Nail disease in pemphigus vulgaris.

Patients affected by pemphigus vulgaris will occasionally present with associated and characteristic nail changes. This manuscript was prepa … estations reported were acute or chronic paronychia and onychomadesis, and involvement of the fingernails. The nail changes may precede or develop concurrently with the mucocutaneous presentation. The diagnosis can be made by histologic identification and/or by the presence of IgG and C3 with direct immunoflourescence testing. Systemic treatment is requ …

New tools in nail disorders.

Tumors of the nail unit may be difficult to diagnose because of the screening effect of the nail plate. In longitudinal melanonychia, several new promising techniques assist with early diagnosis of melanoma (in vivo matrix dermoscopy and immunohistochemistry) as well as sparing as much of the healthy tissues as is possible (shave biopsy technique). Diagnosing nail disorders is in some instances difficult both for the clinician and the pathologist. New tools such as polymerase chain reaction have been proposed for onychomycosis, which accounts for more than half of nail conditions, will allow q

[Fungal infections and nail psoriasis].

Onychomycosis and psoriasis are common diseases and it is possible that they coexist in the same patient. Psoriasis is a common cause of disturbance of the nail morphology. Nail disorders was reported in 10-80% of patients with psoriasis and may be associated with all clinical forms of disease. It has been suggested that the nail dystrophy in patients with psoriasis produce the lose of the natural protective barrier and increase vulnerability to fungal infections. But most authors report that in patients with psoriasis the prevalence of onychomycosis is no higher than in the general population

Interlocking nailing of forearm fractures.

AIM OF THE STUDY: The aim of this retrospective study is to evaluate the results after treating diaphyseal fractures of the radius and ulna with an interlocking intramedullary nail. MATERIALS AND METHODS: Between 2001 and 2005, 78 patients with 118 fractures were treated using the ForeSight intramedullary nail (Smith & Nephew, Memphis, USA). The average patient age was 37.02 years. In the postoperative period, early and late complications were evaluated through radiological and functional methods. The average period of follow-up was 25 months (range: 12-58 months). RESULTS: The average lengt

Diagnosis and management of nail pigmentations.

Longitudinal pigmentation of the nail is very common. The differential diagnosis varies from subungual hematoma, to a fungal infection, to a melanocytic lesion (lentigo, nevus melanoma, etc.) to others. Often, dermatologists do not feel at ease with these pathologies and management is often not clear. In many cases, a biopsy is not helpful because an inadequate technique was chosen. The use of noninvasive techniques such as dermoscopy has been described to be useful for the preoperative evaluation and the management decision. Using these technique, one will be able to reduce the number of unne

New fungal nail infections.

PURPOSE OF REVIEW: The number of people affected by onychomycosis continues to increase. The prevalence of different pathogens in different areas depends on several factors, such as climate, geography and migration. We reviewed the recent literature to identify new agents responsible for onychomycosis. RECENT FINDINGS: Recent studies performed in different countries are not only reporting molds and yeasts as contaminants, but are increasingly reporting them as pathogens. Infection by novel agents is also being reported, although the individual cases do not necessarily indicate that these are

Twenty nail dystrophy in vitiligo.

Twenty nail dystrophy (TND) is a manifestation of several dermatoses and its association with vitiligo is rare. It may be consequent upon a common autoimmune insult to the melanocytes and the nail matrix. We report a patient of acrofacial vitiligo who developed TND. Nail matrix biopsy revealed focal lichenoid reaction and chronic inflammatory infiltrate in the dermal papillae and around blood vessels. Hematological, biochemical and serological investigations showed no evidence of other autoimmune diseases.

Strain in the nail at fingertip compression.

PURPOSE: Distribution of nail strain due to compressive force in the finger pulp was investigated. METHODS: Nail strain of the right index finger of 10 healthy men was measured using three strain gauges with two axes. Each nail strain was measured along the longitudinal and the transverse axis using sensor interface. Three CCD cameras recorded the compressive force and the fingertip deformation. Each subject pressed the right index finger with 30 degrees, 45 degrees and 60 degrees in the contact angle, with the compressive force increased from 0 to 14 N. RESULTS: When the contact angle was 3

Nail pigmentation due to hydroxycarbamide.

A 47-year-old male developed both longitudinal melanonychia and periungual hyperpigmentation 6 months after initiation of hydroxycarbamide (synonym: hydroxyurea) therapy for chronic myeloproliferative disease. Based on the clinical symptoms observed in this patient, the broad differential diagnostic spectrum of hyperpigmentation of the nails is briefly reviewed here. In individuals who undergo hydroxycarbamide treatment, melanonychia might sometimes be seen as side effect and always has to be differentiated from subungual malignant melanoma.

Drug-induced nail diseases.

Although many nail disorders have been associated with drug intake, most reports are anecdotal. Most nail changes caused by drugs are the outcome of acute toxicity to the nail epithelia; nail symptoms depend on which nail structure is damaged. The most com-mon symptoms include Beau's lines/onychomadesis, melanonychia, onycholysis, and periungual pyogenic granulomas. Drug-induced nail abnormalities are usually transitory and disappear with drug withdrawal, but sometimes persist over time. This article reviews drugs that have been consistently associated with nail abnormalities.

[Familial nail patella-syndrome].

Nail patella-syndrome is a rare hereditary (autosomal dominant) disorder resulting from a heterogenous loss of function in the LMXB1 gene on chromosome 9q34. It is associated with multiple deformities. Patients have a characteristic tetrad of pathologic symptoms including fingernail dysplasia, hypoplastic or absence patella, radial head dislocation, iliac horns and in some cases nephropathy and ophtalmo-logical findings (glaucoma). In this study four affected members in one family are presented. All of the familly members presented absence or hypoplastic patella and fingernails dysplasia. Ther…
