Chronic suppurative otitis media: a review.

OBJECTIVES: Chronic suppurative otitis media (CSOM) remains one of the most common childhood chronic infectious diseases worldwide. Although microbial, immunological, and genetically determined factors, as well as Eustachian tube characteristics, are supposed to be involved in the pathogenesis of CSOM, many aspects of the pathogenesis of CSOM still need to be clarified. Optimal treatment strategy has not been established yet. The objective of this review is to present and evaluate the current state of knowledge of CSOM. DESIGN: Systematic narrative review. METHODS: A PubMed search (1966-Janu

Gelfoam/Gelfilm patching following the removal of ventilation tubes.

A follow-up study to document the rate of tympanic membrane perforation following surgical removal of ventilation tubes with Gelfoam/Gelfilm patching in paediatric patients is presented. The initial study performed at The Hospital for Sick Children with no patching following ventilation tube removal had a perforation rate of 10.3%. This follow-up study used Gelfoam/Gelfilm patching of perforations at the time of ventilation tube removal performed between January 1993 and May 1994 at the same hospital. There was sufficient follow-up on 155 ears with (4.5%) documented perforations, a significant

Tympanosclerosis and mini grommets: the relevance of grommet design.

Fifty children with otitis media with effusion undergoing grommet insertion had into one ear a Mini-titanium grommet inserted and into the other ear a Mini-teflon grommet. Post-operative follow-up until after extrusion of the grommets demonstrated only a small difference between the extrusion times of the two grommets (a significant difference of 41 days) and no difference in the degree of tympanosclerosis seen with each grommet. We propose that the mass of a grommet appears to play less of a role than has previously been suggested in the pathogenesis of tympanosclerosis following grommet inse

Grommets, swimming and otorrhoea--a review.

Ever since Armstrong reintroduced the concept of grommet insertion parents have been asking 'may my child swim?', yet there is still no consensus as to the correct answer. This paper reviews the work that has been done on this subject in the last 25 years. A review of the rates of otorrhoea following grommet insertion, irrespective of swimming, shows a variation from 12 to 64 per cent. Evidence suggests that pressures of 12-23 cm H2O are needed to push water through a grommet and that it is unlikely that water will enter the middle ear during surface swimming. Only bath water seems to cause si

Management of the hearing impaired child with serous otitis media.

The majority of research and informative papers dealing with otitis media with effusion (OME) are concerned with normally hearing children who display a mild conductive hearing loss associated with the infection. In contrast, the target of our manuscript is the child who has a previously documented sensori-neural hearing loss that is complicated by the onset of a middle ear infection. It is our clinical experience that OME in the hearing impaired child must be treated aggressively in order to adequately override the additional loss that accompanies a middle ear infection. A number of our patie

Surgical considerations during cochlear implantation: the utility of temporal bone computed tomography.

OBJECTIVE: This paper describes the construction of portals for electrode placement during cochlear implantation and emphasises the utility of pre-operative temporal bone three-dimensional computed tomography. METHODS: Temporal bone three-dimensional computed tomography was used to plan portal creation for electrode insertion. RESULTS: Pre-operative temporal bone three-dimensional computed tomography can be used to determine the orientation of temporal bone structures, which is important for mastoidectomy, posterior tympanotomy and cochleostomy, and when using the round window approach. The Use of Topical Lidocaine Versus Lidocaine Injection for Myringotomy and Ventilation Tube Insertion.

Background and Objectives: Minor otologic procedures in adults are often performed under local anesthesia, either via injection or topical application, thereby avoiding general anesthesia-associated risks. This study aims to compare pain levels with the use of a lidocaine spray versus lidocaine injections. Materials and Methods: Fifty adult patients underwent local anesthetic myringotomy and ventilation tube placement, 30 unilaterally, and 20 bilaterally. Lidocaine injections were administered to 29 patients, and 21 received a lidocaine spray. Postoperatively, they were asked to mark their per

Pneumolabyrinth: a rare complication of stapes surgery.

Pneumolabyrinth is the entrapment of air within the inner ear and is a rare complication of stapes surgery. We report the case of a patient submitted to stapedectomy who, 4 weeks later, suddenly developed right hearing loss, ipsilateral tinnitus and vertigo. On the physical examination, the patient showed no signs of vestibular deficits. Audiometry was compatible with right profound mixed hearing loss and high-resolution CT of the temporal bone revealed the presence of pneumolabyrinth. During exploratory tympanotomy, the prosthesis was found dislodged; the communication between the middle and

Endoscopic Approach for Tympanostomy Tube Insertion in Patients With Trisomy 21.

Otitis media with effusion (OME) is common in children with Down syndrome (DS), affecting 93% of children by age 1 year and 68% of children by age 8 years ( 1,2 ). Resultant conductive hearing loss is present in up to 80% of children with DS and OME, making prompt treatment imperative to prevent adverse effects on language and development ( 2-4 ). Placement of tympanostomy tubes is common treatment for chronic OME with associated conductive hearing loss. However, tympanostomy tube insertion can be challenging in the DS population due to stenotic external auditory canals, a morphologically defi
