Shockwave lithotripsy and endourological management of urinary calculi in children: a single-center 10-year experience.

PURPOSE: Shock wave lithotripsy (SWL) and endourological techniques revolutionized the management of pediatric urolithiasis. We sought to assess the impact of new technology and local practice in the treatment of pediatric urolithiasis during a 10-year period. MATERIALS AND METHODS: Between 1997 and 2006, 125 children (90 boys and 35 girls), aged 18 months to 15 years, were managed in our department for urolithiasis. Stone localization, stone composition, presence of anatomic abnormalities, and treatment modality were evaluated retrospectively. RESULTS: In 102 children, the stone was located

Urolithiasis in kidneys with abnormal lie, rotation or form.

PURPOSE OF REVIEW: The treatment options in urolithiasis in kidneys with abnormalities of form and location can be challenging due to abnormal anatomy (calyceal and renal orientation), relative immobility interfering with movement of equipment, and abnormal relation with other visceral organs. RECENT FINDINGS: In this review, we focus on the different techniques and results of various treatment modalities. The approach to managing these stones should be individualized. We also allude to the results of a few recent series and emphasize various treatment options. SUMMARY: Ultrasound helps in g

Anesthesia for pediatric lithotripsy.

SUMMARY BACKGROUND: We describe 7 years experience of providing anesthesia for children undergoing lithotripsy in a hospital without pediatric inpatient facilities. A pediatric team, including anesthetist, pediatric nurse along with the equipment travel across the city with the patient from the children's hospital. As a high incidence of postoperative vomiting and discomfort was observed, the effect of increasing intraoperative analgesia and the use of antiemetic medication was examined. METHODS: From 1998 to 2004, 69 children (49 boys and 20 girls) were anesthetized for 120 procedures: 67 ex

[FEATURES OF PERCUTANEOUS NEPHROLITHOTRIPSY IN PATIENTS WITH UROLITHIASIS OF SOLITARY KIDNEY].

The article describes the results of percutaneous nephrolithotomy (PCNL) in 49 urolithiasis patients with a solitary kidney. In 44 (89.8%) patients the calculi were located in the kidney, in 5 (10.2%) patients - in the upper third of the ureter. The calculi were single (18; 36.7%), multiple (14; 28.6%) or stag-horn (17; 34.7%). Efficiency of PCNL estimated immediately after surgery and at the end of the third month of observation was 75.5 and 93.9%, respectively. Stone size (p=0.594), the baseline state of urodynamics of the upper urinary tract (p=0.205) did not affect the renal clearance at P

Ultrasonography-guided punctures-with and without puncture guide.

The key requisite of any percutaneous nephrolithotomy technique is access to the collecting system. The kidney has a high degree of vascular network and is liable for vascular injury. Therefore, for an ideal puncture, a percutaneous tract would be developed that leads straight from the skin through a papilla and the target calix into the renal pelvis. Percutaneous renal access can be achieved under fluoroscopic control or using an ultrasonography (US)-guided puncture. The shortcomings and side effects of extensive radiation during therapeutic procedures are well known. The choice of method for

Delayed presentation of retained renal shrapnel: a case report and literature review.

Retained renal foreign bodies (FBs) following blast or firearm injury are rare and usually present early; delayed presentation is a challenging diagnostic scenario. The present report describes a case of retained renal shrapnel presenting with obstructive complications 18 years after the initial injury. Imaging revealed a 24 mm calculus encrusting a retained metallic FB at the left renal hilum, with severe hydronephrosis and reduced differential renal function (27.4%). Following ureteroscopic stenting, percutaneous nephrolithotomy was performed. The surrounding calculus was fragmented, but the

Case Report: A rare case of intestinal and mucinous-type renal pelvis adenocarcinoma masked by complex renal calculi: a diagnostic dilemma and therapeutic challenge.

Primary renal pelvis adenocarcinoma (RPA), particularly the mucinous subtype, is an exceedingly rare and aggressive malignancy often associated with chronic inflammation and long-standing calculi, making its diagnosis challenging due to non-specific symptoms mimicking common urological conditions. We present a 79-year-old male patient with a decade-long history of complex nephrolithiasis and recurrent infections, who underwent multiple percutaneous nephrolithotomy (PCNL) procedures. A pivotal diagnostic moment arose during a subsequent PCNL when extensive "purulent moss" was found without sign

Laparoscopic management for a psoas abscess caused by migrated urolithiasis.

INTRODUCTION: To describe laparoscopic surgery for psoas abscess caused by migrated urolithiasis. CASE PRESENTATION: A 64-year-old female had renal stones in the right kidney for 5 years. She developed right back pain. Her body temperature was 37.4°C, and right costovertebral angle tenderness was detected. In blood examination, her C-reactive protein level was elevated. Computed tomography revealed that one stone had migrated into the right psoas muscle and caused psoas abscess. Another stone was detected in the renal parenchyma. Percutaneous drainage and antibiotic treatment were performed u

Ureteroscopy-assisted retrograde nephrostomy (UARN) without ureteral access sheath (UAS).

INTRODUCTION: We previously described ureteroscopy assisted retrograde nephrostomy (UARN). In UARN, it is possible to continuously visualize the dilation of the ureter from puncture to insertion of the nephroaccess sheath with minimal complication. But in the course of making nephrostomy, UARN requires ureteral access sheath (UAS). UAS has a potential risk of ureteral stricture. Herein, we report the first case of UARN without the use of UAS. PRESENTATION OF CASE: A 53-year-old female was referred to our hospital for treatment of her right renal stone. Because her stone burden was 27mm, we pl

When (and how) to surgically treat asymptomatic renal stones.

Asymptomatic renal stones are identified in 8-10% of screened populations. With the increasing utilization of CT, the number of patients seeking urologic care for incidentally diagnosed renal calculi is likely to increase. Such patients present an important management dilemma: differentiating those to treat surgically from those who can be safely observed. Observational studies have revealed that approximately 50% of asymptomatic stones will progress, but most will not require surgery. Stones >15 mm in diamater and located in the renal pelvis are at highest risk of progression. Although no gui

Saturation effect between operation time and post-PCNL SIRS in pediatric patients.

BACKGROUND: Prolonged operation time has been associated with an increased incidence of postoperative complications in various surgical procedures. However, its role in pediatric percutaneous nephrolithotomy (PCNL) remains unclear. This study aimed to investigate the relationship between operation time and systemic inflammatory response syndrome (SIRS) incidence in pediatric patients undergoing PCNL. METHODS: This retrospective study included 472 pediatric patients who underwent PCNL, the primary exposure of interest was operation time, while the primary outcome was the incidence of postopera
