[Choledochoduodenal fistula of ulcer etiology].

INTRODUCTION: Choledochoduodenal fistulas are very rare and in most cases are caused by a long-lasting and poorly treated chronic duodenal ulcer. They may be asymptomatic or followed by symptoms of ulcer disease, by attacks of cholangitis or bleeding or vomiting in cases of ductoduodenal stenosis. The diagnosis is simple and safe, however treatment is still controversial. If surgery is the choice of treatment, local findings should be taken into consideration. As a rule, intervention involving closure of fistula is not recommended. CASE OUTLINE: The authors present a 60-year-old woman with a

Cytologic diagnosis of Enterobius vermicularis eggs in an enterocutaneous fistula.

A middle-aged female underwent a laparotomy for suspected ovarian cancer and developed a discharging sinus in the right iliac fossa. Smears of the discharge showed helminthic eggs which were characterised as those of Enterobius Vermicularis. The possibility of an enterocutaneous fistula was suggested which was subsequently confirmed during a relook laparotomy.

The authors diagnosed in a fifteen-year-old girl. other wise healthy, a double vesicosigmoidal fistula. It developed probably at the age of two after an acute abdominal attack of inflammatory character which was treated conservatively. The fistulae caused repeated inflammations of the urinary pathways. In the late diagnosis participated among others evaluation of miction cystography which was imperfect from the technical aspect. When repeated, this examination contributed to the diagnosis. After surgery the girl recovered.

Enterocutaneous fistulization due to Actinomyces odontolyticus. Report of a case.

A case of abdominal actinomycosis originating from the sigmoid colon is presented. Actinomyces odontolyticus was isolated; it is a rarity, but should be suspected in the case of a palpable mass and several fistulas developing after a latent period of weeks to months following surgical or inflammatory trauma. The diagnosis is made by anaerobic cultivation. Primary treatment with large doses of penicillin for several weeks may be supplemented by surgery. The strain isolated in the present case was only moderately sensitive to penicillin and was successfully treated with erythromycin.

[Surgical treatment of colovesical fistulas (our experience)].

The Authors study the statistics of the Verona Surgical Centre, concerning 5 cases of colovesical fistula. They support the two-time operation, that is, the colon resection with colo-colic anastomosis sheltered by a decompressive transversostomy and stitching of the vesical breach. They affirm the usefulness of the operation according to Hartmann in patients facing a greater operative risk.

[Appendectomy and Crohn disease].

The Authors, on the basis of the casuistry of the Verona Surgical Centre, concerning the interventions for Crohn's disease from 1973 up to 1983, and comparing the data of literature, study the problem of appendicectomy in Crohn's disease. They suggest the operation of appendicectomy both when there are signs of appendicopathy in course, joined to a pathology of the last ileum ansa, and in presence of apparently sound appendix and clear signs of ileum Crohn's disease. They think it opportune to avoid appendicectomy only in presence of clear signs of involvement of caecum by the morbid process.

Gastrocolic fistula complicating benign gastric ulcer. A case report and review of the literature.

Gastrocolic fistula complicating gastric or colonic surgery is a well-recognized entity. It is extremely rare, however, to encounter this condition in patients with benign peptic ulceration who have not undergone previous abdominal surgery. Such a patient was seen recently at Addington Hospital, Durban. The diagnosis was confirmed by means of barium enema examination and subsequent successful surgical resection of the fistula was performed.

Spontaneous appendicocutaneous fistula: case report and literature review.

A report is presented describing an exceptional case of spontaneous appendicocutaneous fistula (only six cases in the literature). Chronic appendiceal fistulas--that is primary communications between the appendix and an adjacent hollow viscus or the skin--occur rarely. Various types of appendicular fistula are considered and the literature is reviewed. Symptoms depend on location of the fistula and organ involved. The pathogenesis of such an unusual lesion is discussed. Appendectomy and excision or closure of the fistula is the radical therapy when the diagnosis is made.
