Pseudo-Bartter syndrome in an infant without obvious underlying conditions: A case report.

Pseudo-Bartter syndrome (PBS) develops owing to renal or extrarenal chloride loss, leading to hypokalemic alkalosis. Whereas most adult cases result from diuretic/laxative abuse, many infantile cases occur secondary to cystic fibrosis. Rarely, infantile PBS is caused by renal salt loss with anomalies of the kidney/urinary tract or genetic disorders, such as Dent disease. Here, we report the case of a 10-mo-old girl with a one-month history of decreased formula intake and 5.6% body weight loss. She showed typical laboratory findings as PBS, including hypokalemia (2.7 mEq/L) and high levels of b

Anesthetic Considerations for Cesarean Delivery in a Parturient With Severe Gitelman Syndrome.

Gitelman syndrome is an autosomal recessive inherited disorder that impairs the function of thiazide-sensitive sodium-chloride cotransporters in the distal convoluted tubule of the nephron. During labor and delivery, avoidance of sympathetic overactivity, meticulous hemodynamic monitoring, and expedited repletion of potassium and magnesium are required to avoid adverse outcomes. We present a parturient with severe Gitelman syndrome, requiring continuous electrolyte and fluid infusions, who underwent successful cesarean delivery. Potential severe morbidity was avoided with multidisciplinary pla

[Oesophageal ulcer associated with the use of bacampicillin].

INTRODUCTION: Many drugs may provoke oesophageal disorders. In France, doxycyclin, potassium chloride non-steroidal anti-inflammatories are the most frequent agents incriminated. OBSERVATION: A 27 year-old man consulted for dysphagia and odynophagia due to oesophagitis following administration of bacampicillin. COMMENTS: Several drugs can provoke oesophageal disorders including ulcerations. Predisposing circumstances exist. Ingestion of the drugs with a sufficient quantity of water (60 to 100 ml), whilst standing or sitting should be able to limit this type of event.

Factors affecting ciliary beat frequency in the intrapulmonary airways of rats.

The influence of drugs on ciliary beat frequency in the intrapulmonary airways of rats was studied in vitro. Sympathomimetic drugs significantly increased the ciliary beat frequency. Acetylcholine, pilocarpine, aminophylline, prednisolone, potassium iodide, and ammonium chloride also caused cilioexcitation, though to a lesser degree. Phentolamine, 5-hydroxytryptamine, histamine, and acetylcysteine caused an apparent cilioinhibition. However, histamine in high doses also caused an incoordination of ciliary beat. Codeine and phenobarbital were slightly cilioexcitatory in low concentrations and c

A Challenging Case of Persisting Hypokalemia Secondary to Gitelman Syndrome.

There are several causes of hypokalemia, including transcellular shift, renal loss, gastrointestinal loss, and decreased oral intake. Sometimes it is challenging to know the source of the problem; however, with detailed history, physical examination, and appropriate laboratory investigations, the physician should be able to narrow down the differentials diagnosis to reach the right one. One of the rare causes of hypokalemia is Gitelman syndrome, which is a salt-losing tubulopathy that manifests as renal potassium wasting, metabolic alkalosis, hypokalemia, hypomagnesemia, hypocalciuria, and hyp

Anesthesia and Bartter's syndrome: a case report and review.

A 22-year-old female with Bartter's syndrome presented at 40 weeks' gestation for elective cesarean section under general anesthesia. Her usual medication was ibuprofen and potassium supplements. Preoperative potassium was 3.3 mmol/L, and in anticipation of difficulties in fluid, electrolyte, and acid-base management, a central line and urinary catheter were inserted and blood gases measured. In the first 20 hours postdelivery she had a brisk diuresis and required 3.5 L of crystalloid to maintain her central venous pressure and 100 mmol of potassium to prevent significant hypokalemia. The main

Flow activates an endothelial potassium channel to release an endogenous nitrovasodilator.

Flow-mediated vasodilation is endothelium dependent. We hypothesized that flow activates a potassium channel on the endothelium, and that activation of this channel leads to the release of the endogenous nitrovasodilator, nitric oxide. To test this hypothesis, rabbit iliac arteries were perfused at varying flow rates, at a constant pressure of 60 mm Hg. Increments in flow induced proportional increases in vessel diameter, which were abolished by L,N-mono-methylarginine (the antagonist of nitric-oxide synthesis). Barium chloride, depolarizing solutions of potassium, verapamil, calcium-free medi

Potassium Overdose in Patient with Chronic Kidney Disease on Losartan: A Case Report.

INTRODUCTION: Hyperkalemic emergencies can present with weakness, paralysis, sensorimotor deficits, and potentially fatal cardiac conduction abnormalities even in the absence of an elevated serum potassium. Common antihypertensive medications, such as angiotensin-converting enzyme inhibitors or angiotensin II receptor blockers, are associated with serum potassium elevations and can exacerbate hyperkalemia, especially in patients with renal impairment. CASE REPORT: We report a 49-year-old patient who presented to the emergency department six hours following an intentional ingestion of potassiu

[Effect of furosemide on the intrarenal hemodynamics and excretory renal function depending on salt regime].

The experiments on rats showed that the dietary mineral composition influences renal circulation and saluretic function and modifies the related effects of furosemide. In particular, a high NaCl intake reduces the blood perfusion in the renal cortex and increased that in the renal medulla, thus increasing sodium excretion with urine. The use of hyposol, a table salt substituent, enhances the blood flow in the renal medulla and leads to a significant increase in the diuresis, natriuresis, and kaliuresis. Furosemide (10 mg/kg) sharply increases the medullar circulation and electrolyte excretion

Chronotropic action of Na+, K+, Cl- cotransport inhibition in the isolated rat heart.

The chronotropic actions of Na+, K+, Cl- cotransport were investigated by studying the effects of the loop diuretics bumetanide and furosemide, specific inhibitors of the cotransporter, on an isolated rat sino-atrial node preparation. Application of bumetanide decreased the cycle length from 0.334 s (+/- 0.087 S.D.) to 0.279 s (+/- 0.083, n = 16, P = 6.5 x 10(-6)) in Hepes-buffered physiological salt solution (PSS). Similar decreases were recorded in bicarbonate-buffered PSS. Chloride channel blockers indicate that the tachycardia evoked by loop diuretics is not due their blocking of chloride

Total body potassium, potassium retention and potassium intake in protein energy malnutrition.

The total body potassium (TBK) and potassium retention were measured during the early stages of recovery of malnourished children on three levels of potassium intake. A potassium intake of 2 mEq/kg/day resulted in significantly greater potassium retention than one of 3 mEq/kg/day. A higher level of intake (15 mEq/kg/day) did not result in an absolute increase in potassium retention and is not recommended because of its theoretical dangers, although it may have stimulated an increase in potassium capacity.

Isotopic evidence of long-lived volcanism on Io.

Jupiter's moon Io hosts extensive volcanism, driven by tidal heating. The isotopic composition of Io's inventory of volatile chemical elements, including sulfur and chlorine, reflects its outgassing and mass-loss history and thus records information about its evolution. We used submillimeter observations of Io's atmosphere to measure sulfur isotopes in gaseous sulfur dioxide and sulfur monoxide, and chlorine isotopes in gaseous sodium chloride and potassium chloride. We find 34S/32S = 0.0595 ± 0.0038 (equivalent to δ34S = +347 ± 86‰), which is highly enriched compared to average Solar System v…

Bartter syndrome in two siblings--antenatal and neonatal observations.

Bartter syndrome was diagnosed in two siblings born to healthy unrelated parents. Each pregnancy was complicated by severe polyhydramnios. The first child was treated with indomethacin from the age of then weeks on. At the age of six years he is doing very well: height is 109.9 cm (P3) and weight 17.8 (P3). Studies of the amniotic fluid during the mother's second pregnancy showed high chloride concentrations (112, 117, and 119 mEq/l), normal levels of sodium, potassium, calcium and creatinine and low prostaglandin E2 (5.0-22.3 pg/ml) and F2 alpha (36-71.7 pg/ml) concentrations. Evidence for functional atypical nicotinic receptors that activate K+-dependent Cl- secretion in mouse tracheal epithelium.

The present study focused on the influence of nicotinic acetylcholine receptors (nAChR) on ion transport processes in mouse tracheal epithelium. RT-PCR experiments revealed expression of the α3, α4, α5, α7, α9, α10, β2, and β4 nAChR subunits in mouse tracheal epithelium. In Ussing chamber recordings of mouse tracheae, apically applied nicotine (100 μM) induced a dose-dependent increase of the transepithelial short-circuit current (EC(50): 14.6 μM). The nicotine-induced effect (I(NIC)) was attenuated by mecamylamine (25 μM, apical) and methyllycaconitine (1 μM, apical). The nAChR agonist 1.1-di

Patch testing in cases of eczema and dermatitis in Beijing, China.

Of 124 patients with eczema and dermatitis patch tested by the European standard series of allergens in the period from April to October 1989, 59 cases showed sensitivity to 1 or more allergens. Potassium dichromate, nickel sulphate and cobalt chloride were the 3 major sensitizers, with a frequency of 17.9%, 13.8% and 10.6%, respectively. The office and factory workers showed a higher incidence of positive reactions than the student group. The environment of the office and the factory seems to be the potential source of exacerbating factors of eczematous skin disease.

Studies on contact sensitivity to chromium in the guinea pig. The role of valence in the formation of the antigenic determinant.

Guinea pigs sensitized with either the trivalent chromium chloride or the hexavalent potassium dichromate are capable of reacting in vivo and in vitro to challenges with both chromium salts. This double reactivity is retained also after repeated restimulations with only 1 of these chromium compounds. From the failure to select lymphocytes directed specifically against a chromium determinant of a particular valence it is concluded that by sensitization with chromium salts of different valences a common determinant or closely related determinants are formed. It is suggested that this determinant

Identification of susceptibility to malignant hyperpyrexia.

While the serum level of creatine phosphokinase is useful as a screening test for malignant hyperpyrexia it does not provide certain identification of susceptible individuals. A much more accurate prediction may be made by pharmacological testing in vitro of muscle biopsy specimens. Individuals susceptible to malignant hyperpyrexia have muscle with heightened sensitivity to halothane, caffeine, succinylcholine, potassium chloride, and temperature change. Use of this test allows separation of susceptible individuals from those not at risk in families of patients who have experienced malignant h…
