Identification of preferred reminder systems and patient factors to promote adherence in the management of urinary incontinence.

OBJECTIVE: To investigate comfort level and preferences of automated reminder systems (mail, email, text message, phone call, patient-portal message, and/or smartphone application) to promote adherence to recommended therapies for patients seeking care for urinary incontinence (UI) at our urology clinic in Phoenix, Arizona. METHODS: Anonymous surveys were distributed in English to adult patients with UI from 4/2019-5/2019. Patient demographics, UI type, and access to and use of the Internet, smartphone and patient-portal were assessed. Using a Likert scale, patients indicated level of comfort

Surgical Management of Hepatocellular Carcinoma Complicated by Severe Portal Hypertension Secondary to an Arterioportal Shunt: A Case Report.

INTRODUCTION: Arterioportal (AP) shunts are a recognized vascular complication of hepatocellular carcinoma (HCC). Severe AP shunts can cause portal hypertension, which can lead to complications such as variceal bleeding and refractory ascites. However, optimal management of HCC complicated by severe AP shunts remains challenging, particularly in patients with advanced disease. CASE PRESENTATION: A 73-year-old woman presented with melena and massive hematemesis due to esophageal varices. Contrast-enhanced computed tomography revealed multifocal HCC confined to the right hepatic lobe, Vp3 porta

Endovascular treatment of a splenic vein aneurysm through a transhepatic approach.

Aneurysms of the portal vein and its branches have been rarely described. Their natural history is unknown although large ones (>3 cm in diameter) have been reported to cause rupture, thrombosis, duodenal or biliary obstruction, inferior vena cava compression and/or portal hypertension. We report the case of an incidentally diagnosed 4.5 cm splenic vein aneurysm repaired by endovascular treatment through a transhepatic route. The aneurysm was successfully excluded using a covered stent (Viabahn, Gore). The transhepatic route opens the possibility of offering a minimally invasive approach to va

Coronary vein diameter and flow direction in patients with portal hypertension: evaluation with duplex sonography and correlation with variceal bleeding.

OBJECTIVE: The purpose of this study was to compare the diameter of the coronary vein and the direction of flow within it between patients w … ith portal hypertension, 29 (74%) of whom did not have hepatofugal flow in other major veins of the portal system. None of the patients with portal hypertension and hepatopetal flow in the coronary vein had a history of variceal hemorrhage, whereas 40% of those with hepatofugal flow had had variceal hemorrhage. CONCLUSION: The diameter of the coronary vein may …

Hypoplasia of the right hepatic lobe associated with portal hypertension and idiopathic retroperitoneal fibrosis.

A 22-year-old man was referred to our hospital because of thrombocytopenia. Abdominal computed tomography (CT) revealed hypoplasia of the right hepatic lobe, the development of porto-systemic collateral vessels, splenomegaly and a periaortic soft-tissue mass. Laboratory tests and needle liver biopsy indicated no evidence of liver cirrhosis. Consequently, a diagnosis of hypoplasia of the right hepatic lobe associated with portal hypertension and idiopathic retroperitoneal fibrosis was established. Portal hypertension and hypersplenism was thought to be the cause of the thrombocytopenia. CT arte

Retrospective Study to Compare Selective Decongestive Devascularization and Gastrosplenic Shunt versus Splenectomy with Pericardial Devascularization for the Treatment of Patients with Esophagogastric Varices Due to Cirrhotic Portal Hypertension.

BACKGROUND For patients with esophagogastric varices secondary to portal hypertension due to liver cirrhosis, portosystemic shunts and devascularization have become the most commonly used treatment methods. We have developed a novel surgical approach for the treatment of patients with cirrhotic portal hypertension, selective decongestive devascularization, and shunt of the gastrosplenic region (SDDS-GSR). This aim of this study was to compare the efficacy and safety of SDDS-GSR with splenectomy with pericardial devascularization (SPD). MATERIAL AND METHODS A retrospective study was undertaken

Use of ß-blocker therapy to prevent primary bleeding of esophageal varices.

PURPOSE: The purpose of this article is to educate nurse practitioners about the pathophysiology surrounding the development of portal hyper … sophageal varices are a result of the portosystemic collaterals the body develops to decompress the portal system. Hemorrhage from esophageal varices is a major cause of morbidity and mortality. Prevention of a primary bleed is the goal of therapy and is accomplished with nonselective ß-blockers. IMPLICATIONS FOR PRACTICE: Very few patients with portal hyperte …

Excitatory cholinergic innervation in canine portal and mesenteric veins.

A possibility that the canine portal and mesenteric veins may have an excitatory cholinergic innervation similar to that observed in the middle segment of the inferior vena cava (IVC) was investigated. After adrenergic blockade with prazosin, contractile responses of isolated longitudinal strips of the portal and mesenteric veins to transmural electrical stimulation were greatly diminished. Subsequent anticholinesterase (neostigmine or physostigmine) treatment, however, revealed in the nerve-evoked contraction a component that was antagonized by atropine. The same phenomenon was also observed

[MR angiography of pelvic and abdominal vessels].

The assessment of abdominal vessels with conventional methods is invasive and frequently does not yield an accurate diagnosis, particularly in the evaluation of the portal venous system. MR angiography is non-invasive and is well suited for the examination of abdominal vessels. Both vessel morphology and intravascular flow patterns can be characterized with this method. Clinical indications include portal hypertension, portal vein thrombosis, portosystemic shunts, assessment of congenital anomalies, thrombus, and tumor propagation into the inferior vena cava and the renal veins, proximal renal

[Sonographic detection of gas in the vascular system of the liver: an image essay].

Gas within the vascular system of the liver was demonstrated sonographically in 5 patients. In contrast, plain films of the abdomen in these patients failed to demonstrate this serious finding. 3 patients presented with gas within the hepatic portal venous system, 1 patient with gas in the liver veins. In these patients, gas formation was probably caused by sepsis with gas-producing bacteria. In the last patient, air within the hepatic arterial system was caused by insufflation of air thru a port-a-cath catheter located in the main hepatic artery. The characteristic sonographic appearance of i

Complications of cirrhosis. A 50 years flashback.

In patients with cirrhosis and portal hypertension, it is largely the frequency and severity of complications relating to the diseased liver, degree of portal hypertension and hemodynamic derangement that determine the prognosis. It can be considered as a multiple organ failure that apart from the liver involves the heart, lungs, kidneys, the immune systems and other organ systems. Progressive fibrosis of the liver and subsequent metabolic impairment leads to a systemic and splanchnic arteriolar vasodilatation. With the progression of the disease development of portal hypertension leads to for

Intrahepatic cholestasis due to systemic mastocytosis: a case report and review of literature.

A 35-yr-old female presented with symptoms of obstructive jaundice. Liver biopsy, bone marrow aspiration, and biopsy revealed systemic mastocytosis and acute myeloid leukemia. The liver biopsy specimen showed infiltration of mast cells within portal tracts with periductal and portal edema, irregularity of interlobular duct epithelium, and centrizonal cholestasis. Endoscopic retrograde cholangiography was normal. Following chemotherapy treatment with idarubicin and cytarabine for seven days for AML, the bilirubin levels continued to increase for two weeks and then decreased, reaching normal lev

Systemic, pulmonary and mesenteric perfusion and oxygenation effects of dopamine and epinephrine.

The response of the systemic, pulmonary, hepatic and portal circulations to infusion of dopamine and epinephrine was studied in newborn piglets 1 to 3 d of age. Anesthetized animals were instrumented to measure cardiac index (CI), hepatic arterial flow, and portal venous blood flow. Catheters were inserted for measurement of systemic arterial pressure (SAP), pulmonary arterial pressure (PAP), and for sampling of arterial, portal venous, and mixed venous oxygen saturations and plasma lactate levels. Systemic, pulmonary and mesenteric vascular resistance indices (SVRI, PVRI, MVRI), and systemic

Implantable central venous catheter facilitates prophylactic treatment in children with haemophilia.

Twelve children with a severe form of haemophilia A received a totally implantable venous access system (Port-A-Cath) to facilitate regular prophylactic treatment with factor VIII. The indication for implantation was difficulty in obtaining regular access to a peripheral vein. Postoperative bleeding around the portal site occurred in two of 12 cases. After a median duration of follow-up of 26 months (range 5-79 months), none of the systems had needed replacement due to bleeding, septicaemia or thrombosis. One child, with an inhibitor against factor VIII, had an infection at the portal site and

Albumin dialysis reduces portal pressure acutely in patients with severe alcoholic hepatitis.

BACKGROUND/AIMS: In patients with alcoholic hepatitis (AH), inflammation contributes to the severity of portal hypertension. This study evaluates the acute effects of albumin dialysis, using the Molecular Adsorbents Recirculating System (MARS), on portal pressure in AH. METHODS: Eleven patients with AH and portal hypertension were treated with MARS (n=8) or haemofiltration (n=3). All patients had associated organ failure manifested by hepatic encephalopathy (Grade 2 or more) or renal failure. Hepatic venous pressure gradient (HVPG) was measured before, during and after the treatment session.

Glycyrrhizin Attenuates Portal Hypertension and Collateral Shunting via Inhibition of Extrahepatic Angiogenesis in Cirrhotic Rats.

Portal hypertension develops along with liver cirrhosis then induces the formation of portal-systemic collaterals and lethal complications. Extrahepatic angiogenesis plays an important role. Glycyrrhizin has been found to exhibit anti-angiogenic features, which leads to its extensive use. However, the relevant effects of glycyrrhizin on liver cirrhosis and portal hypertension have not been evaluated. This study thus aimed to investigate the impact of glycyrrhizin on portal hypertension-related derangements in cirrhotic rats. Male Sprague-Dawley rats received bile duct ligation (BDL) to induce
