Necrotizing soft-tissue infections may be rapidly fatal because of toxin-induced circulatory collapse. Because of the often nonspecific clinical presentation, prompt diagnosis may be difficult but is imperative as prompt treatment can be lifesaving. This article discusses necrotizing fasciitis and clostridial myonecrosis, and highlights pathogenesis, clinical presentation, diagnosis, and treatment.

Bartonella (formerly Rochalimaea) quintana is the etiological agent of trench fever, a disease extensively reported during the World Wars. Recent molecular biology approaches have allowed dramatic extension of the spectrum of Bartonella infections. B. quintana is now also recognized as an etiological agent of fever and bacteremia, endocarditis, bacillary angiomatosis, and chronic lymphadenopathy. Human immunodeficiency virus-infected patients and/or homeless people are the most vulnerable to infection. Poverty and louse infestation were the main epidemiological factors associated with B. quint…

OBJECTIVES: To evaluate the causes of necrotizing soft-tissue infections (NSTls) and to determine the outcomes of patients with NSTls.
DESIGN: A restrospective survey of the causes and factors associated with the outcomes.
SETTING: An urban community hospital serving an indigent population.
PATIENTS: A consecutive series of patients with NTSls who were treated between December 11, 1990, and December 28, 1995.
INTERVENTIONS: Patients were treated with operative debridement, intravenous antibiotics, and supportive measures.
MAIN OUTCOME MEASURES: Patient outcomes, causes, the extent of infe…

Cytomegalovirus (CMV) infection is the most frequent systemic viral opportunistic infection in AIDS, occurring in almost 40% of patients, at a stage of profound immunodeficiency, with a CD4 cell count lower than 50/microL. The most frequent localizations are retinal and gastrointestinal. Diagnosis of retinis, which can be totally asymptomatic, is based on fundus examination, which should be performed regularly in patients with AIDS and/or low CD4 count. Diagnosis of colitis, as of other rare manifestation (oesophagitis, hepatitis, encephalitis, myeloradiculitis, pneumopathy), relies on the ass

On the occasion of a treatment of frequent infections of the upper respiratory tract an antibody deficiency syndrome was noticed in a 32-year-old female patient. Further investigations revealed a variable immunodeficiency syndrome, a disease belonging to the primary humoral immune defects and presenting with multiple clinical manifestations. Leading symptoms, however, are recurring respiratory infections that may lead to bronchiectasis. Therapy consists in the prevention of infections by periodical intravenous substitution with 7-S immunoglobulins and opportune prescription of antibiotics.

Diabetes mellitus is one of the most frequent metabolic distortions predisposing for infectious diseases. Characteristic features of patients with diabetes mellitus are dysfunctions of professional phagocytes, in particular of polymorphonuclear leucocytes. Infections of skin and soft tissue in particular of the lower extremities, rhinocerebral mucormycosis, invasive external otitis and urinary tract infections are typically associated with diabetes mellitus. Frequently, a rapidly progressive infection requires urgent surgical intervention and parenteral antimicrobial therapy. Short-interval me…

HIV infection in infants and children who acquire it early in life is quite different from HIV infection in adults. The immune deficiency that results occurs in an immunologically naive organism, resulting in a larger role for common pathogens in causing severe infections. The immune deficiency is superimposed on age-related changes in all parts of the immune system so that a CD4 count that would be comforting in an adult or older child is cause for concern in an infant. Manifestations of infection include susceptibility to common and opportunistic infection, but growth failure and neurocognit…

Hantaviruses cause haemorrhagic fever with renal syndrome (HFRS) and hantaviruspulmonary syndrome (HPS), and occur throughout the world. The viruses aretransmitted to humans in aerosols of rodent secretions. About 200 000 people, mainly in Eurasia, develop

Viral infections of the central nervous system in infants and children are uncommon but potentially serious illnesses. Common causes have included the enteroviruses (particularly polioviruses and coxsackieviruses), herpes viruses (type 1 and type 2 herpes simplex, varicella, and cytomegalovirus), arboviruses, rubella, mumps, measles (including subacute sclerosing panencephalitis), and human immunodeficiency virus. Several of these viruses, such as cytomegalovirus, herpes simplex, and vertically transmitted human immunodeficiency virus, may cause congenital infections. Others are acquired later…

Neonatal infections can be considered in three groups, those acquired antenatally, perinatally and nosocomially. For many years it has been recognized that antenatal infections may cause death or serious fetal damage, but only recently have the more subtle features of antenatal infection been recognized. These include particularly the ability of some (such as toxoplasmosis) to produce disease many years later. Perinatal infection is often the result of maternal carriage of organisms, usually asymptomatically, and a variety of treatment approaches including immunotherapy (for hepatitis B) and a…

Cerebrospinal fluid (CSF) shunt infection is one of the most frequent and disabling complications. We reviewed the records of 306 patients who underwent CSF shunt surgery from 1983 through 1992. Six hundred and twelve procedures were performed in these 306 patients. Infection occurred following 46 of the procedures for an infection rate of 7.5% per procedure. The 46 infections involved 39 patients. There were 8 recurrent infections. The infection rate per child was 12.7%. Staphylococcal species were isolated in 50% of all infections. Patients younger than 1 year old and children with multiple …

Bacteremia is an almost daily occurrence in childhood. Fortunately, infections in children are usually easily controlled and seldom produce serious sequelae. The prompt diagnosis and treatment of osteomyelitis is especially important to prevent catastrophic complications. Recent articles have pointed out that fine-needle aspiration may be diagnostic in only about 60% of children with acute hematogenous osteomyelitis and have suggested that other diagnostic modalities, such as white-cell scintigraphy, ultrasound, and computed tomography, may be more sensitive. Recent recommendations about treat…
