Tracheal pressure triggering a demand-flow continuous positive airway pressure system decreases patient work of breathing.

OBJECTIVES: Triggering a ventilator "ON" at the carinal end of the endotracheal tube decreases imposed work of breathing by bypassing the resistance imposed by the breathing circuit and the endotracheal tube. We compared work of breathing during spontaneous ventilation between three methods of triggering the ventilator "ON": a) conventional pressure triggering from inside the ventilator; b) flow-by triggering; or c) tracheal pressure triggering at the carinal end of the endotracheal tube. We hypothesized that the work of breathing would be substantially decreased with tracheal pressure trigger…

Current role of automated ambulatory blood pressure and self-measured blood pressure determinations in clinical practice.

OBJECTIVE: To discuss the clinical indications for use of automated indirect blood pressure measurement (ABPM) and self-monitoring of blood pressure. DESIGN: Available equipment, variations in blood pressure, and settings in which ABPM may be useful are reviewed. RESULTS: Measurement of blood pressure in the physician's office may not reflect the usual blood pressure in other nonmedical environments, such as at work, at home, or during sleep. Self-measurement of blood pressure at home and work and ABPM can provide this additional information. These procedures can be useful not only for deter…

Age and blood pressure measurement: experience with the TM2420 ambulatory blood pressure monitor and elderly people.

The accuracy of the TM-2420 ambulatory blood pressure monitor was assessed in elderly people. Ninety-four subjects (44 men and 50 women), aged 60-94 with systolic blood pressure (SBP) of 97-208 mmHg and diastolic blood pressure (DBP) of 45-109 mmHg, including 23 with isolated systolic hypertension, were studied in three centres. The monitor was compared simultaneously with pairs of observers using the Hawksley random zero sphygmomanometer. The standard deviation of the difference (SDD) between observers was 4.2 mmHg (SBP), 2.9 mmHg (DBP). The mean difference was 0.49 mmHg (SBP) and 0.27 mmHg (…

Reduction of peak inspiratory pressure using high frequency jet ventilation and pressure control ventilation following pneumonectomy.

High peak inspiratory pressure (PIP) during mechanical ventilation is associated with increased risk of barotrauma. High frequency jet ventilation (HFJV) and pressure control ventilation (PCV) have been advocated for the reduction of PIP. The Food and Drug Administration has approved HFJV, respiratory frequency as high as 150 breaths per minute (bpm); however, bpm greater than 150 are still considered for experimental use. At less than 40 bpm, the point where HFJV is no longer considered to be high frequency, PCV is substituted which then becomes the mode of choice because of the ability to co…

[Quality assurance in Riva-Rocci blood pressure measurement: simultaneous sphygmomanometry with open and covered pressure display].

The indirect measurement of arterial blood pressure according to Riva-Rocci is one of the most frequently performed medical diagnostic procedures. When properly performed, this method yields accurate readings compared with intraarterial measurements. However, measurements by means of conventional sphygmomanometers are subject to observer error and variability. Therefore, to eliminate important sources of bias (terminal digit preference of the observer, rate of inflation and deflation of the cuff), and to quantify interobserver variability, a new sphygmomanometer has been developed. The new ins…

Influence of transmural pressure on retrograde pressure pulse transmission velocity in the canine superior vena cava.

The influence of intrathoracic and intraluminal pressure on venous pressure pulse propagation velocity in the superior vena cava was investigated in acute canine experiments. The propagation velocity of distinct points of the venous pressure curve was measured under different conditions of preload, during expiratory and inspiratory apnea and during positive-negative pressure respiration. Under control conditions transmission velocities varied from 1.07 m/s to 3.29 m/s. After volume expansion propagation velocities rose significantly and varied from 1.09 m/s to 6.48 m/s. During expiratory apnea…

Machine learning-based detection of missed inspiratory efforts using esophageal pressure during noisy pressure support ventilation.

BACKGROUND: Missed inspiratory efforts represent one of the most frequent and clinically relevant forms of patient-ventilator asynchrony during assisted mechanical ventilation. Their detection typically requires expert inspection of ventilator waveforms or the use of esophageal pressure (PESO), which limits continuous bedside recognition. Machine-learning approaches may enable automated, real-time identification of these events. METHODS: In this pilot observational physiological study, mechanically ventilated patients receiving pressure support ventilation were recorded under three levels of …

Pulsatile intraspinal pressure drives post-traumatic syringomyelia: evidence supporting the CSF pulsatile pressure dynamic equilibrium hypothesis.

OBJECTIVE: Recent discoveries focused on the role of intraspinal pressure (ISP) in metabolite clearance after spinal cord injury (SCI) have initiated intense research on CSF inflow and outflow pathways. This study aimed to investigate whether the pulsatile ISP wave serves as the primary driver of posttraumatic syringomyelia (PTS) formation and progression, and to determine if a novel subarachnoid‑subarachnoid (S‑S) bypass procedure can effectively attenuate this abnormal pulsatile ISP wave in affected PTS patients. METHODS: In this prospective cohort study of patients with PTS, ISP across the…

Effect of Systolic Blood Pressure Measurement Error on the Cost-Effectiveness of Intensive Blood Pressure Targets.

BACKGROUND: Analyses of clinical trials find that an intensive systolic blood pressure (SBP) target of less than 120 mm Hg is cost-effective compared with a target of less than 140 mm Hg for patients at high cardiovascular disease risk. However, guidelines from the American College of Cardiology and American Heart Association recommend a target of less than 130 mm Hg, citing blood pressure measurement error in routine practice. OBJECTIVE: To evaluate the effect of measurement error on the cost-effectiveness of intensive SBP targets. DESIGN: Microsimulation model varying SBP measurement error…

Rat retinal function attenuation with IOP elevation is impacted by both blood pressure and intracranial pressure.

INTRODUCTION: To consider how blood pressure and intracranial pressure modify the way that retinal function responds to intraocular pressure elevation in rats. METHODS: Six groups of adult Long-Evans rats (n = 7-11 eyes/group, total animals 25) were anesthetized and underwent acute pressure modification. Blood pressure (BP) was measured via a femoral artery cannula and elevated by angiotensin II infusion into the femoral vein. Intracranial pressure (ICP) was set to 0 mmHg, 5 mmHg, or 25 mmHg in three separate groups of rats via a cannula in the lateral ventricle. At each ICP (-5 mmHg, 5 mmHg,…
