction of the ureteropelvic junction, leading to severe hydronephrosis and potential renal damage. Early diagnosis and surgical intervention are essential to preserve renal function and prevent long-term complications. CASE REPORT A 3-year-old boy presented with a 3-day history of worsening abdominal pain, nausea, and vomiting. Physical examination revealed abdominal tenderness, primarily in the right lower quadrant. Notably, there were no genitourinary symptoms such as dysuria, hematuria, or changes i…

Pyeloplasty for the treatment of ectopic renal hydronephrosis in adults: a case series of six patients.

BACKGROUND: Ectopic renal hydronephrosis is a relatively rare condition, with literature primarily consisting of scattered case studies. The ectopic kidney poses technical challenges to surgeons due to its structural and anatomical abnormalities. This study aims to share our initial experiences with minimally invasive pyeloplasty as a management approach for ectopic renal hydronephrosis in adult patients. CASE DESCRIPTION: Between August 2018 and January 2023, six patients underwent minimally invasive pyeloplasty for ectopic kidneys. The patient cohort consisted of four individuals with pelvi…

Value of detected hydronephrosis and hydroureter on ultrasound for detecting a ureteral stone: a retrospective study.

To investigate the predictive value of hydronephrosis and hydroureter in the detection of ureteral stones using ultrasound, a total of 366 patients suffering from ureteral stones confirmed by ultrasound, computed tomography (CT) and their follow-up treatments were consecutively collected. All the included patients underwent systematic ultrasound scanning of the kidney and ureter. The hydronephrosis and hydroureter in each patient were measured using a digital caliper. Then, receiver operating characteristic (ROC) curve analysis was used to determine the optimal cutoff values for hydronephrosis…

Association of the Hydronephrosis Severity Score With Likelihood of Pyeloplasty: A Large Prospective Database Analysis.

OBJECTIVE: To apply and reproduce this scoring system in our prenatal hydronephrosis population with ureteropelvic junction obstruction (UPJO)-like hydronephrosis (HN), specifically looking at determining better HHS cutoffs that would allow for stratification into three risk categories: spontaneous HN resolution, observation, and surgery. METHODS: A prospectively collected prenatal hydronephrosis database was reviewed to extract UPJO-like HN patients. Children with vesicoureteral reflux, primary megaureter, bilateral HN, and other associated anomalies were excluded. Only patients who had an u…

The utility of renal sonographic measurements in differentiating children with high grade congenital hydronephrosis.

INTRODUCTION: Current grading systems for hydronephrosis include a subjective determination of parenchymal 'thickness' and suffer from poor reliability. Use of more objective ultrasonographic measurements including medullary pyramidal thickness (PT) may be useful in augmenting current grading systems by decreasing subjectivity and enhancing prognostic ability. OBJECTIVE: To evaluate the utility of PT measurements in patients with SFU grades 3 and 4 hydronephrosis, we assessed the: 1) the inter-rater reliability of PT measurements, 2) the correlation between relative renal function on nuclear …

Outcome of the treatment of hydronephrosis due to congenital ureteropelvic stenosis according to age at surgery.

BACKGROUND: Congenital kidney and urinary tract anomalies are the most common cause of chronic kidney disease in the first three decades of life. Stenosis of the ureteropelvic junction may cause dilation of the collecting system in the fetal kidney. This study aimed to determine hydronephrosis due to congenital ureteropelvic stenosis treatment outcome according to the age of the intervention. METHODS: We conducted a retrospective descriptive study that included pediatric patients with hydronephrosis secondary to ureteropelvic junction stenosis operated by the Anderson-Hynes open pyeloplasty m…

Outcome analysis of early surgery and conservative treatment in neonates and infants with severe hydronephrosis.

OBJECTIVE: The treatment strategy and timing of ureteropelvic junction obstruction (UPJO) in infants remain controversial. This study aimed to compare the effect of early surgical treatment (EST) and conservative treatment (CT) on neonates and infants with UPJO and their recovery of renal function and morphology. METHODS: Eighty neonates and infants with severe hydronephrosis were enrolled in this study. They received early pyeloureteroplasty or CT. Diethylenetriamine pentaacetate was used to assess renal function. RESULTS: There were no significant differences in renal function or renal ind…

Survival outcomes of patients with cervical cancer and accompanying hydronephrosis: A systematic review of the literature.

Hydronephrosis is a sign of advanced stage disease in patients with cervical cancer. Its presence is believed to negatively affect the survival of patients. To date, however, consensus in this field is still lacking. The purpose of the present systematic review is to gather the available data and to provide directions for future research in the field. We systematically searched Medline, Scopus, Clinicaltrials.gov, EMBASE, Cochrane Central Register of Controlled Trials CENTRA and Google Scholar databases from inception till June 2018. Overall, 22 studies were included in the present systematic …

Diuresis renography in the evaluation and management of pediatric hydronephrosis: What have we learned?

Diuresis renography (DR) is widely used in the evaluation of hydronephrosis and hydroureter in infants and children. The goal of this provocative nuclear imaging examination should be to detect the hydronephrotic kidneys at risk for loss of function and development of pain, hematuria, and urinary tract infection. The reliability of DR is dependent on the acquisition and processing of the data as well as interpretation and utilization of the results. In this review, the key concepts of standardized DR and pitfalls to avoid are highlighted.

Gastrointestinal cancer and bilateral hydronephrosis resulted in a high risk of ureteral stent failure.

BACKGROUND: Urologists frequently encounter malignant ureteral obstruction (MUO) caused by advanced urological or non-urological malignant disease, but the treatment policy is unclear. The present study examined the risk factors for predicting ureteral stent failure in patients with MUO after ureteral stent insertion and the change in the renal function after retrograde ureteral stent insertion in cases of bilateral hydronephrosis. METHODS: A total of 39 patients who required ureteral stent placement for MUO at Yokohama City University Medical Center (Yokohama, Japan) between February 2007 an…

[GIANT CELL CARCINOMA OF THE RENAL PELVIS AND URETER WITH GIANT HYDRONEPHROSIS; A CASE REPORT].

A 78-year-old woman presented with macroscopic hematuria. We diagnosed giant hydronephrosis with a left ureteral stone and multiple renal pelvic carcinoma cT3N0M0 tumors.Open nephroureterectomy proceeded via a peritoneal approach, and 8,600 mL of urine were collected from the renal pelvis. The pelvic carcinoma was histologically confirmed as a rare giant cell variant with high malignant potential. The same variant was found in the upper ureter where a 15-mm stone had lodged. We considered that this was due to rapid progression of the renal pelvic giant cell carcinoma and hydronephrosis during …
