Tumor-induced osteomalacia and symptomatic looser zones secondary to mesenchymal chondrosarcoma.

Tumor-induced osteomalacia is a rare clinical entity that is associated with soft-tissue or skeletal tumors. We present a case report of a patient with a chest wall mesenchymal chondrosarcoma who presented with bone pain. The patient had skeletal changes in the femoral neck and fibula consistent with osteomalacia and laboratory values suggesting phosphate diabetes. The patient was treated with tumor resection and phosphate supplementation with reversal of the signs and symptoms of osteomalacia. Tumor-induced osteomalacia is vitamin-D-resistant and often reversed by complete removal of the tumo

[Tumoral hypophosphoremic osteomalacia].

Various tumoral diseases may induce hypophosphataemic osteomalacia. This is the case, in particular, with mesenchymal tumours and sclerosing metastases of prostatic cancer; much less common causes are diffuse connective tissue dysplasias and non-prostatic epitheliomas. Depending on whether osteomalacia results from a mesenchymal tumour or from osteitis of prostatic origin, the practical problems encountered are quite different. In the first case, the important point is not to miss a mesenchymal tumour when confronted with an acquired and apparently idiopathic hypophosphataemia; repeated examin

Intra-arterial infusion of bromodeoxyuridine and radiotherapy in osteosarcoma and other bone malignancies.

In order to avoid amputation, which does not seem to improve survival in osteosarcoma, we have initiated a limb-preservation program using intra-arterial radiosensitization. Eleven osteosarcomas with soft tissue involvement and six other bone malignancies were prospectively treated according to the following protocol: (a) surgical insertion of an intra-arterial catheter through a Dacron patch, (b) intra-arterial infusion of 15 mg/kg BUDR on days 1-2 every 5 days for 40 days, (c) 600 rads flash irradiation on day 3 every 5 days, X 8. Median follow-up time has been 36 months. In the osteosarcoma

[Superselective arterial tumor angiography and chemotherapy. Preliminary results (author's transl)].

In 7 patients with bone and soft tissue sarcoma super-selective angiography with chemotherapy was performed. Both showed a low rate of complications. In 2 patients we saw a total and in 3 a subtotal tumor-regression. A residual tumor was eliminated after systemic chemotherapy. Beside the tumor vessels treated in the first session, we found other initially unimportant ones. These showed increased size during subsequent angiography and allowed further injection of chemotherapeutic agent into the residues of the tumor. We found that each part of the tumor has its own separate blood supply.

A Cost-Effective Strategy for Mandibular Reconstruction: Fibula Flap With Existing Titanium Plates.

With the rapid advancement of digital and microsurgical techniques, immediate autologous bone grafting following resection of benign and malignant mandibular tumors has become a standard treatment. However, some patients are unable to undergo immediate autologous bone grafting due to various reasons. In such cases, titanium plate implantation (or combined with soft tissue free flap such as anterolateral thigh flap) serves as an alternative to prevent airway compression and maintain facial shape and function. This article reports a case of a patient, referred to as patient 1, who experienced a

Intraarticular calcifying aponeurotic fibroma of the wrist: mimicking gout or calcium pyrophosphate dihydrate deposition disease.

Calcifying aponeurotic fibroma is a rare, benign fibroblastic tumor that typically occurs in the palms of the hands and soles of the feet in children and adolescents. We report an unusual case of a calcifying aponeurotic fibroma with diffuse intra-articular involvement of the carpal joints in a 59-year-old female. Radiographs and computed tomography scans revealed a large lobulated soft tissue mass with multiple stippled calcifications around the carpal joints and numerous erosions of the second to fifth carpometacarpal and intercarpal joints. Magnetic resonance imaging showed diffuse multinod

Malignant chondroblastoma of the rib with scalp metastases in an adult patient: case report with molecular characterization.

Chondroblastomas represent less than 1% of all bone tumors. They are currently classified as benign neoplasms and "benign lung implants" have been described. Exceedingly rare case of malignant chondroblastomas have been published, and only three of them have scalp metastases. We present the case of a 57-year-old healthy man who noticed a rapidly growing mass on his left chest. Radiological imaging revealed an 8 cm expansile and lytic lesion located in the anterior segment of the sixth costal arch with a thinned cortex without periosteal reaction. The tumor was removed with wide margins. Seven

Mortality from second tumors among long-term survivors of retinoblastoma.

BACKGROUND: Children diagnosed with retinoblastoma, a rare cancer of the eye, tend to develop and die of second primary cancers in childhood and adolescence, but few investigations have followed patients into adulthood. Retinoblastoma is frequently caused by inherited mutations of the RB1 tumor suppressor gene. Most patients with germline (hereditary) mutations have bilateral disease. PURPOSE: We sought to quantify the mortality from second malignancies among long-term survivors of retinoblastoma and to identify factors that predispose to these deaths. METHODS: A retrospective cohort study e

The use of free flap for limb salvage in children with tumors of the extremities.

BACKGROUND: The purpose of this study was to evaluate long-term functional outcomes in pediatric oncology patients who underwent limb salvage using free flaps. METHODS: All 22 pediatric oncology patients treated with a free flap for extremity salvage were included in the study from 1999 to 2008. RESULTS: The median patient age was 13.5 years. All but one patient had sarcoma, which involved lower extremity in 45% and upper extremity in 55%. The median bone defect length was 13.9 cm: reconstructed with vascularized free fibula in 68% (in 23% osseous allograft was also used) and used fibula gro

Giant Soft Tissue Leiomyosarcoma of the Left Lower Extremity: Case Presentation With a Review of the Literature.

Leiomyosarcoma (LMS) accounts for approximately 5-10% of soft tissue sarcomas, with an estimated incidence in the United States (US) of less than one case/200,000 persons, more frequent in women than men. Approximately two-thirds of LMSs are retroperitoneal, abdominal, and mediastinal. Localized, soft tissue LMSs represent a lower percentage, with the lower limbs and trunk being the most frequently involved sites. LMSs larger than 5 cm (so-called giants) are even rarer, and to date have been little reported in the literature. In this paper, we present the case of a giant LMS of the left lower

Ponticulus posticus is a frequent radiographic finding on lateral cephalograms in nevoid basal cell carcinoma syndrome (Gorlin-Goltz syndrome).

UNLABELLED: Nevoid basal cell carcinoma syndrome (NBCCS) is a predisposition to a rare tumor type with a variable phenotype. Besides tumors, skeletal alterations, such as bifid ribs or frontal bossing constitute the phenotype. Recently, a variant of the first cervical vertebra, the ponticulus posticus, was reported to occur in 50% of patients with NBCCS as revealed by analysis of lateral cephalograms. MATERIALS AND METHODS: Lateral cephalograms of eight patients with NBCCS were studied for the presence of ponticulus posticus. RESULTS: The ponticulus posticus was present in all patients. In o

Giant nevi: a review.

Congenital nevi are benign neoplasms that are present at birth and composed of nevomelanocytes. Approximately 1-3% of all newborns have congenital pigmented nevi, and the number of nevi increases with age, peaking by late adolescence to 20-40 nevi in an individual. Giant congenital nevi are often defined as nevi that are greater than 20 cm in diameter in an adult, or nevi that occupy 2% or more of the body surface area. Histologically, nevi are transformed melanocytes, which are normally highly dendritic cells interspersed among basal keratinocytes. The genetic basis of these lesions is not kn

"Cyst" on the forearm of a 28-year-old female: Case report of a CIC-rearranged sarcoma.

Capicua transcriptional repressor (CIC)-rearranged sarcomas are part of the group of Ewing-like sarcomas or atypical Ewing sarcomas which, thanks to the progress in molecular diagnosis, are being defined by particular genetic abnormalities separating this group into distinct entities with their own particular histological and immunohistochemical features, as well as different survival outcomes. We report the case of a healthy 28-year-old female presenting with a tender lesion on her forearm which after ultrasound examination was clinically favored to represent an infected sebaceous cyst. Myeloid Sarcoma.

Hematological malignancies can manifest as extramedullary soft tissue masses in relatively rare cases. The rarity of it causes a diagnostic and therapeutic challenge. One of the rarest manifestations is myeloid sarcoma (MS). MS develops as part of acute myeloid leukemia, myeloproliferative neoplasm, or myelodysplastic syndrome or at relapse, especially following allogeneic hematopoietic stem cell transplant. The tumor displays high myeloperoxidase expression, hence the color green, and is called chloroma. It most commonly appears in lymph nodes, skin and soft tissues, bone, testes, gastrointes

Chronic lymphocytic leukemia/small lymphocytic lymphoma involving the aortic valve.

Chronic lymphocytic leukemia/small lymphocytic lymphoma is a neoplasm composed of monomorphic small B lymphocytes in the peripheral blood, bone marrow, spleen, and lymph nodes, forming proliferation centers in tissue infiltrates (Muller-Hermelink HK, Montserrat E, Catovsky D, et al. Chronic lymphocytic leukaemia/small lymphocytic lymphoma, in Swerdlow SH (ed). WHO Classification of Tumours of Haematopoietic and Lymphoid Tissues. LHRH receptor expression in sarcomas of bone and soft tissue.

AIM: Luteinizing hormone releasing hormone (LHRH) is a neurohormone, secreted by the hypothalamus, which regulates the secretion of gonadotropins, luteinizing hormone (LH) and follicle stimulating hormone (FSH) from the pituitary. LHRH acts by binding to receptors located in the pituitary gland. These receptors (LHRH receptors) have also been found in the cytoplasm of many tumor cells that involve both the reproductive and non-reproductive organs. These receptors have been demonstrated in prostate and breast cancers, endometrial carcinomas, renal cell carcinoma, lymphoma, carcinoma of liver, p

Atypical bone metastasis and radiation changes in a colon cancer: a case report and a review of the literature.

We report a unique case of skeletal metastasis with prominent soft tissue extension from a colon cancer in a 44-year-old male patient. Four years after the diagnosis of colon cancer, plain radiographic examinations revealed a prominent soft tissue mass associated with cortical destruction of the right femur. Palliative radiotherapy of the right femur was performed. After completing radiotherapy, massive remineralization was seen in the soft tissue component. With the assistance of pre- and post-radiotherapy computerized tomography images, obtained in the process of planning the radiotherapy, w

Hyoepiglottic ligament in supraglottic cancer.

The hyoepiglottic ligament (HL) is a connective tissue structure that serves as the roof of both the paraglottic and the preepiglottic spaces and thereby anatomically separates the supraglottic larynx from the tongue base. Whole mount serially sectioned larynges with supraglottic cancer were reviewed to help clarify cephalad spread of cancer in this region. The whole mount slides were analyzed from 70 laryngectomy specimens that were resected for supraglottic cancer. The HL was breached by cancer in 13 specimens, and all of these displayed clinical and histopathologic invasion of the preepiglo
