BACKGROUND: Cervical compressive myelopathy (CCM) is a progressive, degenerative spine disease and the most common cause of spinal cord dysfunction in older individuals. Current clinical guidelines for spinal surgery recommend multimodal intraoperative monitoring (IOM) during spinal surgery as a reliable and valid diagnostic adjunct to assess spinal cord integrity. The aim of this study was to evaluate the effect of positive changes during IOM on the functional status in patients with CCM.
METHODS: Patients who underwent spinal surgery with IOM due to CCM were enrolled. During the surgery, pa

Microelectrode recording (MER) is used to confirm electrophysiological signals within intended anatomic targets during deep brain stimulation (DBS) surgery. We describe a novel technique called intraoperative CT-guided extrapolation (iCTE) to predict the intended microelectrode trajectory and, if necessary, make corrections in real-time before dural opening. Prior to dural opening, a guide tube was inserted through the headstage and rested on dura. Intraoperative CT (iCT) was obtained, and a trajectory was extrapolated along the path of the guide tube to target depth using targeting software.

OBJECTIVE: To validate the findings of preoperative motor short-segment nerve conduction studies (SSNCSs) by intraoperative SSNCSs in patients with cubital tunnel syndrome.
METHODS: We prospectively recruited patients with ulnar neuropathy at the elbow (UNE) localized distal to the medial epicondyle (ME). Preoperatively, motor SSNCSs and ultrasonography (US) were performed. Immediately after surgical dissection of the humeroulnar aponeurotic arcade (HUA), intraoperative near-nerve motor SSNCSs were performed, and compared to preoperative findings.
RESULTS: We studied 36 arms with UNE in the

BACKGROUND: Footdrop secondary to L5 root injury is a rare complication associated with lumbar surgery. It is unclear whether intraoperative neuromonitoring can detect such an injury.
CASE DESCRIPTION: A 54-year-old man who had had bilateral chronic L5 radiculopathy underwent L4-S1 lumbosacral decompression and fusion. During surgery, the patient lost transcranial electrical motor evoked potentials (tceMEPs) from the left tibialis anterior (TA) at the time of L5-S1 intervertebral cage placement. Neither intraoperative free-run electromyography nor somatosensory evoked potentials detected any

Iatrogenic neurologic deficits after lumbar spine surgery are rare complications, but important to recognize and manage. Complications such as radiculopathy, spinal cord compression, motor deficits (i.e. foot drop with L5 radiculopathy), and new onset radiculitis, while uncommon do occur. Attempts at mitigating these complications with the use of neuromonitoring have been successful. Guidance in the literature as to the true rate of iatrogenic neurologic deficit is limited to several case studies and retrospective designed studies describing the management, prevention and treatment of these de

INTRODUCTION: CMT4B2 is a rare subtype of CMT caused by pathogenic mutations in the myotubularin-related protein-13/set binding factor 2 (MTMR13/SBF2) gene. Nerve conduction velocities are markedly reduced and focally folded myelin sheaths are present on nerve biopsies. We presented two patients from two related Portuguese families with peripheral neuropathy caused by a novel mutation in the MTMR13/SBF2 gene.
CASE REPORT: Family 1: Patient 1: A 30-year-old woman, with disease onset in early childhood presented pes cavus and hammertoes and walked with a steppage gait. Muscle weakness was prese

BACKGROUND: Gliomas adjacent to the corticospinal tract (CST) should be carefully resected to preserve motor function while achieving maximal surgical resection. Modern high-field intraoperative magnetic resonance imaging (iMRI) enables precise visualization of the residual tumor and intraoperative tractography. We prospectively evaluated the extent of resection and distance between the tumor resection cavity and CST using 3-T iMRI combined with motor evoked potentials (MEP) in glioma surgery.
METHODS: Participants comprised patients who underwent surgery for solitary supratentorial glioma lo

Extracorporeal membrane oxygenation (ECMO) is a form of temporary cardiopulmonary bypass for patients with acute respiratory or cardiac failure refractory to conventional therapy. Its usage has become increasingly widespread and while reported survival after ECMO has increased in the past 25 years, the incidence of neurological injury has not declined, leading to the pressing question of how to improve time-to-detection and diagnosis of neurological injury. The neurological status of patients on ECMO is clinically difficult to evaluate due to multiple factors including illness, sedation, and p

Electrodiagnostic testings including electromyography have become increasingly important tools in clinical practice of neurology. These tests should not merely be referred to a technician, but be viewed as a clinical tool used by a neurologist. Recent advances in clinical electromyography, nerve conduction tests, and tests for axonal function including the newly developed threshold electrotonus are reviewed. These not only serve as a diagnostic tool, but also are useful in clinical trials by providing an objective parameter of nerve function. Computerized threshold tracking techniques such as

The occurrence of mechanical nerve injury in connection with anaesthesia and surgery is probably more common than generally believed, and by no means all cases are reported to the patient injury claims department. Both incorrect positioning of the patient on the operating table and pressure from retractors and other instruments can contribute to the occurrence of such injuries. Both neurological and neurophysiological procedures should be used to localise the injury, particularly for the purpose of insurance assessment. Most important of all, however, is prevention of injury. Risk minimisation

Thoraco-abdominal aneurysm (TAA) repair carries a significant risk of spinal cord infarction. The latter results from irreversible changes in the spinal cord arterial network, e.g., sacrifice of the segmental arteries. Intra-operative neurophysiology with somatosensory and especially motor evoked potential (SEP and MEP respectively) monitoring, has emerged as an effective tool to assess the efficiency of the collateral blood flow, detect reversible spinal cord ischemia and guide the peri-operative multidisciplinary management to prevent postoperative paraplegia. The main roles of such monitori

Assessment of clinical utility involves a series of steps based primarily on published peer-reviewed medical literature. Relevant publications usually use the scientific method, appropriate control groups, blinded reading, prospective design, and other study elements. Assessments are more credible when conducted by those who do not have a conflict of interest in the technique. A detailed assessment of digital and quantitative EEG was conducted recently by the American Academy of Neurology. The American Clinical Neurophysiology Society was a joint sponsor. This assessment concluded that digital

Preservation of facial nerve function is extremely important in vestibular schwannoma surgery. Intraoperative transcranial facial motor evoked potential (Tc-fMEP) monitoring was used, and its accuracy and the correlations of Tc-fMEP results with postoperative facial nerve function at various time points were investigated. Factors associated with postoperative deterioration of facial nerve function were also examined. Forty-five consecutive cases of vestibular schwannoma that underwent surgery at our hospital from January 2013 to July 2022 were retrospectively reviewed. The correlation between

INTRODUCTION: We investigated transcutaneous laryngeal ultrasonography (TLUS) for assessing vocal fold (VF) mobility during thyroid and parathyroid surgeries, emphasizing its precision and utility in signal loss scenarios.
METHODS: Between October 2020 and January 2023, we performed TLUS, Doppler sonography, laryngeal twitch response (LTR) palpation, and electromyography (EMG) to monitor recurrent laryngeal nerve function during neck surgeries. Postoperatively, the VF activity was verified using fiberoptic video laryngoscopy (FL). Concordance with FL was categorized based on VF activity.

BACKGROUND: Malignant middle cerebral artery infarction (MMI) is generally treated with decompressive craniectomy (DC) followed by neurointensive care (NIC). However, prognostic values of autonomic and complexity indices remain unclear. We aimed to explore heart rate variability (HRV), baroreflex sensitivity (BRS), and signal complexity (SC) in patients with MMI during post-DC NIC in relation to demographics, stroke severity, NIC variables [intracranial pressure (ICP), cerebral perfusion pressure (CPP), and Pressure Reactivity index (PRx)], and outcome.

PURPOSE: Trigeminal neuralgia (TN), a debilitating condition, is commonly treated with microvascular decompression (MVD). However, effective intraoperative neurophysiological monitoring remains challenging. This study introduces a novel electrophysiological technique using sensory-masseter response (SMR) to monitor trigeminal nerve compression during MVD.
METHODS: A total of 34 patients with TN underwent MVD. A concentric neurostimulator was employed to systematically deliver microcurrent stimulation to various intracranial segments of the trigeminal sensory root. We specifically probed the s…

OBJECTIVE: Carotid endarterectomy (CEA) is an effective preventive treatment for stroke, but carries a risk of cerebral hypoperfusion during carotid artery clamping. Intraoperative monitoring of somatosensory evoked potentials (SSEPs) is commonly used to detect hypoperfusion. However, predictors of significant SSEP amplitude decrease remain unclear. This study aimed to identify risk factors associated with substantial SSEP amplitude decreases during CEA with routine shunting and characterize patients at increased risk for intraoperative cerebral hypoperfusion.
METHODS: We retrospectively anal
