Methods of monitoring brain oxygenation.

INTRODUCTION: Posttraumatic brain ischemia or hypoxia is a major potential cause of secondary injury that may lead to poor outcome. Avoidance, or amelioration, of this secondary injury depends on early diagnosis and intervention before permanent injury occurs. However, tools to monitor brain oxygenation continuously in the neuro-intensive care unit have been lacking. DISCUSSION: In recent times, methods of monitoring aspects of brain oxygenation continuously by the bedside have been evaluated in several experimental and clinical series and are potentially changing the way we manage head-injur

[Intra-abdominal pressure measurement using a bladder catheter].

In case of abdomen compartment syndrome, measurement of bladder pressure is an accurate means to assess peritoneal pressure. Authors describe measurement technique and pitfalls to avoid.

Use of an anesthesia cerebral monitor bispectral index to assess burst-suppression in pentobarbital coma.

A seven-year-old child with generalized status epilepticus who was placed in a barbiturate coma was monitored with the bispectral index monitor in addition to the standard full channel electroencephalogram. This child had a low bispectral index number and high suppression ratio on the bispectral index monitor when the desired level of pentobarbital coma was induced. There was excellent correlation of the bispectral index monitor to the suppression ratio. The burst rate also correlated well to the bispectral index number and to the suppression ratio. Therefore the bispectral index monitor could

Anesthetic considerations in a patient with visceral leishmaniasis.

PURPOSE: To describe the anesthetic problems in a patient with visceral leishmaniasis undergoing general anesthesia. CLINICAL FEATURES: A 17-yr-old man with visceral leishmaniasis was booked for emergency appendectomy. He received parentral sodium stibogluconate 600 mg per day. The patient was pale, afebrile and had hepatosplenomegaly. Preoperative investigations showed a hemoglobin of 6.2 g.dL(-1), platelet count of 80 x 10(9).L(-1) and serum albumin of 2.1 g.dL(-1). The electrocardiogram and chest x-ray were normal. Anesthesia was induced with 100 microg fentanyl and 50 mg propofol iv and t

[Monitoring of cytomegalovirus infection in patients with kidney transplantation: the first experience in Cuba].

Kidney transplantation is at present the most efficient therapeutic tool to treat end-stage chronic kidney failure. Graft rejection and infections are the most important barriers to the success of transplantation, whereas cytomegalovirus (CMV) has been the most reported causal agent of morbidity in this type of patients. The evolution of a group of 12 patients who underwent kidney transplantation at the Nephrology Institute was studied during 16 weeks aimed at monitoring the infection caused by CMV. 58.4% of these patients were positive and 28% showed clinical evidences of the disease. Most of

Assessment of clinical severity and investigation of uncomplicated gastroesophageal reflux disease and noncardiac angina-like chest pain.

Heartburn, suggesting gastroesophageal reflux, is common. Epidemiological studies have shown that 36% to 44% of adults experience heartburn at least once a month, 14% weekly and 7% once a day. Heartburn and regurgitations are the typical symptoms of gastroesophageal reflux disease (GERD). When present as predominant symptoms, they are quite specific but not very sensitive. Clinical severity of GERD does not predict the severity of the underlying condition. The diagnostic approach to patients with GERD depends on the clinical presentation and the question to be answered -Is abnormal reflux pres

[Serial computed thermography for Raynaud's phenomenon of patient with progressive systemic sclerosis (PSS)].

We reported a case with progressive systemic sclerosis, a relatively rare disease in childhood. Raynaud's phenomenon (RP) of the patient was serially monitored by computed thermography for 1 year. Although clinically apparent RP manifested only in winter season, thermographical changes were documented through all season, suggesting that RP is a constitutional sign which may emerge in winter by provocation of atmosphere. Thus, we recommend to use the words, "Raynaud's phenomenon" and " Raynaud's symptom", in an appropriate meaning. RP of the patient, however, varied depending on the time determ

The Australian Incident Monitoring Study. The capnograph: applications and limitations--an analysis of 2000 incident reports.

The first 2000 incidents reported to the Australian Incident Monitoring Study were analysed with respect to the role of the capnograph. One hundred and fifty-seven (8%) were first detected by a capnograph and there were a further 18 (1%) in which capnography was contributory. Of the 1256 incidents which occurred in association with general anaesthesia 48% were "human detected" and 52% "monitor detected". The capnograph was ranked second and detected 24% of these monitor detected incidents; this figure would have been nearly 30% if a correctly checked, calibrated capnograph had always been used

Biomonitoring exposure to metal compounds with carcinogenic properties.

Several metals such as arsenic, beryllium, chromium and nickel are carcinogenic to man when they occur in certain well-defined physicochemical forms. The carcinogenic potential of these metals is linked to their mutagenic properties. The determination of the metal or possibly of its metabolites in biological media and the cytogenetic examination of somatic cells are two methods that can currently be used to monitor exposure of populations at risk. Due to the use of inappropriate methodology, the value of the positive cytogenetic results published so far appears questionable. By contrast, the c

Management of raised intracranial pressure.

This review has been written at an unfortunate time. Novel questions are being asked of the old therapies and there is an abundance of new strategies both to lower ICP and protect the brain against cerebral ischaemia. In the United Kingdom, the problem is to ensure that appropriate patients continue to be referred to centres where clinical trials of high quality can be undertaken. One of the success stories of the past decade has been the decline in the number of road accidents as a result of seat belt legislation, improvements in car design and the drink/driving laws. Hence, fortunately there

Twelve hour overnight oesophageal pH monitoring in patients with reflux symptoms.

Results of continuous 12 hour overnight pH monitoring (duration of pH less than 4) were reviewed in 112 patients with heartburn or regurgitation, or both, and in 56 normal subjects. Patients had more reflux than normal subjects. Medically controlled patients (n = 51) had less acid reflux than patients who subsequently underwent reflux surgery (n = 61), but there was a considerable overlap between those two groups. Surgery was followed by a reduction in acid reflux to a value similar to that in normal subjects. Patients in whom surgery was deemed to have failed had more reflux after the operati

Stress and hypertension--a critical review.

Recurrent psychosomatic stress has been popularly thought to cause permanent hypertension by repeated reactive blood pressure elevations. These are considered by some to be caused by various mechanisms, including an increased sympathetic outflow and a decreased parasympathetic inhibition, or possibly an inborn functional abnormality in the walls of the resistance vessels, which becomes a structural abnormality as a result of the elevated pressure. Recent work, however, has questioned some of these concepts. Many of the assumptions in the past concerning the ill-effects of elevated blood pressu
