[Can persistent toenail fungus be successfully treated with a laser?].

Onychomycosis is a common disease seen in dermatology practice. Most patients with onychomycosis opt for treatment due to the social stigma attached to the unsightly appearance, as well as the pain that can at times make walking difficult. However, in many cases, onychomycosis is resistant to oral antifungal medication, which is the first-line therapy for this disease. In recent years, we have attempted a new treatment method using a long-pulsed 1,064nm Nd :YAG laser (Cutera Inc., Brisbane, CA, USA) in refractory cases with onychomycosis. Using 1) a laser beam with a spot size of 5.0 mm and 2)

Segmental Lichen Planus Pigmentosus Successfully Treated With Picosecond Neodymium-Doped: Yttrium-Aluminium-Garnet Laser and Trifarotene in a Korean Patient.

Lichen planus pigmentosus (LPP) is a rare variant of lichen planus that presents as hyperpigmented patches, most commonly in individuals with darker skin tones, and is difficult to treat effectively. We report a case of a 22-year-old Korean male with LPP who showed considerable improvement following combination therapy with trifarotene and a picosecond neodymium-doped yttrium-aluminium-garnet (Nd:YAG) laser. Considering the inherently variable clinical course of LPP and the challenges associated with its treatment, the combination of topical retinoids and picosecond Nd:YAG lasers appears to be

Low-level green laser promotes wound healing after carbon dioxide fractional laser therapy.

BACKGROUND: The carbon dioxide (CO2 ) fractional laser resurfacing has become one of the hottest therapies for dermatoses. However, complications such as skin swelling, prolonged erythema, post-inflammatory hyperpigmentation, and scar formation remain. Low-level laser (LLL) therapy is accepted to promote skin wound healing and regeneration, decrease inflammation and pain, and modulate immunoreaction with low-dose laser of different wavelength. 532 nm laser therapy is commonly used to remove pigmented spots and to tender skin, but not utilized in wound care. OBJECTIVE: We aimed to determine th

Photobiomodulation associated to cellular therapy improve wound healing of experimental full thickness burn wounds in rats.

Adipose derived stromal vascular fraction (SVF) is a method of cell therapy potentially applicable for treatment of full thickness burns. Here we investigated if the association of photobiomodulation (PBM) with SVF therapy could improve wound healing in experimentally induced full thickness burn wounds in rats compared to the topical agent 2% silver sulfadiazine in a dose-dependent manner. Sixty-six male Wistar rats were divided in 4 groups containing 5 animals each which received the following treatments: 2% sulfadiazine (SD), SVF, SVF plus PBM at 30 mW (SVFL30), and SVF plus PBM at 100 mW (S

Laser treatment and its implications for photodamaged skin and actinic keratosis.

Treatment of widespread actinic keratoses (AKs) and extensive photodamage is a challenge. One of the treatment options is laser therapy, whereby physicians have the option of using ablative lasers (CO2 and Erbium Yttrium Aluminium Garnet) or nonablative fractional laser systems. With ablative laser systems, the superficial layers of the skin are ablated, including epidermal and superficial dermal actinic damage. Re-epithelialization occurs from uninvolved skin and keratinocytes from follicles. When using a CO2 laser, additional cosmetic improvements are a result of removal and tightening of th

Melasma: treatment strategy.

Melasma, a hypermelanosis of the face, is a common skin problem of middle-aged women of all racial groups, especially with dark complexion. Its precise etio-pathogenesis is evasive, genetic influences, exposure to sunlight, pregnancy, oral contraceptives, estrogen-progesterone therapies, thyroid dysfunction, cosmetics, and drugs have been proposed. Centro-facial, malar, and mandibular are well-recognized. Epidermal pigmentation appears brown/black, while dermal is blue in color, and can be distinguished by Wood's lamp illumination. The difference may be inapparent with mixed type of melasma in

[Psoriasis vulgaris. An indication for lasers?].

Treatment of psoriasis is often complicated and remains a challenge. Along with the many new immunomodulatory approaches, various laser systems have been employed in an attempt to treat chronic plaque psoriasis. These include CO(2), Er:YAG, pulsed dye, 1302 nm Nd:YAG and excimer lasers. After reviewing and critically evaluating the literature, we conclude that the 308 nm excimer laser has proven its efficacy, not only in comparison with other laser systems, but also when compared with conventional 311 nm narrow-band UVB treatment. It is an efficient and promising therapeutic option for treatin
