We report the results of a retrospective study of 108 cases of pleural empyema collected in the Pneumo-Phthisiology Department of the National Hospital of "Point G" from September 1984 to August 1990. The sex-ratio is 3/1 in favour of males with a mean age of 35.5 years. The pleural empyema represent 2.2% of all patients admitted in the Hospital and 20.2% of liquid effusions of the Pleura observed during the study period. Ophthalmomyiasis, infestation of the eye by dipterous fly larvae, may result in sequelae ranging from minor irritation to blindness, disfigurement, and death. Infestation with Cochliomyia hominivorax, a medically significant and economically important species, exemplifies the mechanisms by which larvae can cause tissue damage. Although clinical history may be suggestive of ophthalmomyiasis, the definitive diagnosis requires direct observation of larvae. Natural history of hymenoptera sting allergy is less life threatening in children than in adults. Eighty cases of brain abscess treated in the University Hospital, BHU, Varanasi, India have been reviewed. Chronic suppurative otitis media was the commonest cause, followed by compound injuries. The overall mortality was 15%. In seven cases the causative organism was pseudomonas, resistant to most antibiotics. Prior to the availability of CT the mortality was 23.3%; after the routine use of CT for diagnosis the mortality fell to 10%. A high mortality (57%) was observed in patients who had pseudomonas. The best results were in patients who had been managed by excision of the abscess capsule.

Authors discuss, presenting case reports, the operative indications of Helal's metatarsal serial osteotomy. In their first case this kind of operation was performed because of metatarsalgia, following traumatic amputation of the hallux. In their second case, in a young female with rheumatoid arthritis, considering the age of the patient and the destruction in the metatarsophalangeal joints too, Helal's metatarsus osteotomies were completed with the resection of the basis of the ground phalanx. In both cases good correction was reached and without any complaints.

The keys to a better outcome in the management of ruptured aneurysm are early diagnosis, aggressive resuscitation, and early operation, with prompt achievement of proximal control. The HELLP syndrome--H for haemolysis, EL for elevated liver enzymes and LP for low platelet count, is a serious complication of hypertension of pregnancy. We report five cases of this syndrome. The haemolysis, which is the consequence of a microangiopathic haemolytic anemia, is not always present: we report four such cases. The recognition of the HELLP syndrome even in the absence of hypertension is important and must be thought of when epigastric or dorsal pain occur in the second half of pregnancy. No liver biopsy was done in this small series.

High endothelial venules (HEV) are specialized postcapillary venules that play a central role in lymphocyte migration and recirculation. This study examined the occurrence and morphology of HEV-like vessels in healthy gingiva and in lesions of chronic gingivitis in children, experimental gingivitis in adults, adult periodontitis, and in periapical granulomas. Iatrogenic vascular injuries occurring at our institution were reviewed and several special problems not previously well described were found. These include carotid and femoral pseudoaneurysms, occult hemorrhage and knotting of the angiographic catheter.
