A randomized controlled trial on the clinical value of umbilical Doppler velocimetry in antenatal care.

OBJECTIVE: Our aim was to assess the clinical value of umbilical Doppler velocimetry with regard to maternal hospitalization policy, obstetr … ement during labor and the occurrence of fetal distress during labor. In the control group a higher perinatal mortality rate was observed. In the Doppler group the risk ratio of perinatal mortality of fetuses and infants weighing > or = 500 gm was 0.45 (95% confidence interval 0.21 to 0.94), as compared with the control group. Neonatal mortality was equal in both gro …

Advancing the newborn and stillbirth global agenda: priorities for the next decade.

Remarkable advances have been made over the past decade in defining the burden of newborn mortality and morbidity and stillbirths, and in identifying interventions to address the major risk factors and causes of deaths. However, progress in saving newborn lives and preventing stillbirths in countries lags behind that for maternal mortality and for children aged 1-59 months. To accelerate progress, greater focus is needed on improving coverage, quality and equity of care at birth-particularly obstetric care during labour and childbirth, and care for small and sick newborns, which gives a triple

Perinatal outcome and clinical features of monochorionic monoamniotic twin gestation.

AIM: To clarify perinatal outcomes of monoamniotic (MM) twin pregnancies. MATERIAL AND METHODS: MM twins delivered in seven tertiary perinatal centers during the last decade were retrospectively evaluated. All pregnant women were scheduled to begin inpatient management at around 24 weeks of gestation and undergo a planned cesarean section beyond 32 weeks. Pregnancy outcomes, prevalence of fetal death and cord entanglement, perinatal mortality and neuromorbidity rate at discharge were examined. RESULTS: The study group comprised 38 MM twin pregnancies (76 fetuses). Cord entanglement was confi…

Preterm birth in Latvia: Two-decade national trends, structure, and risk factors.

INTRODUCTION: Latvia, alongside other Baltic and Nordic countries, exhibits some of the world's lowest preterm birth (PTB) rates. We sought … 5-year intervals, assessing temporal trends in PTB and term birth outcomes, including neonatal and perinatal mortality, labor onset (spontaneous or indicated), and prematurity subgroups, alongside known PTB risk factors (n = 16). Joinpoint regression identified significant trend shifts, with statistical significance set at p < 0.05. RESULTS: Latvia's overall PTB ra …

Identifying avoidable causes of perinatal deaths in a district hospital in Lesotho.

BACKGROUND: Certain determinants can be associated with avoidable perinatal deaths, and audits are needed to establish what these determinants are, and what can be done to prevent such deaths. OBJECTIVES: The study aimed at identifying and describing determinants associated with avoidable perinatal deaths at a district hospital in Lesotho and strategies to curb their occurrence. METHOD: A retrospective descriptive study was conducted using 142 anonymised obstetric records from January 2018 to December 2020. A data collection tool was adopted from the Perinatal Problem Identification Programm

Syphilis in pregnancy: The impact of "the Great Imitator".

Syphilis remains a common congenital infection over the globe. There has been a tremendous rise in the number of congenital syphilis cases worldwide in the last 20 years. It affects large numbers of pregnant mothers in high burden regions causing a significantly high perinatal mortality and morbidity, which can be easily prevented by early antenatal screening and treatment. Diagnosis of maternal syphilis in pregnancy mainly based on clinical symptoms, serological tests and direct identification of treponemes in clinical samples. However, the diagnosis can be challenging due to the relapsing-re…

Comparison of perinatal outcomes in facilities before and after Global Network's Helping Babies Breathe Implementation Study in Nagpur, India.

BACKGROUND: The Helping Babies Breathe (HBB) educational program focuses on training of first-level birth attendants in neonatal resuscitation skills for the first minute of life (The Golden Minute). Pre-post studies of HBB implementation in sub-Saharan Africa and Asia have shown reductions in facility-based very early neonatal mortality and stillbirth rates. However, the Global Network pre-post HBB Implementation Study (GN-HBB-IS) found no difference in day 7 perinatal mortality rates (PMR-D7) among births to women participating in the Global Network's Maternal and Newborn Health Registry. To…

The rising incidence and impact of non-medically indicated pre-labour cesarean section in Latin America.

In the poorest populations of the world the difficulties of performing a surgical procedure lead to extremely low cesarean section rates associated with very high perinatal mortality. Meanwhile the proportion of births by cesarean section has been increasing for several decades in many areas of the world, reaching alarmingly high rates especially in Latin America. This review aims to describe this secular trend. The causes of the increase in cesarean deliveries are analyzed with a multidimensional approach, trying to recognize the reasons behind the choice of the route of delivery. We are faci…

Impact of increased rate of caesarean section on perinatal outcome: sociolegal evaluation.

Sharp rise in the caesarean section rate, over the past years has been causing lot of concerns. It is to be evaluated whether the decrease in perinatal mortality rate is due to the rising rate of caesarean section. Five hundred consecutive patients were selected on whom caesarean section had been performed. Both mother and baby were followed till they are discharged from the hospital. Foetal distress was the commonest indication in primigravidae who underwent caesarean section. The other indications of caesarean section in this study were breech, severe pre-eclampsia, eclampsia, cord prolapse,…

Developing a systems approach to prevent meconium aspiration syndrome: lessons learned from multinational studies.

Passage of fetal bowel movement (meconium) is common (in about one out of six births), and in some the staining of the amniotic fluid is a sign of fetal distress. Inhalation of meconium (aspiration syndrome, in upto one out of five to eight such births) just before or at birth may be preventable by a coordinated approach by well-trained and informed birth attendants. Respiratory failure secondary to meconium aspiration syndrome (MAS) remains a major cause of morbidity and mortality in the neonatal population. Infants with hypoxemic respiratory failure because of MAS, persistent pulmonary hyper

A five-year study of prematurity.

To get a comprehensive picture of prematurity and neonatal death or survival, all factors on the maternal and fetal records that might be pertinent were recorded. This information, having to do with factors such as age, gravida of the mother, blood group incompatibility, period of gestation, weight at birth, type of delivery, medication and anesthesia administered to the mother, is presented in tabular form. In the study of perinatal mortality it was noted that failure to establish normal pulmonary ventilation is the most common cause of death in the neonatal period. Maternal history of relati…
