Today's treatments: how do you choose?

OBJECTIVE: To provide a view on how the clinician can select appropriate treatment when managing individual patients with endometriosis. METHODS: Review of randomized controlled trials and personal experience. RESULTS: The main determinants of therapy choice are personal experience and patient acceptability. Placebo-controlled trial results support the use of naproxen, dydrogesterone, danazol and leuprolide for pain relief. Laser laparoscopy is more effective than expectant management for pain relief. In direct comparisons, oral contraceptives, Zoladex, danazol, gestrinone, nafarelin and leu

Treatment of bladder pain syndrome and interstitial cystitis: a systematic review.

INTRODUCTION AND HYPOTHESIS: Bladder pain syndrome/interstitial cystitis (BPS/IC) has various treatments; however, no standardized treatment has been established. The aim was to analyze different types of treatment of BPS/IC and their effectiveness. METHODS: A literature review with a search strategy for articles related to BPS/IC published between 1990 and 2014 was conducted on MEDLINE, PUBMED, and SCOPUS. Only randomized controlled trials in women were included in the meta-analysis, while other experimental studies were used as bases for a systematic review of the topic. Clinical trial qual

Effects of whole-body vibration training on resting blood pressure in adults: a systematic review and meta-analysis of randomised controlled trials.

OBJECTIVE: To investigate the effect of whole‑body vibration training (WBVT) on resting blood pressure (BP) in adults and its moderating factors through a meta‑analysis. METHODS: PubMed, Cochrane Library, Embase, and Web of Science were systematically searched up to 2 December 2025, with systolic BP (SBP) and diastolic BP (DBP) as the primary outcomes. Using Stata 17.0 software, effect sizes were pooled with a random‑effects model, and results were reported as weighted mean differences (WMDs) with 95% confidence intervals. RESULTS: This meta-analysis included 14 randomised controlled trials

Adjunctive non-pharmacological interventions for motor symptoms in Parkinson's disease: a scoping review of randomized controlled trials.

BACKGROUND: Motor symptoms in Parkinson's disease (PD), particularly gait, balance, freezing, and axial impairment, can remain troublesome despite optimized dopaminergic treatment. The literature on non-pharmacological adjuncts is diverse, and the label "complementary and alternative therapy" is used inconsistently across conventional rehabilitation, mind-body practices, acupuncture, cueing, and neuromodulation. OBJECTIVE: To map randomized evidence on adjunctive non-pharmacological interventions for PD motor symptoms, while distinguishing complementary/integrative practices from conventional

Comparison of Fasting and Nonfasting Regimens Outcomes Before Catheterization Laboratory Procedures: A Systematic Review and Meta-Analysis.

Guidelines recommend fasting before catheterization laboratory (cath lab) procedures. However, recent studies question the necessity of fasting, suggesting that fasting may not provide significant clinical benefits. This study aims to evaluate the existing evidence between outcomes for patients with fasting and nonfasting regimens. We conducted a comprehensive search for randomized controlled trials comparing fasting and nonfasting protocols before cath lab procedures. Outcomes included hypoglycemia, aspiration pneumonia, contrast nephropathy, all-cause mortality, and cardiovascular mortality.

Data sharing in stepped-wedge cluster randomized trials: suboptimal data availability despite "data available upon request".

BACKGROUND AND OBJECTIVES: Data sharing enhances transparency, facilitates reproducibility, and promotes innovation in health research. For statisticians, access to data from real trials is essential to develop, validate, and refine statistical methods. Within a collection of published stepped-wedge cluster randomized trials (SW-CRTs), we aimed to describe the prevalence and types of data sharing statements, the actual availability of data after emailing authors, and factors associated with data obtainment. METHODS: We identified SW-CRTs published between 2016 and 2022 from a previous systema

Evaluating the utility of melatonin in spine surgery: a systematic review and meta-analysis of randomized clinical trials.

BACKGROUND: Spine surgery is increasingly performed worldwide, and acute postoperative stressors such as pain and anxiety remain highly prevalent despite historical management with opioids and other pharmacological agents. Recently, interest has emerged in melatonin administration given its endogenous physiological roles, low cost, favorable adverse event profile, and documented benefits throughout surgical literature. PURPOSE: This study aims to consolidate the existing evidence on melatonin's utility specifically in spine surgery, an area not yet comprehensively evaluated, to inform clinica

Using marketing strategies to improve recruitment and retention in clinical trials: a scoping review.

BACKGROUND: Participant recruitment and retention into randomised trials (RCTs) is key to the success of any trial yet remains a major challenge. Recruiting and retaining trial participants is akin to an organisation trying to engage with new audiences. Marketing is one way in which organisations engage with audiences as a strategic approach to create, communicate, deliver and exchange offerings of value to consumers. Clinical trials could benefit from strategically implementing a broad array of principles and practices from marketing theory. It could enable trialists to better understand how

Effectiveness of savoring interventions: A systematic review and meta-analysis of randomized controlled trials.

Savoring, a positive psychology intervention, has gained growing attention for its potential to enhance positive emotions and well-being while alleviating negative emotional symptoms such as depression and anxiety. This systematic review and meta-analysis synthesized findings from randomized controlled trials (RCTs) evaluating the efficacy of savoring interventions on emotional outcomes. Twenty independent RCTs with 4805 participants were included, yielding 45 effect sizes. A three-level random-effects model with robust variance estimation (CR2) was employed to account for statistical dependen

The impact of arts-based interventions on alleviating motor and non-motor symptoms in Parkinson's disease: A meta-analysis and systematic review.

Grounded in the conceptual framework of arts-based rehabilitation, this study systematically evaluated the effectiveness of arts-based interventions (ABIs) in alleviating motor and non-motor symptoms among individuals with Parkinson's disease (PD). A systematic review and meta-analysis were conducted following PRISMA and Cochrane guidelines by searching PubMed, Web of Science, Embase, and the Cochrane Library through October 2025. Thirty-four randomized controlled trials (RCTs) were included. Meta-analysis suggested that ABIs significantly improved motor symptoms, as indicated by reductions in

Stratification in small randomised clinical trials and analysis of covariance: Some simple theory and recommendations.

A simple device for balancing for a continuous covariate in clinical trials is to stratify by whether the covariate is above or below some target value, typically the predicted median. The object is to improve balance of the covariate and hence efficiency of the treatment estimate particularly if the trial is small, as may be the case if the disease in question is rare. This raises an issue as to which model should be used for modelling the effect of treatment on the outcome variable, Y. Should one fit, the stratum indicator, S, the continuous covariate, X, both or neither? When a covariate is

Fan Therapy for Dyspnea in Palliative and Supportive Care: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Up to 70% of patients near the end of their lives experience dyspnea, a common and upsetting symptom of advanced disease. Fan therapy is a straightforward nonpharmacologic intervention that may provide symptom relief, although trial results have been mixed. To address this uncertainty, we conducted a systematic review and meta-analysis of randomized controlled trials (RCTs), including both parallel-group and crossover designs, evaluating face-directed fan therapy in adults with advanced disease, such as advanced cancer, COPD, interstitial lung disease, or heart failure. METHODS: W

Non-pharmacological interventions for antipsychotic drug-related metabolic abnormalities: a systematic narrative review.

BACKGROUND: Antipsychotic treatment is commonly associated with weight gain, disturbances in glucose and lipid metabolism, and an increased risk of cardiovascular disease, representing a major clinical challenge in long-term psychiatric care. In recent years, randomised controlled trials have evaluated a range of non-pharmacological interventions targeting antipsychotic drug-related metabolic outcomes. However, substantial variability exists across studies in terms of intervention components, outcome measures, and reported effects, resulting in marked heterogeneity of the available evidence. A
