OBJECTIVE: The evolution of antibiotic resistance is far outpacing the development of new antibiotics, causing global public health concern about infections that will increasingly be unresponsive to antimicrobials. This risk of emerging antibiotic resistance may be meaningfully altered in highly AIDS-immunocompromised populations. Such populations fundamentally alter the bacterial evolutionary landscape in two ways, which we seek to model and analyze. First, widespread, population-level immunoincompetence creates a novel host environment with disrupted selective pressures. Second, within AIDS-

Worldwide, CKD is seen in one in nine adults; type 2 diabetes (T2D) is the most common cause. The population of just two countries in Asia, India and China, constitutes about 35% of the global population and is larger than the population of Africa or the combined populations of Europe and the Americas. CKD Global Outcomes (Kidney Disease Improving Global Outcomes) Clinical Practice Guideline for the Evaluation and Management of CKD recommends the use of foundational therapies such as lifestyle modifications, detection and treatment of hypertension, and pharmacologic management to forestall car

Improved survival in sickle cell disease (SCD) has led to a growing population of older adults who experience a high burden of chronic pain. With aging, pain often shifts from episodic vaso-occlusive crises to persistent, multifactorial syndromes driven by cumulative tissue injury, central sensitization, and progressive organ damage. This evolution presents significant clinical challenges, particularly in the context of comorbidities, polypharmacy, and age-related physiological changes. This narrative review examines the epidemiology, pathophysiology, and management of chronic pain in aging ad

Down syndrome is the most common chromosomal disorder in the United States, occurring in about 14.14/10,000 births. It is associated with multiple medical anomalies, including cardiac, gastrointestinal, musculoskeletal, and genitourinary abnormalities, which increases the burden of morbidity for this patient population. Management is typically directed toward optimizing health and function throughout childhood and into adulthood; however, consensus regarding their management in adulthood is controversial. The burden of congenital cardiac diseases in children with trisomy 21 is well established

IMPORTANCE: Awareness of swallowing dysfunction in the pediatric population is growing. As many as 50% of parents report that their otherwise healthy children have a feeding problem. Dysphagia is increasingly common in the pediatric population, especially as advances in health care improve the survival of extremely premature infants and children with complex congenital anomalies. Symptoms of dysphagia and aspiration prompt referral to otolaryngologists.
OBSERVATIONS: Dysfunction can exist at any of the 4 phases of the physiologic swallow. Dysphagia manifests differently in children at each ag

Obesity is one of the most prevalent somatic comorbidities in individuals with major depressive disorder and greatly affects the course and prognosis of that disorder. The bidirectional relationship between major depressive disorder and obesity often creates a feedback cycle that challenges both patients and health-care providers. Gaps in interdisciplinary collaboration and limitations in knowledge transfer hinder the effective management of this patient population. This narrative Review synthesises current evidence from obesity and major depressive disorder research, offering a comprehensive

Heart failure (HF) remains a significant health challenge globally, placing a heavy burden on individuals, families, and healthcare systems. The prevalence of HF continues to rise, posing substantial public health concerns. This burden is particularly pronounced among the Black Population, who face higher prevalence, earlier onset, and greater severity of HF compared to other racial and ethnic groups. This review explores the multifaceted landscape of HF in Black individuals by examining epidemiological patterns, pathophysiological mechanisms, clinical presentations, treatment disparities, and

BACKGROUND: Substance use disorders (SUDs) are a global public health problem with more adverse impacts among the population at risk. Healthcare workers, including physicians, are at increased risk of substance use problems in developing countries, including India, due to prolonged working hours, financial stress due to poor pay during early career, and poor work-life balance.
AIM: We conducted a narrative review to assess the rate and pattern of substance use among Indian physicians.
METHODS AND RESULTS: As per limited prevalence studies among Indian physicians since the year 2000, tobacco

BACKGROUND: Evidence-based medicines, technologies, and protocols exist to prevent many of the annual 300,000 maternal, 2.7 million neonatal, and 9 million child deaths, but they are not being effectively implemented and utilized in rural areas. Nepal, one of South Asia's poorest countries with over 80% of its population living in rural areas, exemplifies this challenge. Community health workers are an important cadre in low-income countries where human resources for health and health care infrastructure are limited. As local women, they are uniquely positioned to understand and successfully n

BACKGROUND: Perinatal mood disorders are common yet underdiagnosed and un- or undertreated. Barriers exist to accessing perinatal mental health services, including limited availability, time, and cost. Automated conversational agents (chatbots) can deliver evidence-based cognitive behavioral therapy content through text message-based conversations and reduce depression and anxiety symptoms in select populations. Such digital mental health technologies are poised to overcome barriers to mental health care access but need to be evaluated for efficacy, as well as for preliminary feasibility and a

BACKGROUND: Burn injuries are a major global health problem, particularly in low- and middle-income countries, accounting for most burn-related deaths and disabilities. In Saudi Arabia, burns remain a frequent cause of morbidity, often resulting from domestic accidents involving hot liquids, open flames, or electricity. The Jazan region, with its dense population and reliance on traditional cooking methods, is considered at higher risk. Understanding public knowledge, attitudes, and practices regarding burns and first aid is essential for guiding preventive strategies and health education effo

Bladder cancer, the most prevalent urinary system malignancy, is characterized by high recurrence rates and prolonged treatment. While therapeutic advances have substantially improved survival, they have also heightened the prominence of financial toxicity (FT)-a treatment-related economic burden that severely impairs the quality of life of patients and their families. This review synthesizes evidence on the prevalence, influencing factors, impact on health outcomes, and management strategies of FT in bladder cancer, offering insights for individualized clinical support and health policy refin

Poisoning represents one of the most common medical emergencies encountered in young children in the United States, and accounts for a significant proportion of emergency room visits for the adolescent population. Poisoning is a significant and persistent cause of morbidity and mortality in children and adolescents. The scope of toxic substances involved in poisoning is broad, and requires health care providers to have an extensive knowledge of signs and symptoms of poisoning and specific therapeutic interventions and antidotes. Most children who ingest poisons suffer no harm; however, health

Homeless individuals face major barriers in accessing regular healthcare, leading to the development of street medicine as a distinct humanitarian field. To enhance continuity and quality of care, consistent documentation is crucial. However, electronic health records are rarely used in European street medicine. An interdisciplinary European workshop identified four key research fields for medical informatics in street medicine: (1) Research ethics: The creation of ethical guidelines that promote co-creation and active communication with patients. (2) System development: Designing a secure, an

Schistosomiasis is one of the Neglected Tropical Diseases that affects over 200 million people worldwide, of which 29 million people in Nigeria. The principal strategy for schistosomiasis in Nigeria is a control and elimination program which comprises a school-based Mass Drug Administration (MDA) with limitations of high re-infection rates and the exclusion of high-risk populations. The World Health Organization (WHO) recommends guided case management of schistosomiasis (diagnostic tests or symptom-based detection plus treatment) at the Primary Health Care (PHC) level to ensure more comprehens
