We describe a 66-year-old male with immunoglobulin G4-related disease (IgG4-RD) presenting with minimal change disease (MCD). A 45-year-old man presented with follicular exanthema in his lower limbs, alternating bowel habits and significant weight loss. Mevalonate kinase deficiency (MKD) is a rare inborn error of metabolism caused by mutations in the mevalonate kinase (MVK) gene.
