BACKGROUND: The purpose of this study is to establish the validity of F-18-deoxyglucose positron emission tomography-computed tomography (FDG-PET-CT) scan staging for cutaneous melanoma when a musculoskeletal image abnormality is detected.
METHODS: An institutional review board (IRB)-approved prospective database was queried to identify 342 melanoma patients treated between 4/1999 and 12/2007. A total of 682 whole-body FDG-PET-CT scans performed for staging were retrospectively reviewed to identify FDG-avid lesions in the deep soft tissues/muscle, bone or joints (i.e., musculoskeletal sites).

BACKGROUND: Knee joint pathologies/injuries are one of the most common musculoskeletal complaints in adults worldwide. The aetiologies of knee joint disorders are diverse. Magnetic resonance imaging (MRI) is a sophisticated method of detecting and characterizing knee pathologies. This study was conducted to document the clinical presentation and MRI patterns of knee joint abnormalities in a group of adults in Lagos, Nigeria, and to juxtapose it with reports from other climes.
METHODOLOGY: A retrospective hospital-based analysis of the knee MRI of 158 adult Nigerians was conducted in a single

Nail psoriasis is a common and clinically significant manifestation of psoriasis that may be difficult to recognize, quantify, and differentiate from other nail disorders. Onychoscopy has emerged as a non-invasive imaging technique that enables detailed visualization of nail matrix, nail-bed, and vascular abnormalities. This narrative review critically synthesized the current evidence on the diagnostic and clinical applications of onychoscopy in nail psoriasis. Literature search of PubMed/MEDLINE, Scopus, and Google Scholar was conducted, and the available evidence indicates that pitting, onyc

BACKGROUND: Voriconazole is frequently used to treat fungal infections in solid organ transplant patients. Recently, there have been reports suggesting that prolonged voriconazole therapy may lead to periostitis.
AIM: Here we present two cases of voriconazole-induced periostitis in solid organ transplant patients.
CASE PRESENTATION: Voriconazole was given to two transplant patients-one with a liver transplant and the second with a heart transplant, to treat their fungal infections. Both developed voriconazole-induced toxicity. While undergoing voriconazole therapy, they had incapacitating bo

Calcific tendinopathy most commonly affects the rotator cuff, whereas involvement of the patellar tendon is exceedingly rare. Persistent anterior knee pain following patellar fracture fixation is usually attributed to postoperative stiffness, hardware irritation, or altered biomechanics, potentially leading to underrecognition of concomitant tendon pathology. Musculoskeletal ultrasound is well suited for evaluating postoperative tendon disorders because it enables detailed assessment of tendon abnormalities while simultaneously depicting adjacent soft tissues and surgical implants. This presen

Singleton Merten Syndrome (SMS) is a rare autosomal dominant disorder caused by IFIH1 or DDX58 mutations, characterized by chronic inflammation leading to vascular calcifications, valvular disease, musculoskeletal abnormalities, dental dysplasia, skin findings like psoriasis and glaucoma. We report an 8-year-old boy with a DDX58 mutation associated SMS who presented with difficulty walking and ankle pain, found to have extensive aortoiliac and mesenteric artery narrowing. Advanced imaging with computed tomography (CT) and cinematic rendering provided detailed vascular mapping, underscoring the…

Ehlers-Danlos syndrome (EDS) comprises a group of rare connective tissue disorders marked by multisystem involvement and considerable clinical variability. This report presents two cases of young individuals with complex medical histories involving joint hypermobility, musculoskeletal deformities, ocular abnormalities, and unexplained cardiovascular symptoms. Both cases were initially misclassified under more common subtypes of EDS. However, comprehensive clinical evaluation and application of updated classification criteria led to a re-evaluation of the initial diagnoses. These cases highligh

Sarcoidosis is a chronic granulomatous disease characterized by non-caseating granuloma formation. It usually involves the lung, but may also affects many organs and systems such as the musculoskeletal system, eye, skin, and heart. The data on the course of sarcoidosis during pregnancy are controversial. Generally, sarcoidosis patients do not have a decrease in fertility, and there is no increase in the incidence of congenital abnormalities or premature birth. On the other hand, sarcoidosis may progress during pregnancy. In this report, we discussed the effects of pregnancy on the disease acti

Congenital knee dislocation (CKD) is a rare malformation characterized by hyperextension deformity of the knee with anterior tibia displacement, present at birth. Rarely reported, CKD might occur as an isolated deformity or commonly associated with musculoskeletal abnormalities, with the most common ones being developmental dysplasia of the hip (DDH) and clubfoot. The etiology is unknown, but CKD has been associated with certain intrinsic and extrinsic factors. Treatment with conservative methods at an early stage is most likely to yield successful results. We report here a rare case of succes…

Acromegaly is a rare condition characterized by excessive secretion of growth hormone (GH), which is almost always due to a pituitary adenoma. Acromegaly is associated with significant morbidity such as hypertension, type 2 diabetes, cardiomyopathy, obstructive sleep apnoea, malignancy and musculoskeletal abnormalities. Acromegaly has also been associated with increased mortality in several retrospective studies. This review will focus on the epidemiological data relating to mortality rates in acromegaly, the relationship between acromegaly and malignancy, the role of GH and insulin-like growt…

Loeys-Dietz syndrome (LDS) is an increasingly recognized autosomal-dominant connective tissue disorder with distinctive radiological manifestations, including arterial tortuosity/aneurysms, craniofacial malformations and skeletal abnormalities. LDS exhibits a more aggressive course than similar disorders, such as Marfan or the vascular subtype of Ehlers-Danlos syndrome, with morbidity and mortality typically resulting from complications of aortic/arterial dissections. Early diagnosis, short-interval follow-up imaging and prophylactic surgical intervention are essential in preventing catastroph

It is well known that Down syndrome (DS) affects multiple organ systems, including cardiovascular, gastrointestinal, hematological, and musculoskeletal. Involvement of the lungs by a characteristic form of subpleural cystic disease is well described in the pathology literature. The imaging characteristics of changes caused by this disease have not been previously described in detail in the radiology literature, possibly because they are not well appreciated on plain chest radiographs. We present a child with DS in whom extensive cystic lung disease was discovered incidentally at CT, and review

We describe the spectrum of clinical and histologic abnormalities of 11 women with L-tryptophan induced eosinophilia-myalgia syndrome. The illness is characterized by musculoskeletal symptoms including myalgias, arthralgias and paresthesias. The physical findings consist of muscle tenderness, neuropathies, rash, peripheral and periorbital edema. Electroneurography performed in 10 patients demonstrated a neuropathy in 5 and myopathic changes in 3. Skin and muscle biopsies showed fascial edema, inflammation and perivascular infiltrates in the skin, whereas perineural infiltrates and venulitis we

Cogan syndrome is a multisystem inflammatory vascular disease, characterized by nonsyphilitic interstitial keratitis and vestibuloauditory symptoms. Recent reports have directed attention to involvement of other organ systems. Respiratory, cardiovascular, gastrointestinal, and musculoskeletal problems are common, as are laboratory abnormalities and general symptoms such as fever, chills, and weight loss. Prominent neurologic problems in two patients prompted a review of 79 cases of Cogan syndrome. More than half had nervous system involvement, including electroencephalographic or spinal fluid

Musculoskeletal problems are commonly reported after stroke resulting in abnormal gait biomechanics, pain, and limitation in performing activities of daily living. Anterior circulate ligament is the most frequently injured knee ligament accounting for approximately 50% of all ligament injuries; however, post stroke anterior cruciate ligament injury is rarely reported. We present a case of a 58-year-old female admitted for inpatient stoke rehabilitation after a left middle cerebral artery stroke. After gaining considerable functional recovery, she was planned to be discharged in two weeks' time
