Congenital nephrogenic diabetes insipidus: the current state of affairs.

The anti-diuretic hormone arginine vasopressin (AVP) is released from the pituitary upon hypovolemia or hypernatremia, and regulates water reabsorption in the renal collecting duct principal cells. Binding of AVP to the arginine vasopressin receptor type 2 (AVPR2) in the basolateral membrane leads to translocation of aquaporin 2 (AQP2) water channels to the apical membrane of the collecting duct principal cells, inducing water permeability of the membrane. This results in water reabsorption from the pro-urine into the medullary interstitium following an osmotic gradient. Congenital nephrogenic…

[Osmotic cerebral oedema: the role of plasma osmolarity and blood brain barrier].

There are five types of oedema: vasogenic, cytotoxic, interstitial, hyperemic and osmotic. The differences lie on the type and localization of the oedema, the state of the blood-brain barrier (BBB) and the pathological context. Under physiological conditions, the osmolarity of extra cellular fluids (ECFs) is equal on both sides of the BBB. However, the pathophysiological variations of circulating osmolarity (including acute hyponatremia and hypernatremia) do not affect, at the same time, the osmolarity of cerebral ECFs. This situation generates an osmotic gradient on either side of the BBB. Th…

BACKGROUND: Electrolyte disorders are among the most frequent presentations in emergency internal medicine and may represent initial manifes … d restriction remaining first-line therapy and osmotic agents and tolvaptan as second-line options. Hypernatremia may occur in the context of diabetes insipidus and requires careful fluid management and, in central forms, desmopressin substitution. Hypokalemia can result from hyperaldosteronism or hypercortisolism and should prompt endocrine evaluation, especia …

Clinical profile of neonates with hypernatremic dehydration in a nephrology clinic.

AIM: The aim of the study was to assess clinical profile of neonates with hypernatremic dehydration (HD) and identify risk factors associated with acute kidney injury (AKI). MATERIALS AND METHODS: A retrospective study included 18 neonates with HD (serum Na ≥150 mmol/L) hospitalized in the Department of Pediatrics and Nephrology between the years 2009-2019. RESULTS: The age at presentation was 7.5±4.7 days (range 2-18), weight loss was 15.9±8.3% (range 7.1-32.6) and serum Na range was 151- 192 mmol/L (median 155.5 mmol/L). In 12 (67%) neonates, breast or mixed fed, HD occurred due to inadequ

A fatal case of hypernatraemic dehydration in a neonate.

Problems with lactation can result in hypernatraemic dehydration in the neonate, with potentially severe adverse consequences. This is illustrated in this fatal case of a 10 day old neonate who presented with excessive hypernatraemic dehydration due to insufficient breast milk intake, resulting in cerebral sinus vein thrombosis with cerebral haemorrhage and infarction. Differential diagnosis included excessive sodium intake (through inappropriately mixed formula or house remedies or through hyperaldosteronism) and high water deficit (renal or gastrointestinal losses, nephrogenic or central dia

Neurogenic hypernatraemia: case report.

A 49-year-old man was submitted to neurosurgery for a cranio-pharyngioma. The lesion, which appeared to involve the antero-inferior wall of the third ventricle, caused lack of appropriate antidiuretic hormone (ADH) release in response to hypernatraemia and plasma hyperosmolality. The probable mechanism of this hypothalamic syndrome is suggested.

Congenital glucose-galactose malabsorption due to SLC5A1 mutation: A case of hypernatraemic dehydration.

Congenital glucose-galactose malabsorption (GGM) is an exceedingly uncommon autosomal recessive metabolic state defined by persistent diarrhoea along with serious dehydration. It is a disease that is difficult to consider in differential diagnosis and may be fatal if left untreated. This report details the clinical and diagnostic progress of a two month-old Turkish infant exhibiting episodes of severe recurrent watery diarrhoea. Molecular testing revealed that the patient has a compound heterozygous variant in SLC5A1. The patient has been asymptomatic with fructose based formula. Paediatrician

Feasibility of strategies to manage dysnatremia in pediatric CKRT: a simulation study.

BACKGROUND: Correction of dysnatremia during continuous kidney replacement therapy (CKRT) presents unique technical challenges. Most publish … d in patients ≤ 20 kg and at higher sodium targets (≥ 120 mmol/L for hyponatremia, ≥ 161 mmol/L for hypernatremia) due to the inability to maintain the minimum post-filter flow standard in pediatric practice. Liberalizing other parameters had minimal effect; only the removal of the post-filter flow minimum restored full feasibility. CONCLUSIONS: Bag adjustment …

What is the aetiology of dysnatraemia in COVID-19 and how is this related to outcomes in patients admitted during earlier and later COVID-19 waves? A multicentre, retrospective observational study in 11 Dutch hospitals.

OBJECTIVES: To evaluate the relationship among dysnatraemia at hospital presentation and duration of admission, risk of intensive care unit (ICU) admission and all-cause mortality and to assess the underlying pathophysiological mechanism of hyponatraemia in patients with COVID-19. Our hypothesis is that both hyponatraemia and hypernatraemia at presentation are associated with adverse outcomes. DESIGN: Observational study. SETTING: Secondary care; 11 Dutch hospitals (2 university and 9 general hospitals). PARTICIPANTS: An analysis was performed within the retrospective multicentre cohort stu

Successful Systemic Thrombolysis in a Neonatal Occlusive Abdominal Aortic Thrombus Secondary to Hypernatremic Dehydration: A Case Report and Literature Review.

A 6-day-old neonate presented with hypernatremic dehydration and weight loss. Fluids were started for rehydration. He was subsequently found to have signs of reduced lower limb perfusion and investigations revealed an abdominal aortic thrombus and bilateral occlusion of renal arteries. Systemic thrombolysis led to complete resolution of renal artery thrombi with no major complications. A review of similar cases with hypernatremic dehydration and thrombosis is provided.

Central pontine myelinolysis in the hyperosmolar hyperglycaemic state.

OBJECTIVE: To report a rare association of central pontine myelinolysis (CPM) with hyperosmolar hyperglycaemic state (HHS). CLINICAL PRESENTATION AND INTERVENTION: A diabetic female presented with HHS and prolonged severe hypernatraemia. The metabolic derangement was adequately treated with proper correction of both hyperglycaemia and hypernatraemia. Lack of improvement in the presenting confusional state and the development of a fresh neurological deterioration led to the suspicion of CPM that was confirmed with magnetic resonance imaging. She fully recovered after 4 weeks with no specific m

Clinical approach to disorders of salt and water balance. Emphasis on integrative physiology.

Our purpose was to illustrate the utility of an approach that begins with simple principles of physiology to patients who have a disturbance in salt and water balance (Table 1). At times, the physiology is restricted to the kidney and body fluid compartments. In these settings, the goals of therapy are defined by calculating a tonicity balance--electrolyte-free water balances simply do not provide the needed information [3]. At other times, performing balances of other solutes such as urea reveal that another critically important problem is present (tissue catabolism). Thus the physiologic ana
