Early-versus Late-Onset Alzheimer Disease: Long-Term Functional Outcomes, Nursing Home Placement, and Risk Factors for Rate of Progression.

BACKGROUND/AIMS: Whether age at onset influences functional deterioration in Alzheimer disease (AD) is unclear. We, therefore, investigated risk factors for progression in activities of daily living (ADL) and nursing home placement (NHP) in cholinesterase inhibitor (ChEI)-treated patients with early-onset AD (EOAD) versus late-onset AD (LOAD). METHODS: This 3-year, prospective, observational, multicenter study included 1,017 participants with mild-to-moderate AD; 143 had EOAD (onset <65 years) and 874 LOAD (onset ≥65 years). Possible sociodemographic and clinical factors that could affect fun

Person-Centered Care, Care Climate, and Quality of Life: Evidence From Inpatient Settings in Kazakhstan.

BACKGROUND: Person-centered care (PCC) is recognized as a core indicator of high-quality nursing and organizational performance. Although PCC and supportive care climates have been associated with patient outcomes, limited evidence exists from Central Asia on how these factors relate to patients' quality of life (QOL). AIM: To examine associations between adult inpatients' perceptions of PCC, the care climate, and their QOL in Kazakhstan. DESIGN: Cross-sectional analytical study. METHODS: A total of 255 adult inpatients from two public hospitals completed the Patient-Centered Inpatient Scal

Effect of virtual reality assimilated the BOPPPS model on evidence-based healthcare curriculum: A mixed-methods study.

OBJECTIVE: Evidence-based healthcare (EBHC) has emerged as a global standard in clinical practice, offering a structured approach to improving patient outcomes. For nurses, EBHC serves as a critical framework for identifying and applying the best available evidence when addressing clinical challenges. This study aimed to examine the effectiveness of integrating virtual reality (VR) into the BOPPPS (Bridge-in, Objective, Pre-test, Participatory Learning, Post-test and Summary) teaching model to enhance the design and delivery of EBHC courses. MATERIALS AND METHODS: Participants were first-year

Linear scleroderma: a Neuman nursing perspective.

Although quite a rare pediatric disease, linear scleroderma (LS), a chronic disease, may leave scarring and may have devastating effects on a child and family. This article provides an overview of the disease, presents a case history of a boy with LS, and discusses how nurses and other health-team members assisted the child and family in managing the illness. The description of the nursing care is based on the Neuman systems model and on the nursing diagnoses of fear, disturbed self-concept, and impaired physical mobility.

Comfort as process and product, merged in holistic nursing art.

Although many scholars discuss nursing as an art and science, the practice of nursing art remains unspecified and elusive, especially to learners. Nursing art is the esthetic use by nurses of scientific and humanistic principles of care applied creatively within specific contexts of care. One form of nursing art is called comfort care. Comfort care entails the process of comforting actions and the product of enhanced comfort, both of which have been described in separate programs of research. Because the process of comfort includes the product of comfort, the construct of comfort care designat

Mapping Scandinavian Hospital-at-Home Organisations and Interventions for Patients With Acute Somatic Illness-A Scoping Review.

INTRODUCTION: Hospital-at-Home (HaH) has been implemented in several countries. However, HaH interventions vary depending on organisational structures and target populations. In Denmark, Norway, and Sweden, HaH is offered in selected regions and municipalities, often as pilot or quality projects. Currently, there are no guidelines or systematic overviews of Scandinavian HaH models. AIM: This review aims to map published research that describes Scandinavian HaH organisations, interventions, and patient characteristics for acute somatic illness. METHODS: In this scoping review, papers were ide

The consultative process used in outreach: a narrative account.

AIM: The aim of this paper was to provide a narrative account of the communication skills used in an effective outreach consultation utilizing Neighbour's consultative model. Other consultation models were considered; however, because of their overly comprehensive approach or emphasis on behaviour modification, these were deemed inappropriate. BACKGROUND: The nursing profession has endured significant changes of late and as a result is developing more autonomous roles in both the community and the acute health care settings. In the past, the term consultancy was used within the medical contex

Nurse work environment, missed nursing care, moral distress, and burnout among ICU nurses.

OBJECTIVES: To test whether missed nursing care and moral distress mediate the association between the nurse work environment and burnout among adult ICU nurses. METHODS: This study used data from 4258 direct care nurses in adult ICUs from the 2024 Penn Nurses4All Study. Structural equation modeling with serial multiple mediation examined direct and indirect associations between the work environment and burnout through missed nursing care and moral distress, adjusting for nurse characteristics. RESULTS: Nurses averaged 14.1 years of experience (SD = 11.4); most were female (2429; 79.6%), wor

Optimizing Agreement Between Bedside Nurse-Documented and Trained Researcher Delirium Assessments in the ICU.

OBJECTIVES: Delirium is common and harmful in the ICU. The Intensive Care Delirium Screening Checklist (ICDSC) and Confusion Assessment Method for the ICU (CAM-ICU) are validated tools recommended for delirium identification. However, the accuracy of bedside nurse-documented delirium assessments in the ICU is inconsistent, limiting utility in clinical research. We sought to evaluate and optimize agreement between bedside nurse-documented and trained researcher delirium assessments. DESIGN, SETTING, AND PATIENTS: Critically ill adults with acute respiratory failure or sepsis in ICUs in large a

Impacts of Perioperative Comprehensive Nursing Intervention on Postoperative Urinary Incontinence and Quality of Life of Patients Undergoing Laparoscopic Radical Prostatectomy.

To evaluate the impact of perioperative comprehensive nursing intervention on postoperative urinary incontinence, various aspects of patient well-being were assessed. The comprehensive group, that received the nursing intervention, demonstrated significant improvements in self-care skills, health knowledge level, self-care responsibility, and self-concept compared to the standard group. The findings indicate that perioperative comprehensive nursing intervention has a remarkable effect on patients undergoing laparoscopic radical prostatectomy. This nursing intervention not only effectively impr

Nurse-Led Trauma-Informed Correctional Care for Women.

PURPOSE: Incarcerated women are a vulnerable and unique population of special concern to nurses as they have high rates of mental illness. In this article, the authors discuss how trauma exposure contributes to mental illness in incarcerated women through abuse, socioeconomic factors, and the prison environment, how this trauma exposure manifests in the inmate survivor, and the related implications for practice. CONCLUSIONS: A history of trauma and victimization is related to complex mental health issues which affect the majority of justice-involved women. The correctional environment can exa

Factors associated with spiritual care competencies in Taiwan's clinical nurses: A descriptive correlational study.

AIMS AND OBJECTIVES: To determine factors associated with nurses' spiritual care competencies. BACKGROUND: Holistic nursing care includes biopsychosocial and spiritual care. However, nurses are limited by a lack of knowledge, time constraints and apprehension of assessing spiritual issues, which leaves them unable to assess and meet patients' spiritual needs. Thus, when patients experience spiritual distress, clinical nurses lose the opportunity to support spiritual growth and self-actualisation. In Taiwan, spiritual care, religion and culture are unique compared to those in other countries.

Use of a hearing assistance device in nursing homes.

An inexpensive hearing assistance device can improve communication with nursing home residents. In patients who do not have prescribed hearing aids or who are being audiologically evaluated, voice amplification with such a device facilitates initial assessment as well as ongoing care. Such devices are available at local consumer outlets for under $30, making them available to persons who are unable to afford more sophisticated models.

A theory-based approach to nursing shared governance.

BACKGROUND: The discipline of nursing uses a general definition of shared governance. The discipline's lack of a specified theory with precepts and propositions contributes to persistent barriers in progress toward building evidence-based knowledge through systematic study. PURPOSE: The purposes of this article were to describe the development and elements of a program theory approach for nursing shared governance implementation and to recommend further testing. METHOD: Five studies using multiple methods are described using a structured framework. The studies led to the use of Lipsey's meth

The value and role of skin and nail assessment in the critically ill.

Over the last 10 years, numerous political drivers have paved the way for the development of new and innovative models of practice within the critical care environment. Many of these new models of practice required practitioners to perform detailed and comprehensive patient assessment. Within critical care, respiratory assessment is increasingly regarded as an essential element of a critical care practitioner's profile. However, the assessment of other body systems has been adopted more slowly. This article argues that skin and nail assessment should be ascribed a high priority within practiti
