[Anatomical and pathophysiological bases of renal disorders in sickle cell syndrome patients and carriers of the trait].

The process of intraerythrocytic gelling of haemoglobin S is exacerbated by physiochemical and circulatory conditions which dominate in the renal medulla. In the first few years of life, the sickle cell disturbance is reversible by exchange transfusions. At about the age of 15, this is no longer reversible in homozygotes and is replaced by anatomical lesions which gradually destroy the renal functional capacity for concentrating urine, for excreting H+ and K+ ions and for secreting uric acid. The crises are acute rheological events which temporarily accentuate these disorders which appear to b

Clinical implications of sickle-cell trait and glucose-6-phosphate dehydrogenase deficiency in hospitalized black male patients.

To determine whether sickle-cell trait and glucose-6-phosphate dehydrogenase deficiency influence the course and fatality rates of certain diseases requiring hospitalization, especially those associated with thrombotic phenomena, we conducted a co-operative study of 65,154 consecutively admitted, black male patients in 13 Veterans Administration hospitals. The overall frequency of sickle-cell trait was 7.8 per cent and of glucose-6-phosphate dehydrogenase dificiency 11.2 per cent. Both conditions were present in 0.9 per cent of those examined. There were regional, but no age-dependent, differe

Bilateral Avascular Necrosis of the Femoral Head in a Patient With Sickle Cell Trait: A Case Report.

Sickle cell trait (SCT), historically regarded as a benign carrier state, may nonetheless be associated with severe complications such as bilateral avascular necrosis (AVN) of the femoral head in select individuals. This risk is often underrecognized, particularly in regions with high SCT prevalence, leading to delayed diagnosis and management. We report the case of a 43-year-old Yemeni man who presented with progressive bilateral hip pain over eight months without traditional risk factors except smoking. His family history was notable for sickle cell disease and trait. Physical examination re…

Exercise Collapse Associated With Sickle Cell Trait: Diagnosis, Treatment, Prevention, Current Controversies, and Gaps in the Literature.

Sickle cell trait (SCT) is often benign but can be associated with occasional hemoglobin sickling, particularly in the setting of physical exertion. Screening for SCT, while controversial, reduces exertional events and death. Exertional sickling (ES) is the presumed and hypothesized etiology of collapse and death in athletes, where sickling, microvascular occlusion, and muscle necrosis result in ischemia, hyperkalemia, arrhythmia, and fulminant rhabdomyolysis. Due to the lack of clarity surrounding ES, the term exercise collapse associated with SCT (ECAST) was developed to describe individuals…

Action taken upon the detection of sickle cell trait (heterozygous hemoglobinopathy S) in newborn screening in Spain.

INTRODUCTION: Since 2021, sickle cell disease (SCD) has been included in all 17 neonatal screening programmes in Spain. The carrier state for hemoglobin S (HbS) is also detected collaterally; although clinically benign, it appears to increase the risk of certain complications, with genetic counselling being the main reason for its recognition. The Red Cell Disorders Working Group of the Spanish Society of Pediatric Hematology and Oncology (SEHOP) conducted this study to examine current practices in the management of its detection. MATERIALS AND METHODS: A survey was carried out among represen

Coexisting systemic lupus erythematosus with lupus nephritis and sickle cell trait: a case report of rare combination.

INTRODUCTION AND IMPORTANCE: Systemic lupus erythematosus (SLE) and sickle cell disease (SCD) are relatively common disorders but their coexistence in an individual is rare. Because of the similarities in the musculoskeletal, central nervous system, and renal manifestations in both diseases, diagnosis of SLE in patients with SCD can be difficult to establish. Both the SCD and sickle cell trait (SCT) can have a cumulative assault in the progression of chronic kidney disease (CKD). CASE PRESENTATION: We report a case of a 23-year-old female from an endemic region of SCD in Nepal who presented i…

Bilateral Avascular Necrosis of the Hips in a Patient With Sickle Cell Trait and Chronic Alcohol Use.

Sickle cell trait (SCT) is generally considered a benign carrier state, unlike sickle cell disease (SCD), which is frequently associated with complications such as avascular necrosis (AVN). While AVN affects approximately 30% of patients with SCD, it is rarely reported in individuals with SCT and is not well understood. A 29-year-old African American male with SCT presented with progressively worsening hip pain. He reported chronic heavy alcohol use but denied steroid use, trauma or previous sickle cell crises. Physical exam demonstrated severely limited hip range of motion and antalgic gait. …

Delayed Plasmodium falciparum Malaria in Pregnant Patient with Sickle Cell Trait 11 Years after Exposure, Oregon, USA.

Delayed Plasmodium falciparum malaria in immigrants from disease-endemic countries is rare. Such cases pose a challenge for public health because mosquitoborne transmission must be rigorously investigated. We report a case of delayed P. falciparum malaria in a pregnant woman with sickle cell trait 11 years after immigration to the United States.

Modification of deglycerolization procedure improves processing and post-thaw quality of cryopreserved sickle trait red cell concentrates.

Red blood cell (RBC) transfusion is a critical therapy for those with sickle cell disease (SCD). Alloimmunization is frequent for those with SCD and may limit the availability of matched RBC. Cryopreserved RBCs, from family members or donors with a similar RBC antigen profile could provide a viable alternative to avoid further alloimmunization and prevent hemolytic transfusion-related events. However, cryopreserved SCD and Sickle Cell trait (S-trait) donor RBC units suffer from reduced recovery following deglycerolization. This study proposes and tests a modified deglycerolization protocol usi…

Incidence of hemoglobinopathies and spatialization of newborns with sickle cell trait in Mato Grosso do Sul, Brazil.

OBJECTIVE: To evaluate the incidence of variant hemoglobins of newborn samples from the Neonatal Screening Center in the state of Mato Grosso do Sul, Brazil, and to analyze the distribution and spatial autocorrelation of newborns with sickle cell trait. METHODS: Samples from 35,858 newborns screened by the Neonatal Screening Center. The samples with inconclusive diagnosis were submitted to electrophoretic, chromatographic, cytological and molecular analyses. The spatial distribution analysis of newborns with sickle cell trait was performed by spatial autocorrelation. RESULTS: A total of 919 …

Acceptability to and Engagement With a Virtual Sickle Cell Trait Education Program (SCTaware): Single-Center Prospective Study.

BACKGROUND: Public health programs are tasked with educating the community on health topics, but it is unclear whether these programs are acceptable to learners. Currently, these programs are delivered via a variety of platforms including in-person, virtually, and over the telephone. Sickle cell trait (SCT) education for parents of children with this trait is one of many education programs provided by the Ohio Department of Health. The novel SCTaware videoconference education program was developed by a research team after central Ohio's standard program transitioned from in-person to telephone…

Physiological jump in erythrocyte redox potential during Plasmodium falciparum development occurs independent of the sickle cell trait.

The redox state of the host-parasite unit has been hypothesized to play a central role for the fitness of the intraerythrocytic blood stages of the human malaria parasite Plasmodium falciparum. In particular, hemoglobinopathies have been suggested to cause a more oxidizing environment, thereby protecting from severe malaria. Here we determined the redox potential of infected wild-type (hemoglobin AA) or sickle trait (hemoglobin AS) erythrocytes using parasite-encoded variants of the redox-sensitive green-fluorescent protein 2 (roGFP2). Our non-invasive roGFP2 single-cell measurements revealed

Study of association between sickle cell trait and renal dysfunction among young adults in South-west Nigeria.

BACKGROUND: Although sickle cell disease has become a recognized etiology of chronic kidney disease (CKD), the sickle cell trait (SCT) variant was until recently believed to be a benign carrier state with little or no effect on the health of affected individuals. However, recent studies now appear to suggest an association between SCT and CKD. OBJECTIVE: The objective of the study is to determine the association between SCT (hemoglobin AS) and renal dysfunction among young Nigerian adults. METHODOLOGY: This was a cross-sectional, descriptive study among apparently healthy undergraduates of A…

Transgenic sickle cell trait mice do not exhibit abnormal thermoregulatory and stress responses to heat shock exposure.

There remains controversy over whether individuals with sickle cell trait (SCT) are vulnerable to health risks during physical activity in high temperatures. We examined thermoregulatory and stress-related responses to heat exposure in SCT and wild-type (WT) mice. No significant differences in core temperature (Tc) were observed between SCT and WT mice during heat exposure. There was no correlation between peak Tc during heat exposure and levels of hemoglobin S in SCT mice. Basal levels of circulating inflammatory and stress-related markers were not significantly different between SCT and WT m…
