A peripheral palsy of the facial nerve that results in muscle weakness on one side of the face usually manifests as Bell's palsy. Glioma in the left half of the pons and middle cerebellar peduncle is a rare cause of isolated infranuclear facial paralysis. We describe a case with simultaneous occurrence of cluster headache-like pain and multiple sclerosis. Both neuroimaging and neurophysiology (trigeminal evoked potentials) revealed a demyelination plaque in the pons, at the trigeminal root entry zone, on the side of pain. Although that type of lesion is usually associated with trigeminal neuralgia pain, we hypothesize that in this case it may be linked with the concomitant cluster headache, possibly by activation of trigemino-vascular mechanisms.

Management of Labbé's vein in operations of cerebellar pons angle (CPA) and clivus regions was reported. Ligation of the Labbé's vein in the left side resulted in hematoma in the temporal lobe or aphasia. In preservation of Labbé's vein, either on the left or right side, only one case of intracerebral hematoma occurred. The reason for this difference was discussed. In protecting Labbé's vein, continuous ventricular draining and removal of the inferior temporal gyrus if necessary was effective.

Intracranial MR imaging was performed in five patients with achondroplasia. All patients had narrowing of the subarachnoid space at the level of the foramen magnum that was mainly due to protrusion of the posterior aspect. Three patients had compressive deformities of the brainstem and/or upper cervical spine. Among them, two patients had deformities of the pons. Relative upward displacement of the brainstem was seen in all patients. Hydrocephalus was seen in three patients.

The first clinical case of transient traumatic locked-in syndrome is presented. The locked-in state lasted for 3 months. The patient's neurologic deficit and the abnormal auditory evoked responses pointed to a lesion involving the left pons and ventral midbrain. The locked-in syndrome caused by a traumatic damage of the brain stem structures can be transient and thus it may have a better prognosis than the locked-in syndrome secondary to the occlusion of the basilar artery.

On the basis of two cases some problems of the locked-in-syndrome are discussed. In most cases the cause of the syndrome is an infarct in the ventral part of the pons due to an occlusion of the vertebral or basilar artery. There is a total loss of movement exept for the movement of the eyes in all directions and the eye-closing. Although vigilance is almost not disturbed the complete loss of movement often results in a false diagnosis of cerebral coma. The EEG is an important diagnostic aid.
