[Clinical outcome of a bilateral tibial pylon fracture treated with a minimally invasive technique].

INTRODUCTION: Intraarticular fractures of the distal tibia are traumas accounting for 1% of lower extremity fractures. The tibial pylon fracture is a metaphyseal lesion that spreads inside the ankle and is difficult to treat with any method. CASE REPORT: This is the clinical outcome at one year, of a 40-year-old male patient with a bilateral tibial pylon fracture, treated with the MIPO (minimally-invasive plate osteosynthesis) technique. RESULTS: It was possible to reduce complications such as bone and soft tissue infections, early arthrosis and pseudoarthrosis, and to achieve the patient's

Congenital failure of formation of the upper limb.

This article provides an overview of the various classification systems for radial longitudinal, central longitudinal, and ulnar longitudinal deficiencies. It looks specifically at radius deficiency, hypoplastic thumb, index finger pollicization, narrowed thumb web, forearm-elbow deformities, and hand abnormalities. Various surgical approaches to these conditions and their results are explored. The authors conclude that longitudinal failure of formation deformities comprises a substantial portion of the anomalies of the upper limb. Although much of the past and current congenital hand literatu

Anterior subcutaneous transposition of the ulnar nerve.

Anterior, subcutaneous ulnar nerve transposition decompresses the ulnar nerve and, by transposing anterior to the medial epicondyle, eliminates longitudinal traction forces applied to the nerve during elbow flexion. This article reviews the indications and contraindications of the technique and describes the surgical technique in detail.

Does vertebroplasty cause incident vertebral fractures? A review of available data.

Vertebroplasty has been in practice in the United States for approximately 10 years and has been described as providing significant benefit to patients with painful vertebral compression fractures. Although the procedure appears to provide dramatic pain relief, it is not without complications. The primary point of discussion in this paper is whether vertebroplasty predisposes patients to the development of additional vertebral fractures, at a rate higher than that seen in the absence of vertebroplasty. To date there remains no definitive answer to this question. There is, however, a significan

Neuromuscular scoliosis: diagnostic and therapeutic considerations.

Neuromuscular scoliosis in a child presents as a diagnostic and therapeutic dilemma to the pediatric neurologist, the pediatric neurological surgeon, and the pediatric orthopaedist. Because of its progressive nature, it tends to be refractory to conservative management and requires active intervention. Neuromuscular scoliosis is a symptom, and the diseases leading to the deformity must be addressed together with the spinal abnormality. The framework of the diagnosis and management is addressed in this article.

Medical Malpractice Litigation Following Hindfoot Arthrodesis.

Given high patient expectations in the setting of complex surgeries, orthopedic surgeons are at risk of being subject to malpractice claims which can impose significant economic and psychological burden. This study investigates malpractice claims against orthopedic surgeons and podiatrists performing hindfoot arthrodesis and determine factors associated with plaintiff verdicts and settlements using the Westlaw legal database. The database was queried for all cases involving hindfoot arthrodesis using the terms "malpractice" and either "ankle fusion," "arthrodesis," "subtalar fusion," "tibiotal

The maxillary orthopedic splint.

Craniomaxillary orthopedic correction of the skeletal imbalance of Class II malocclusions is often the desired method of treatment in these cases. The ability to apply a "pure" orthopedic force to the maxillary complex has so far eluded research efforts. However, the use of a maxillary splint with a high-pull extraoral traction assembly has been shown to be most effective in reducing Class II skeletal dysplasias through a combination of dentoalveolar and basal bone changes. Force delivery to the maxillary complex in Class II skeletal jaw disharmonies is through the teeth. The philosophy behind

The Crush Index: Orthopedic Trauma as an Economic Indicator.

The purpose of this study was to evaluate the relationship between economic activity and the incidence of high-energy orthopedic trauma. California's Office of Statewide Health Planning and Development patient discharge database was queried to identify monthly orthopedic trauma incidence from 1995 to 2010. Patient inclusion required 1 diagnosis code and 1 associated procedural code for fractures of the femur, tibia, ankle, pelvis, or acetabulum. Data on composite market indices, energy and transportation use, and unemployment were obtained from government sources. Statistical analysis was perf

Strategies to Promote Health Equity for Orthopaedic Surgery Patients Who Speak a Language Other Than English.

The growing linguistic diversity in the United States presents substantial challenges to equitable health-care delivery. This article outlines strategies to promote health equity for orthopaedic surgery patients who speak a language other than English, including supporting the accessibility of professional interpreter services, enhancing language-specific patient educational resources, ensuring equity in the care tools that are used to improve access and engagement, strengthening the multilingual workforce in orthopaedic surgery, and standardizing the collection of language data elements for q

Postoperative human motion evaluation in orthopedics based on kinematic constraints and improved IMU-generated data processing algorithm.

Traditional methods for evaluating postoperative human motion in orthopedics suffer from high equipment costs, time-consuming detection processes, and can only provide static two-dimensional image information. To improve the efficiency and accuracy of postoperative human motion assessment in orthopedics, this study proposes an improved algorithm on the basis of kinematic constraints and inertial measurement units for postoperative human motion assessment in orthopedics. Compared with other methods, the average error and standard deviation of measuring human joint range of motion after orthoped

Nationwide Effect of COVID-19 on Cases Performed During Pediatric Orthopaedic Surgery Fellowship Training in the United States.

INTRODUCTION: The COVID-19 pandemic negatively affected surgical training in the United States. We hypothesized that reported case volume during pediatric orthopaedic surgery fellowship training would decrease markedly during the 2019 to 2020 academic year, which corresponded with the COVID-19 outbreak. METHODS: The Accreditation Council for Graduate Medical Education provided nationwide case logs for accredited pediatric orthopaedic surgery fellows (2017 to 2021). Annual reported case volumes were extracted and summarized as means ± SD. Parametric tests were used to compare annual case volum

Factors influencing treatment recommendations for base of 5th metatarsal fractures in orthopaedic residency programs.

BACKGROUND: Management of proximal 5th metatarsal fractures remains a controversial topic in orthopaedic surgery. Both operative and non-operative approaches have been described in the clinical setting. This confusion has led to non-standardized treatment recommendations for proximal 5th metatarsal fractures. This study was designed to analyze concordance rate of treatment recommendations between orthopaedic trainees and orthopaedic foot and ankle experts. METHODS: An online survey containing 14 cases of proximal 5th metatarsal fractures were distributed to 92 orthopaedic residents in two ACG

The Welsh Clan That Transformed Orthopedic Surgery.

Modern orthopedic surgery arose from Hugh Owen Thomas (1834-1891) and Sir Robert Jones (1857-1933), uncle and nephew, two descendants of a family of bonesetters from North Wales and Liverpool, respectively. Thomas combined a formal medical education with his hereditary vocation into treatments that have endured for more than a century. The epitome is the Thomas splint, which was credited with a decrease in the mortality from open femur fractures to 20 from 80 per cent during World War I and is still in use for the emergency transport of lower extremity injuries. Jones was 15 when he became Tho

Early Practice Patterns of Hand Surgeons: An American Board of Orthopaedic Surgery Database Study.

PURPOSE: To investigate the variety of cases that are performed by newly graduated fellowship-trained orthopedic hand surgeons. METHODS: All cases submitted by board-eligible orthopedic surgeons taking part II of their board examination between 2004 and 2013 were obtained from the database of the American Board of Orthopaedic Surgery (ABOS). Each case was then categorized based on the fellowship training of the treating surgeon and whether it was a hand surgery case. Each hand surgery case was then further categorized into 1 of the 8 surgical categories used by the ABOS to determine eligibili

Medical Missions in the Middle East: An Orthopaedic Surgeon's Experience of a Series of Lower Extremity Cases.

Orthopaedic trauma accounts for a great deal of disability worldwide. There are many impoverished nations affected by war wherein victims suffer blast injuries associated with mines, missiles, high-powered gunshots, and bombings. One way to address this is through international medical missions sponsored by industrialized nations. It is imperative that practitioners have a basic understanding of the type of injuries that may be encountered in these nations impacted by war and conflict. Therefore, we described a small number of various lower extremity injuries seen by one orthopaedic surgeon du
