Strengthening health systems capacity to monitor and evaluate programmes targeted at reducing abortion-related maternal mortality in Jessore district, Bangladesh.

BACKGROUND: Abortion related deaths as a proportion of maternal mortality appears to have fallen dramatically in Bangladesh from 5 % in 2001 to 1 % in 2010. Yet complications from menstrual regulation (MR) and unsafe abortion continue to cause deleterious health, economic and social consequences for women in the country. METHODS: This quasi experimental design study with a baseline (January to December 2008) and an endline survey (August to October 2009) was conducted in 69 public, private, and NGO sector health facilities in Jessore district of Bangladesh with the objective of adapting and i…

Impact of maternal diabetes mellitus on mortality and morbidity of very low birth weight infants: a multicenter Latin America study.

OBJECTIVES: To compare mortality and morbidity in very low birth weight infants (VLBWI) born to women with and without diabetes mellitus (DM). METHODS: This was a cohort study with retrospective data collection (2001-2010, n=11.991) from the NEOCOSUR network. Adjusted odds ratios and 95% confidence intervals were calculated for the outcome of neonatal mortality and morbidity as a function of maternal DM. Women with no DM served as the reference group. RESULTS: The rate of maternal DM was 2.8% (95% CI: 2.5-3.1), but a significant (p=0.019) increase was observed between 2001-2005 (2.4%, 2.1-2.

Comparison of maternal mortality and morbidity between trial of labor and elective cesarean section among women with previous cesarean delivery.

OBJECTIVE: This study was undertaken to assess the safety of trial of labor after previous cesarean delivery. STUDY DESIGN: Retrospective cohort study of 308,755 Canadian women with previous cesarean delivery between 1988 and 2000. Occurrences of in-hospital maternal death, uterine rupture, and other severe maternal morbidity were compared between women with a trial of labor and those with an elective cesarean section. RESULTS: Rates of uterine rupture (0.65%), transfusion (0.19%), and hysterectomy (0.10%) were significantly higher in the trial-of-labor group. Maternal in-hospital death rate

Divergent Epidemiology of Maternal Mortality in Sub-Saharan Africa: The Case for Differentiated Regional Strategies to Achieve 2030 SDG Targets.

BACKGROUND: Despite global efforts, sub-Saharan Africa continues to account for 70% of maternal deaths, yet systematic cause-specific analyses across African regions remain limited. We examined 33-year trends in maternal mortality to identify epidemiologic divergence and guide differentiated intervention strategies. METHODS: Using the 2023 Global Burden of Disease Study, maternal mortality ratios (MMR), cause-specific deaths, and age-specific patterns were analysed across five African regions (Central, Eastern, Northern, Southern, and Western Africa) from 1990 to 2023. Absolute and relative c…

Maternal and Neonatal Mortality in Somalia: A Persistent Women's Health Crisis Requiring Primary Health Care and Community Health System Reform.

This policy-oriented commentary aims to interpret the drivers of maternal and neonatal mortality in Somalia through the Three Delays Model and the World Health Organization health-system building-block framework and to propose a Somalia-specific policy framework for strengthening the continuum of care. It draws on purposively selected peer-reviewed literature, national and international reports, and health-system evidence. Its distinctive contribution is to frame maternal and neonatal mortality as both a women's health and reproductive-health equity crisis and a priority for health-system refo

Maternal ontogeny modulates auxin-associated cutting mortality in Amentotaxus formosana, with genotype-specific responses revealed by Cox proportional hazards modelling.

Amentotaxus formosana (Taxaceae) is an endangered relict conifer for which stem-cutting propagation is essential for ex situ conservation. Seed-based regeneration is unreliable in its small, fragmented, dioecious populations. However, extreme and uncharacterized cutting mortality has severely bottlenecked these efforts. Because conventional static endpoint evaluations mask the critical temporal dynamics of rhizogenesis and failure, the underlying drivers of this attrition remain obscure. Here, we applied Kaplan-Meier estimation and Cox proportional hazards modelling to a two-stage factorial ex

Maternal health care policy intervention and its impact on perinatal mortality outcomes in Ghana: evidence from a quasi-experimental design.

OBJECTIVE: This study aimed to evaluate the impact of Ghana's free maternal health care policy on stillbirth and perinatal death since its implementation a decade ago. STUDY DESIGN: The study used the propensity score matching method, a quasi-experimental design technique and secondary data to construct two groups of mothers with a history of perinatal deaths who subscribed to the 'free' maternal health care policy versus mothers who did not. METHOD: The study merged two rounds of repeated cross-sectional data sets obtained from the Ghana Demographic and Health Survey (GDHS), 2008 and 2014,

Neural tube defects: Prevalence, mortality, and maternal characteristics in two departmental hospitals in the northwestern region of Nicaragua, 2006-2018.

BACKGROUND: Congenital anomalies are the fifth most common cause of neonatal mortality in Nicaragua, and neural tube defects (NTDs) are the most common of all cases of lethality associated with a birth defect. Prevalence and mortality estimates are needed to propose effective intervention strategies that prevent NTDs over time. METHODS: A cross-sectional study was carried out in northwestern Nicaragua from January 2006 to December 2018. All cases of NTDs (anencephaly, spina bifida, and encephalocele) were registered in hospital surveillance systems, and the medical histories of the mothers an

Risk management of pregnant women and the associated low maternal mortality from 2008-2017 in China: a national longitude study.

BACKGROUND: Reducing maternal mortality is one of the key targets of the Sustainable Development Goals (SDGs). In response to the impact of increased birth rate on maternal and child safety following the implementation of the two-child policy in 2013, the Chinese government implemented the risk management strategy (namely Five Strategies for Maternal and Newborn Safety, FSMNS) to reduce maternal mortality ratio (MMR). We aimed to analyze the changes in the proportion of pregnant women at high risk screened before and after the implementation of the risk management strategy and the association …

Maternal near-miss and mortality associated with hypertensive disorders of pregnancy remote from term: a multicenter observational study in Ghana.

BACKGROUND: Maternal death rates remain high in many low- and middle-income countries. Hypertensive disorders of pregnancy account for 18% of maternal mortality in Ghana. The maternal near-miss approach was designed to evaluate severe (acute) complications in pregnancy, which is useful to detect potential areas for clinical care improvement. OBJECTIVE: This study aimed (1) to determine the incidence of severe maternal complications, maternal near-miss cases, and mortality cases associated with hypertensive disorders of pregnancy remote from term and (2) to assess the health system's performan…

Can the primary health care model affect the determinants of neonatal, post-neonatal and maternal mortality? A study from Brazil.

BACKGROUND: The state of São Paulo recorded a significant reduction in infant mortality from 1990 to 2013, but the desired reduction in maternal mortality was not achieved. Knowledge of the factors with impact on these indicators would be of help in formulating public policies. The aims of this study were to evaluate the relations between socioeconomic and demographic factors, health care model and both infant mortality (considering the neonatal and post-neonatal dimensions) and maternal mortality in the state of São Paulo, Brazil. METHODS: In this ecological study, data from national officia…

Assessing maternal risk for fetal-infant mortality: a population-based study to prioritize risk reduction in a healthy start community.

Study goals were to distinguish between maternal risk factors for fetal versus infant mortality, and to identify which maternal characteristics contributed the greatest risk of mortality overall. This case-control retrospective study abstracted data on more than forty maternal characteristics from 261 prenatal and delivery records: all 26 fetal deaths, all 40 infant deaths and 195 randomly selected surviving births in a high-mortality Healthy Start community. Bivariate and multivariate analyses were conducted. The fetal-mortality population was significantly more likely than the infant-mortali
