Hemophilia B With Intracranial Hemorrhage Rehabilitation in High-Dependency Unit: A Case Report.

Hemophilia B, an X-linked recessive coagulation disorder, poses significant risks of life-threatening intracranial hemorrhage (ICH). This case report details the multidisciplinary rehabilitation of a 41-year-old male with moderate hemophilia B (FIX activity: 3.9%) and ICH in a resource-limited setting. Admitted to a high-dependency unit postneurosurgical intervention, the patient received low-dose prophylactic coagulation factor IX (maintained at 34.6%-66.2%) alongside real-time coagulation monitoring. A stepwise rehabilitation protocol was implemented, including early passive joint mobilizati

Myasthenia Gravis - An Updated Review.

Myasthenia gravis (MG) is an autoimmune neuromuscular junction (NMJ) disorder characterized by fatigable weakness in skeletal and ocular muscles. The etiology is the presence of autoimmune antibodies against postsynaptic acetylcholine receptors (AChR) or other muscle-specific proteins. The result is muscle end plate dysfunction and weakness. While symptoms may be limited to ocular muscles (ocular MG/OMG), systemic generalization of symptoms (generalized MG/gMG) can frequently occur. Diagnosis relies on a history of fluctuating weakness, diurnal variation in muscle strength, physical examinatio

Serum protein biomarker profile distinguishes acetylcholine receptor antibody seropositive myasthenia gravis patients from healthy controls.

There is an unmet need for objective disease-specific biomarkers in the heterogeneous autoimmune neuromuscular disorder myasthenia gravis (MG). This cross-sectional study identified a signature of 23 inflammatory serum proteins with proximity extension assay (PEA) that distinguishes acetylcholine receptor antibody seropositive (AChR+) MG patients from healthy controls (HCs). CCL28, TNFSF14, 4E-BP1, transforming growth factor alpha (TGF-α), and ST1A1 ranked top biomarkers. TGF-β1 and osteoprotegerin (OPG) differed between early- and late-onset MG, whereas CXCL10, TNFSF14, CCL11, interleukin-17C

Cardiovascular autonomic dysfunction induced by mechanical insufflation-exsufflation in Guillain-Barré syndrome.

Mechanical insufflation-exsufflation (MI-E) is an effective airway clearance device for impaired cough associated with respiratory muscle weakness caused by neuromuscular disease. Its complications on the respiratory system, such as pneumothorax, are well-recognized, but the association of the autonomic nervous system dysfunction with MI-E has never been reported. We herein describe two cases of Guillain-Barré syndrome with cardiovascular autonomic dysfunction during MI-E: a 22-year-old man who developed transient asystole and an 83-year-old man who presented with prominent fluctuation of bloo

Proposal of a new clinical protocol for evaluating fatigability in adult SMA patients.

OBJECTIVE: Spinal Muscular Atrophy (SMA) is a genetic neuromuscular disease affecting the lower motor neuron, carrying a significant burden on patients' general motor skills and quality of life, characterized by a great variability in phenotypic expression. As new therapeutic options make their appearance on the scene, sensitive clinical tools and outcome measures are needed, especially in adult patients undergoing treatment, in which the expected clinical response is a mild improvement or stabilization of disease progression. METHODS: Here, we describe a new functional motor scale specifical

Fetal Surveillance in Pregnancies with Myasthenia Gravis.

Myasthenia gravis (MG) is an autoimmune condition, that commonly impacts adult women of reproductive age. Myasthenia gravis in pregnancy is rare, but the incidence is higher in different geographical areas. Pregnancies in mothers with MG can have an unfortunate outcome. Acetylcholine receptor antibodies may pass into the fetal circulation and can affect the fetal neuromuscular junction, generating transient MG or even fetal arthrogryposis. The 2016 and 2021 International Consensus Guidance for Management of Myasthenia Gravis issued by Myasthenia Gravis Foundation of America is lacking in recom

Low serum vitamin D levels in patients with myasthenia gravis.

Myasthenia gravis (MG) is a chronic autoimmune neuromuscular disease. Vitamin D has important roles both in the autoimmune response and in skeletal muscles. We investigated the levels of 1,25-dihydroxy vitamin D [1,25(OH)2D] and 25-hydroxy vitamin D [25(OH)D] in patients with MG and healthy subjects. MG patients were classified by disease stage, age of onset and treatment status whether or not to taking immunosuppressive agents. MG patients had lower plasma 25(OH)D levels (mean, 18.8 ± 8.4 ng/mL) than healthy controls (26.3 ± 6.1 ng/mL) (p < .05). 1,25(OH)2D levels showed slightly high in MG p

Obstructive sleep apnoea syndrome in children and anaesthesia.

Obstructive sleep apnoea syndrome (OSAS) is a common medical disorder among adults, which is increasingly being recognized in children too. It is a breathing disorder characterized by upper airway obstruction with or without intermittent complete obstruction that disrupts normal breathing during sleep. Anatomical and neuromuscular disorders are mainly responsible for this disorder. This disorder leads to a state of chronic hypoxemia, which has significant cardiac, pulmonary and central nervous system implications. Diagnosis of OSAS is based on thorough history and clinical examination along wi

The treatment of adrenocortical carcinoma with o,p'-DDD: prognostic implications of serum level monitoring.

Thirty-four patients with adrenocortical carcinoma were treated with o,p'-DDD. Twenty-eight patients presented with metastases at entry, and spillage of tumour cells occurred at surgery in 6 other patients. Eight patients had objective tumour regression, of whom 7 had serum levels over 14 micrograms/ml. The 3 patients with a lasting remission had levels of greater than 15, greater than 25, greater than 25 micrograms/ml respectively during prolonged periods. Increased survival times were found in the group of 14 patients with o,p'-DDD serum levels higher than 14 micrograms/ml when compared with

Anesthetic Challenges in Debulking Surgery of Massive Plexiform Neurofibroma With Neurofibromatosis Type 1: A Case Report of Intraoperative Generalized Involuntary Movements and Severe Blood Loss.

Neurofibromatosis type 1 is a multisystem genetic disorder commonly associated with plexiform neurofibromas, benign but highly vascular tumors that present significant perioperative challenges. Surgical debulking of plexiform neurofibromas is often associated with difficult anesthetic management due to tumor vascularity, hemorrhage risk, airway considerations, and the need for intraoperative neurophysiological monitoring. We report the case of a 48-year-old man with neurofibromatosis type 1 who underwent debulking of a large right thigh plexiform neurofibroma under general anesthesia without n

Delayed Emergence Due to Severe Respiratory Acidosis Following Prolonged Spine Surgery: A Case Report.

Delayed emergence from anesthesia is a frequent challenge in surgical settings. Common etiologies include residual drug effects, metabolic derangements, or neurological insults. We report a case of a 55-year-old woman who underwent an elective six-hour lumbar laminectomy and cervical implantation surgery. Despite an uneventful intraoperative course and routine neuromuscular blockade reversal, the patient exhibited somnolence and ventilatory failure after reversal and extubation. Arterial blood gas (ABG) analysis revealed severe respiratory acidosis. The patient required re-intubation and contr

Negative-pressure pulmonary oedema following upper airway obstruction after tracheal extubation in an adolescent undergoing general anaesthesia for dental extractions.

A healthy athletic adolescent male with a remote history of asymptomatic childhood asthma underwent outpatient multiple molar extractions under general anaesthesia with tracheal intubation. Shortly after gentle tracheal extubation, he developed acute upper airway obstruction with oxygen desaturation and increased work of breathing, despite careful oropharyngeal aspiration and confirmed reversal of neuromuscular blockade. Initial airway manoeuvres and face-mask ventilation with 100% oxygen against high airway resistance provided only transient improvement, requiring tracheal re-intubation and p

Severe Prolonged Hypocalcemia After Denosumab Administration in a Patient With Sleeve Gastrectomy: A Case Report.

Denosumab is a human monoclonal antibody that inhibits RANKL, thereby suppressing osteoclast-mediated bone resorption and increasing bone mass in patients with osteoporosis. Although generally well tolerated, denosumab is associated with hypocalcemia, particularly in individuals with impaired calcium and vitamin D homeostasis. We report the case of a 40-year-old woman with a history of sleeve gastrectomy who developed severe, symptomatic hypocalcemia following a single dose of denosumab despite receiving vitamin D supplementation. She presented with neuromuscular symptoms, QTc prolongation, pr

Non-linear measures of movement variability in multiple sclerosis: a clinical narrative review of Lyapunov exponent and entropy applications in balance and gait.

Human movement variability, the natural fluctuation in motor performance across repeated tasks, is a fundamental characteristic of healthy biological systems, and its alteration is a hallmark of neurological dysfunction. The use of non-linear measures provides a powerful suite of complementary tools for capturing the complexity of this variability, revealing patterns in motor control that traditional linear metrics, based on time and distance, often miss. By quantifying aspects such as stability, adaptability, and the predictability of movement, these methods provide critical insights into neu

Evaluation of the Performance of New Fluorescence Immunoassay POCTs for Determining the Value of Vitamin D in Whole Blood.

VD (VD), a hormone-like, fat-soluble molecule, is essential for several biological processes, such as gene regulation, calcium balance, bone health, immune function, antiviral defense, and neuromuscular activity. Its deficiency is associated with various disorders, including chronic hypocalcemia and increased risk of severe diseases, such as COVID-19. Monitoring 25-hydroxyVD (25-OH-D) levels is vital, with serum 25-OH-VD being the standard marker. While chromatography and immunometric assays are well-established, innovative point-of-care (POC) platforms like AFIAS-1® (Boditech & Menarini, Gang
