First-time traumatic patellar dislocation: a systematic review.

Acute patellar dislocations can result in patellar instability, pain, recurrent dislocations, decreased level of sporting activity, and patellofemoral arthritis. The initial management of a first-time traumatic patellar dislocation is controversial with no evidence-based consensus to guide decision making. Most first-time traumatic patellar dislocations have been traditionally treated nonoperatively; however, there has been a recent trend in initial surgical management. We performed a systematic review of Level I-IV studies to make evidence-based medicine recommendations on how a clinician sho

Long-term outcome of total knee arthroplasty in Charcot joint: a 10- to 22-year follow-up.

The results of 11 total knee arthroplasties in 9 patients with a Charcot joint secondary to neurosyphilis were reviewed. The mean age was 60.1 years, and follow-up averaged 12.3 years. Endomodel rotating hinge prostheses (Waldemar Link GMBH and Co, Hamburg, Germany) were implanted in all knees. The mean knee score increased from 44.9 before surgery to 95.0 at the final follow-up, and the mean function score increased from 45.0 before surgery to 93.6 at the final follow-up. Complications were noted in 3 knees, including 2 dislocations of the knee and 1 deep infection. The clinical results of to

Results of medial retinacular imbrication in patients with unilateral patellar dislocation.

Many surgical methods are used for treating malaligned patella after patellar dislocation, but one surgical method may not be appropriate for all patients. The purpose of this study was to determine the radiographic and subjective outcomes of patients who underwent imbrication of redundant medial structures. A total of 42 patients met the inclusion criteria of suffering a traumatic, unilateral patellar dislocation and had no J-sign, no patella alta, asymmetry on Merchant view radiograph with normal patella alignment on the noninvolved side, and MRI showing injury to the medial retinaculum. Pat

Management of patellar instability using knee arthroscopy and combined anterior and medial tibial tubercle transfer: A case report and literature review.

INTRODUCTION AND SIGNIFICANCE: The Fulkerson osteotomy, also known as anteromedial tibial tubercle transfer, is a surgical procedure used to address patellar instability. It aims to restore the extensor mechanism of the knee, primarily benefiting patients with recurrent patellar dislocations. CASE PRESENTATION: We present a case of a patient with chronic recurrent patellar dislocation. After arthroscopic evaluation of the patellofemoral joint, the Fulkerson osteotomy was performed arthroscopically along with a medial patellar retinacular plication. CLINICAL DISCUSSION: Selecting the appropri

Surgical reconstruction of severe patellofemoral maltracking.

Dysfunction of the patellofemoral mechanism presents in many ways. Results from different realignment procedures show great variability in patient outcome. A surgical technique is presented that attempts to correct all the abnormalities of patellofemoral maltracking. The procedure consists of a lateral release, a vastus medialis (obliquus) tendon advancement, and a tibial tubercle transfer. Along with being moved medially, the tubercle also is moved distally to correct patella alta and elevated anteriorly to reduce patellofemoral joint reaction forces. One hundred seven knees in 84 patients we

The surgical treatment of blunt and penetrating injuries of the popliteal artery.

Thirty-six patients sustaining blunt and penetrating injuries to the popliteal artery underwent vascular reconstruction with an average ischemic interval of eight and one-half hours (range: four to 14 hours). There were 16 associated venous injuries, 15 nerve injuries, and 20 fractures with or without knee dislocations. Arterial repair was performed by saphenous vein interposition graft in 23 patients and by end-to-end anastomosis in 16 patients (three patients had more than one operation). Eight of the venous injuries were reconstructed and the remaining eight veins were ligated. Fasciotomy w…

The Journey Bicruciate Knee Replacement: Design Modifications Yield Better Early Functional Results and Reduce Complications.

The Journey-I total knee replacement was designed to improve knee kinematics but had several complications including early dislocation. The Journey-II modification was introduced to reduce these while maintaining high function. To assess whether the modified Journey-II prosthesis has succeeded in its designers aims, we undertook an observational study of prospectively recorded data to analyze and compare the two knees. A total of 217 Journey-I and 129 Journey-II knees were identified from the department's prospectively collated registry and were assessed at 1 year by a comparative statistical

Underlying diagnosis predicts patient-reported outcomes after revision total knee arthroplasty.

OBJECTIVE: To assess the association of underlying diagnosis with outcomes after revision total knee arthroplasty (TKA). METHODS: For this cohort study we used prospectively collected data from the Mayo Clinic Total Joint Registry on all revision TKA patients from 1993 to 2005 with 2- or 5-year response to a validated knee questionnaire that assesses pain and function. We used logistic regression to assess the odds of moderate-severe activities of daily living (ADL) limitations and moderate-severe index knee pain 2 and 5 years after revision TKA. Odds ratios (ORs) and 95% CIs are presented.

Deterioration of long-term clinical results after the Elmslie-Trillat procedure for dislocation of the patella.

We examined 39 patients (45 knees) who had undergone an Elmslie-Trillat procedure for recurrent or habitual dislocation of the patella with a follow-up of more than ten years. The mean age at the time of surgery was 18.4 years; the mean follow-up was 161 months (120 to 238). Using Fulkerson's functional knee score, 41 knees (91%) had an excellent or good result at a mean follow-up of 45 months, and this was maintained in 29 (64%) at the final review. The main cause of deterioration in the clinical results was the onset or worsening of patellofemoral joint pain, not patellar instability.

Factors influencing knee valgus alignment in Crowe type IV hip dysplasia after total hip arthroplasty.

BACKGROUND: Valgus deformity of the knee remains a complaint after total hip arthroplasty (THA) among some patients with Crowe type IV hip dysplasia. We aimed to identify the knee alignment in these patients before and after surgery, and to explore the factors contributing to postoperative knee valgus alignment. MATERIALS AND METHODS: We retrospectively reviewed a series of Crowe type IV patients who received THA between February 2010 and May 2019 in our hospital. The patients' medical data were collected from the hospital information system. On both preoperative and postoperative full limb l

Framing Patellar Instability: From Diagnosis to the Treatment of the First Episode.

The patellofemoral joint (PFJ) is a complex articulation between the patella and the femur which is involved in the extensor mechanism of the knee. Patellofemoral disorders can be classified into objective patellar instability, potential patellar instability, and patellofemoral pain syndrome. Anatomical factors such as trochlear dysplasia, patella alta, and the tibial tuberosity-trochlear groove (TT-TG) distance contribute to instability. Patellofemoral instability can result in various types of dislocations, and the frequency of dislocation can be categorized as recurrent, habitual, or perman

Predisposing Anatomical Patellofemoral Factors for Subsequent Patellar Dislocation.

BACKGROUND: Primary patellar dislocation is a relatively uncommon knee injury but carries a high risk of recurrence, particularly in young and physically active adolescent individuals. Anatomical features of the patellofemoral joint have been implicated as key contributors to instability. The purpose of this study was to evaluate anatomical risk factors associated with recurrent patellar dislocation following a primary traumatic event, using MRI-based parameters. METHODS: Fifty-four patients who sustained a first-time lateral patellar dislocation were included. MRI was used to measure tibial

An unclassified tibial plateau fracture: Reverse Schatzker type IV.

The most commonly accepted system of classification for tibia plateau fractures is that of Schatzker. Increasingly, both high energy injuries and atypical osteoporotic fragility failures have led to more complex, unusual and previously undescribed fracture patterns being recognized. We present a case of a patient with a previously unreported pattern of tibia plateau fracture and knee dislocation. We highlight the challenges confronted and present the management and the outcomes of his injury. A 28-year old male motorcyclist was involved in a head on collision with a truck and was transferred b…
