Treatment and Relapse Prevention of Typical and Atypical Optic Neuritis.

Optic neuritis (ON) is an inflammatory condition involving the optic nerve. Several important typical and atypical ON variants are now recognized. Typical ON has a more favorable prognosis; it can be idiopathic or represent an early manifestation of demyelinating diseases, mostly multiple sclerosis (MS). The atypical spectrum includes entities such as antibody-driven ON associated with neuromyelitis optica spectrum disorder (NMOSD) and myelin oligodendrocyte glycoprotein antibody disease (MOGAD), chronic/relapsing inflammatory optic neuropathy (CRION), and sarcoidosis-associated ON. Appropriat…

Optic Neuritis-Independent Retinal Atrophy in Neuromyelitis Optica Spectrum Disorder.

BACKGROUND: A limited number of studies have investigated the presence of ongoing disease activity independent of clinical relapses in neuromyelitis optica spectrum disorder (NMOSD), and data are conflicting. The objective of our study was to examine whether patients with aquaporin-4 (AQP4)-IgG seropositive NMOSD exhibit progressive retinal neuroaxonal loss, independently of optic neuritis (ON) attacks. METHODS: In this single-center, longitudinal study, 32 AQP4-IgG+ NMOSD patients and 48 healthy controls (HC) were followed with serial spectral-domain optical coherence tomography and visual a…

Intravenous Immunoglobulin in the Treatment of Adalimumab-associated Optic Neuritis.

Optic neuritis (ON) is a rare complication of tumor necrosis factor (TNF)-α inhibitors. The autoantibody serostatus, treatment, and outcome of TNF-α inhibitor-associated ON remain unclear. We herein report a 50-year-old woman with ON following adalimumab therapy. The patient presented with decreasing visual acuity of the right eye, quickly diminishing to light perception. Anti-aquaporin-4 (anti-AQP4) and anti-myelin oligodendrocyte glycoprotein antibodies were negative. Adalimumab was discontinued, and intravenous methylprednisolone and intravenous immunoglobulin (IVIg) were administered. Howe…

Post COVID-19 Myelin Oligodendrocyte Glycoprotein Antibody-Associated Optic Neuritis.

Neurological manifestations of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) are relatively common. Although some may be consequences of direct cellular viral invasion (neurotropism), many represent post-infectious inflammation mediated by autoimmune mechanisms. We herein report the case of a 69-year-old diabetic male who presented with bilateral sub-acute, progressive loss of vision 45 days after suffering a presumed SARS-CoV-2 related pneumonia. He had bilateral optic disc oedema. Magnetic resonance imaging showed uniform contrast enhancement of both optic nerves without spina

MRI Characteristics of NMO, MOG and MS Related Optic Neuritis.

Acute isolated optic neuritis can be the initial presentation of demyelinating inflammatory central nervous system disease related to multiple sclerosis (MS), neuromyelitis optica (NMO) or myelin oligodendrocyte glycoprotein antibody disease (MOG-AD). In addition to the well-characterized brain and spinal cord imaging features, important and characteristic differences in the radiologic appearance of the optic nerves in these disorders are being described, and magnetic resonance imaging (MRI) of the optic nerves is becoming an essential tool in the differential diagnosis of optic neuritis. Wher…

Acute optic neuritis: What do complementary tests add to diagnosis?

INTRODUCTION: Optic neuritis (ON) is the inflammation of the optic nerve due in many cases, to a pathological immune response. Since its symptoms can be subtle, diagnosis is sometimes challenging. The value of complementary tests for diagnosis and prognosis of ON was demonstrated in retrospective analysis, but their utility in the acute period of ON has been scarcely studied. The aim of this study is to determine the usefulness of clinical assessment, optical coherence tomography (OCT), visual evoked potentials (VEP) and orbit magnetic resonance imaging (MRI) for making diagnosis and prognosis…

Influence of retinal NMDA receptor activity during autoimmune optic neuritis.

Autoimmune optic neuritis (AON), a model of multiple sclerosis-associated optic neuritis, is accompanied by degeneration of retinal ganglion cells (RGCs) and optic nerve demyelination and axonal loss. In order to investigate the role of N-methyl-d-aspartate (NMDA) receptors in mediating RGC degeneration, upstream changes in the optic nerve actin cytoskeleton and associated deterioration in visual function, we induced AON in Brown Norway rats by immunization with myelin oligodendrocyte glycoprotein. Subsequently, visual acuity was assessed by recording visual evoked potentials and electroretino…

Unilateral Optic Neuritis in Children: Experience of a Tertiary Centre.

INTRODUCTION: Optic neuritis (ON) is an inflammation of the optic nerve that can be associated with a multitude of different systemic conditions and primary CNS disorders. In children, only around 25% of unilateral ON cases are considered idiopathic. Despite being considered a clinical diagnosis per se, neuroimaging plays a crucial role in the diagnostic pathway for these patients. METHODS: Demographic, clinical, and imaging data of all patients presenting with unilateral ON at a tertiary paediatric centre over 40 months (January 1, 2016, to April 30, 2019) were retrospectively analyzed. RES…

Promising recovery biomarkers after first event acute demyelinating optic neuritis.

BACKGROUND: Acute demyelinating optic neuritis is often a first episode of subsequent CNS demyelinating disease, most frequently multiple sclerosis (MS). The majority of the patients have substantial recovery of the high contrast visual acuity (HCVA) and 30-2 perimetry, but reduced contrast sensitivity persists in 56% of eyes after an episode of optic neuritis. Cross sectional studies of MS patients show low contrast visual acuity (LCVA) correlates modestly with RNFL thickness and macula ganglion cell +IPL layer (GCL) thickness. Considering the potential severity of the vision deficits at onse…

Acute Myelitis, Recurrent Optic Neuritis, and Seizures Over 17 Years.

Recent discovery of several autoantibodies, such as aquaporin-4 immunoglobulin G antibodies (AQP4-IgG), myelin oligodendrocyte glycoprotein immunoglobulin G antibodies (MOG-IgG) and glial fibrillary acidic protein immunoglobulin G antibodies (GFAP-IgG), has greatly facilitated differential diagnosis of autoimmune disorders of the central nervous system. Here we report an interesting case with a history as long as 17 years. Only until she was tested positive for MOG-IgG that her diagnosis was revised from multiple sclerosis to MOG-associated disease (MOGAD). Our case illustrates the significanc

Presumed Primary Bacterial Rhinosinusitis-Associated Optic Neuritis in a Cat.

Acute bacterial rhinosinusitis is a common illness in children and can lead to complications such as preseptal/orbital cellulitis, orbital/subdural/cerebral abscessation, osteomyelitis, meningitis, and optic neuritis with blindness. Primary bacterial infections leading to rhinosinusitis in cats is rare and descriptive reports are lacking. The current report describes a cat with Escherichia coli and Actinomyces spp. infections causing severe chronic rhinosinusitis and subsequent loss of vision. Treatment with antibiotics and prednisolone coincided with a complete resolution of nasal disease-rel…

Diagnostic implications of MOG/AQP4 antibodies in recurrent optic neuritis.

The present study aimed to detect myelin oligodendrocyte glycoprotein (MOG) and aquaporin-4 (AQP4) antibodies in serum specimens of patients with recurrent optic neuritis (RON) through establishing 293 cells with stable expression of MOG and the complete genomic sequence as the substrate using a cell-based assay (CBA). Furthermore, the clinical features of MOG antibody-positive recurrent optic neuritis (MOG-RON) were assessed. A total of 43 RON patients admitted to Beijing Tongren Hospital from December 2014 to May 2015 were enrolled, including 11 males and 32 females. The serum was collected …

Functional connectivity alterations in a murine model of optic neuritis.

The basis for neuronal dysfunction following inflammatory demyelination of the central nervous system (CNS) remains poorly understood. We characterized the network response to white matter injury in the anterior visual pathway using an experimental model of optic neuritis (ON), as ON is often an early manifestation of immune-mediated CNS demyelination in multiple sclerosis (MS). Optical intrinsic signal imaging was performed before and after the induction of ON in mice to measure changes in cortical network functional connectivity. We observed a greater loss of connectivity between homotopic v…
