Advances in microsurgical reconstruction of the congenitally adactylous hand.

A series of 18 microsurgical toe transfers for congenitally adactylous hands was done from 1983 to 1994. The functional results achieved depended on the approach and technical features applied to hand reconstruction in congenital amputations. Function was improved significantly by using 3-jointed second toes, including the metatarsophalangeal joint, and by performing meticulous intrinsic repair. The use of donor arteries changed during the 11-year study period toward the goal of perfect revascularization with a minimized ischemic time. Abundant nerve supply was found to be important for achiev

High spatial resolution imaging of bone mineral using computed microtomography. Comparison with microradiography and undecalcified histologic sections.

RATIONALE AND OBJECTIVES: The application of various high-resolution (< 100 microns) imaging techniques for in vitro bone mineral analysis is explored. METHODS: The techniques of contact microradiography and microtomography, using the x-ray spectrum filtered out of synchrotron radiation (SR) and conventional staining techniques, are compared to each other by presenting a variety of different samples. The relationship between radiation exposure and spatial resolution micro-computed tomography (CT) images of a finger bone is explored. The relevant properties of SR are explained. RESULTS: In CT

Bennett's fracture combined with fracture of the trapezium. A report of three cases.

Three cases are reported in which open reduction and internal fixation were required to stabilize an unstable first carpometacarpal joint with simultaneous fracture of the trapezium and Bennett's fracture. The results were good in terms of range of movement and radiological appearance, all of them returning to normal activities, including heavy manual work.

Percutaneous distraction lengthening in brachymetacarpia.

Brachymetacarpia is a condition manifesting a shortened metacarpal that is caused by early closure of the epiphyseal plate and believed to arise idiopathically. The correction for brachymetacarpia is usually for cosmetic reasons as brachymetacarpia does not impair function. The literature indicates several different approaches to lengthening digits, including single-stage lengthening and rapid distraction lengthening with a bone graft. However, gradual distraction is the preferred method due to excellent cosmetic outcomes and few postoperative complications. This article reports a case of brac

Relation of lifestyle factors to metacarpal bone mineral density was different depending on menstrual condition and years since menopause in Japanese women.

OBJECTIVE: To examine whether the relation between lifestyle and metacarpal bone mineral density (BMD) varied with life-stages. DESIGN: Cross-sectional study. SETTING: Taking questionnaires related to lifestyle and examining metacarpal BMD by computed X-ray densitometry at a local health care center. SUBJECTS: Out of 750 Japanese women aged 40-69 y old who underwent screening for osteoporosis, we selected 535 healthy subjects without medical conditions or lifestyle factors known to affect bone metabolism. RESULTS: A cross-sectional comparison of BMD and years since menopause (YSM) resulted

[A comparative study of metacarpal resection and translocation after amputation of the middle finger].

The authors studied 43 patients having sustained an amputation of one middle finger treated either by simple ray resection or translocation. The index-middle translocation was followed by more minor postoperative complications compared to all other techniques. A global score, taking into account daily activities, cosmetic result lack of mobility, residual pain, decreased strength, time off work, and return to work gave 131 points for simple third ray resection compare to index middle translocation (-252 points). Surprisingly, reduction of hand span on X ray study was minimal but strength was n

Common hand injuries in the athlete.

Common injuries to the hand and wrist of athletes have been discussed under the headings of ligament injuries, fractures, tendon injuries, and neurovascular injuries. Although the frequency and mechanisms of certain injuries may be different in the athlete than in the nonathlete, the principles of proper early diagnosis and adequate treatment are the same whether the patient is an athlete or not.

Identification of cartilage injury using quantitative multiphoton microscopy.

OBJECTIVE: Cartilage injury can lead to post-traumatic osteoarthritis (PTOA). Immediate post-trauma cellular and structural changes are not widely understood. Furthermore, current cellular-resolution cartilage imaging techniques require sectioning of cartilage and/or use of dyes not suitable for patient imaging. In this study, we used multiphoton microscopy (MPM) data with FDA-approved sodium fluorescein to identify and evaluate the pattern of chondrocyte death after traumatic injury. METHOD: Mature equine distal metacarpal or metatarsal osteochondral blocks (OCBs) were injured by 30 MPa comp

Bone lengthening for congenital differences of the hands and digits in children.

PURPOSE: We investigated the growth of bones after lengthening to evaluate the effectiveness and usefulness of bone lengthening in congenital hand deficiencies. METHODS: Bone lengthening was performed to treat congenital hand deficiencies including hypoplastic thumb, hypoplasia of the small finger, cleft hand, metacarpal synostosis, brachymetacarpia, symbrachydactyly, and amniotic band syndrome. A total of 15 patients (1-13 years old) underwent lengthening of 23 bones. The average of distraction was 10 mm. The mean follow-up period was 59 months. The length of the bones at follow-up evaluatio

Results of biaxial total wrist arthroplasty with a modified (long) metacarpal stem.

PURPOSE: To evaluate the results of Biaxial (DePuy Orthopedics, Inc., Warsaw, IN) total wrist arthroplasty (TWA) with a long metacarpal stem. METHODS: A retrospective review of the results of 17 long-stem metacarpal components for the Biaxial TWA implanted between 1993 and 1997 is presented. RESULTS: After surgery pain and grip strength improved markedly. Overall motion improved but only radial deviation was significantly better. All of the patients were satisfied. At the most recent follow-up evaluation 4 cases showed evidence of radiographic lucency about the cement mantle, but with no gro

Metacarpal index in Nigerian adults and children.

The metacarpal index (MI) is used in screening for patients with arachnodactyly especially Marfan's syndrome. It is a radiographic measure of the slenderness of the 2nd-5th metacarpals, which was measured in a total of 608 Nigerians of which 55 (9%) were children and 553 (91%) were adults. The study shows that the MI increases steadily in children approaching the adult value at about the age of 9 years in females and 13 years in males coinciding with menarche and puberty respectively. A metacarpal index of up to 11 can be considered to be within normal limits for a Nigerian adult. In both chil

Divergent fracture-dislocation of the second carpometacarpal joint and the three ulnar carpometacarpal joints.

A case of divergent dislocation between the second metacarpal and the three ulnar metacarpals is presented. Severe edema, fragments of the trapezoid, and interposed volar ligaments prevented successful closed reduction. Open reduction enabled restoration of the carpometacarpal complex with excellent recovery of hand function.

[Fracture-dislocation of the second carpometacarpal joint. Case report and review of the literature].

A 26-year-old man was involved in a car crash then he presented an isolated trauma of the hand. We found out a dorsal dislocation of the second carpometacarpal joint with trapezoid fracture and a Rolando fracture. In emergency, closed reduction with percutaneous wires to stabilize, was performed. In the same time, the Rolando fracture was treated by internal fixation (plate). In a second time, after computerized tomography of the carpus, the trapezoid was reduced through a dorsal incision and held with two Kirschner-wires. At last follow-up one year after injury, the result was good. All movem

Surgical treatment of congenital metacarpal synostosis of the ring and little fingers.

Fifteen hands with congenital metacarpal fusions were treated operatively by osteotomy combined either with the use of a silicone block or a distraction device. The aim of the silicone block was to separate the fused metacarpals, but it was buried in the metacarpals at follow-up, with recurrence of fusion. By the distraction technique, 13 mm of lengthening was obtained. The average length of the little metacarpal was 88% of that of the long finger metacarpal. Both methods were effective in correcting the abduction deformity of the little finger by a mean of 34 degrees, and increasing the range

[Middle-hand and finger fractures].

An account is given in this paper of 299 metacarpal and phalangeal fractures. Cases of that kind have a negative impact on the absence-for-illness record in national economy. Careful conservative therapy based on clear-cut concepts and unambiguous indications for surgery are considered necessary for the purpose of shortening absence periods. Conservative treatment still is applied to most of all phalangeal fractures (94 per cent); while less than 25 per cent of all metacarpal fractures are successfully handled by surgery.
