Metastatic involvement of level IIb nodal station in oral squamous cell carcinoma: A clinicopathological study.

BACKGROUND: The purpose of this study was to determine the prevalence of Level IIb metastasis in patients with oral squamous cell carcinomas (OSCCs). MATERIALS AND METHODS: A total of 110 newly diagnosed oral cavity cancer patients requiring surgery as the primary modality were included in the study. Preoperative clinical examinations were done and tumor-node-metastasis staging was noted. Intraoperatively, Level IIb nodal tissue was dissected and sent separately. RESULTS: A total of 129 neck dissections (58 SOHD, 67 modified neck dissections, and 4 radical neck dissections) were carried out …

Patterns of lymph node metastases to the submuscular recess.

OBJECTIVE: To determine the incidence of lymph node metastases in the submuscular recess (level 2B) in selective neck dissections. The goal was to evaluate the oncologic need for dissection in this area owing to the increased morbidity from devascularization of the spinal accessory nerve. METHODS: A prospective analysis of 74 patients who underwent 80 neck dissections from July 2002 to March 6, 2004, was undertaken. Seventy-six of the 80 neck dissections were performed in patients with squamous cell carcinoma, whereas the remainder were done for papillary carcinoma of the thyroid. All submusc…

Nodal disease in purely glottic carcinoma: is elective neck treatment worthwhile?

OBJECTIVE: Although there is a generalized understanding of the relatively low overall incidence of nodal disease from purely glottic carcinoma, the exact role for elective neck treatment in the management of this disease remains controversial. The purpose of this study was to identify the incidence of occult nodal disease (including paratracheal) in patients who have glottic carcinoma without significant extraglottic extension and to identify which patients are at risk for this. A retrospective chart review of 92 such patients who had either undergone neck dissection or been observed for a mi…

[Salvage surgery for neck recurrence or residue of nasopharyngeal carcinoma after primary radiotherapy].

OBJECTIVE: To investigate the surgical procedures and prognosis for neck recurrence or residue of nasopharyngeal carcinoma(NPC) after primary radiotherapy. METHODS: A total of 153 cases with neck recurrence or residue after radiotherapy in NPC who received salvage neck surgery between January 2001 and December 2011 were retrospectively analyzed. There were rN1 109 cases, rN2 17 cases and rN3 27 cases. Of them 17 cases received bilateral neck dissection(ND) simultaneously. The surgical procedures included the modified radical neck dissection (MRND) in 66 cases, radical neck dissection (RND) in…

Preliminary multi-institutional prospective pathologic and molecular studies support preservation of sublevel IIB and level IV for laryngeal squamous carcinoma with clinically negative neck.

Level II-IV selective neck dissection, often performed bilaterally, has become the procedure of choice for elective dissection of the clinically negative (N0) neck in the treatment of laryngeal cancer. The most significant morbidity of this procedure is dysfunction of the accessory nerve, incurred by the necessity of mobilization and retraction of the nerve in order to remove the contents of sublevel IIB. Other morbidity includes possible injury to the phrenic nerve and chylous fistula. These complications are associated with the dissection of level IV. A number of prospective multi-institutio…

Surgical management of the neck in patients with metastatic melanoma in parotid lymph nodes.

BACKGROUND: The role and extent of neck dissection in patients with parotid metastatic cutaneous head and neck melanoma remain unclear. The aims of this study were to determine the incidence and patterns of cervical node involvement in patients with parotid metastatic melanoma, and to determine if a limited lymphadenectomy of the clinically negative neck is appropriate. METHODS: Patients who underwent parotidectomy and neck dissection for clinically apparent parotid metastatic melanoma, irrespective of neck status, were identified from two prospectively maintained databases. RESULTS: A total…

Retrospective observational study of occult cervical lymph-node metastasis in T1N0 tongue cancer.

OBJECTIVE: Delayed neck metastasis is the most significant prognostic factor for early tongue cancer. The main strategies for controlling cervical lymph nodes in Japan are elective neck dissection or watchful waiting. Elective neck dissection offers significantly better survival, but adversely impacts patient quality of life; consequently, here we investigated how to identify high-risk patients warranting elective neck dissection. METHODS: We retrospectively evaluated 67 patients with T1N0 oral tongue squamous cell carcinoma who underwent primary treatment in our department from April 2001 to…

Lymphatic drainage patterns of head and neck cutaneous melanoma: does primary melanoma site correlate with anatomic distribution of pathologically involved lymph nodes?

The aim of this study was to analyse patterns of metastatic spread from cutaneous head and neck melanoma, which are said to be highly variable. The medical records of 145 patients with pathologically proven metastatic melanoma were reviewed retrospectively. The location of pathologically positive lymph nodes was compared with clinically predicted spread, and patients with metastatic disease in areas outside of predicted drainage patterns were considered aberrant. There were 33 curative and 73 elective neck dissections. 21 of 77 patients undergoing parotidectomy had positive results for metasta…

Fatal Vertebral Artery Dissection Following Self-Manipulation of the Cervical Spine.

Cervical spine manipulation is a known risk factor for vertebral artery dissection. In this report, we present a case of fatal vertebral art … of cerebellar and brainstem infarcts, herniation, and diffuse hypoxic-ischemic injury. A posterior neck dissection was performed to expose the vertebral arteries, which showed grossly visible hemorrhage and dilation. There was no evidence of traumatic injury to the bone or soft tissue of the head or neck. Bilateral dissection tracts were readily appreciated on m …

Is it possible to intraoperatively modulate the extent of thyroidectomy in small papillary thyroid carcinoma?

BACKGROUND: Thyroid lobectomy is the preferred option for small, unifocal papillary thyroid carcinoma. Involvement of the central neck lymph nodes is an indication for total thyroidectomy plus central neck dissection. We aimed to verify if frozen section examination of ipsilateral central neck nodes can identify the subgroup of patients scheduled for thyroid lobectomy intraoperatively who could benefit of more extensive initial operative treatment. METHODS: Ninety-four consenting patients with clinically unifocal cN0 papillary thyroid carcinoma underwent thyroid lobectomy plus ipsilateral cen…

Rare anatomical variant of the cervical internal carotid artery.

Neck dissection is routine for the management of cancers of the head and neck. Knowledge of anatomical variants in the carotid artery system at various levels of dissection should prevent inadvertent injury to major blood vessels, which can cause massive bleeding. We report a rare 180° sharp bend in the cervical course of the internal carotid artery at level II, which we found during a modified neck dissection.

Anatomical diversity of inferior belly of the omohyoid muscle - Anatomical, physiological and surgical paradigm.

Omohyoid muscle is one of the infrahyoid muscles of the neck which consists of two bellies combined at an angle by an intermediate tendon. The inferior belly is a flat, narrow band, which inclines forwards and upwards in the lower part of the neck. It generally originates from the upper border of the scapula, medial to scapular notch. The present case showed unilateral anomalous attachment of the inferior belly of the omohyoid on the medial part of clavicle on left side. Inferior belly was 2.2cm lateral to left sternoclavicular joint with 3.2 and 1.5cm in length and breadth, innervated by ansa
