[A case of tuberculoma of the orbit with sphenoidal origin].

We report the case of an orbital and sphenoidal tuberculosis observed in a young Indian man living in France. The orbital disease is presenting as an orbital mass with a radiologic bony lytic aspect, without systemic manifestation. A malignant process was suspected. The diagnosis was obtained by the culture for Mycobacterium tuberculosis of the biopsy specimen. Orbital involvement in tuberculosis is a rare manifestation in developed countries. Most cases reported in the literature were reported from Asia and Africa. Orbital tuberculosis may become established in one of two ways: primarily by h

Sphenoid wing meningioma occurring as a lateral orbital mass.

The differential diagnosis of a mass presenting in the lateral aspect of the orbit with bony erosion and involving the skin includes many malignancies, predominantly sarcomas. A total evaluation of the mass to include histopathologic characteristics and the true extent of the disease is critical to proper management of the patient. A 68-year-old man was referred for therapy with a diagnosis of fibrosarcoma. Evaluation of the disease revealed contiguous tumor from the right temporal region into the posterior orbit, with displacement of the globe and extension intracranially. During operation a

Carcinoma of prostate presenting as intracranial tumor with multiple cranial nerve palsies.

Carcinoma of the prostate is one of the commonest of cancers occurring in men over the age of fifty years. A small focus of cancer in the prostate can be active biologically and give rise to metastases without producing any symptoms of bladder neck obstruction. In such cases, the attention may be directed away from the prostate. A review of literature reveals that practically no organ in the body is exempt from metastasis from a carcinoma of the prostate. A routine rectal examination and if necessary a biopsy of the prostate should not be omitted in any patient beyond the age of fifty years. A

Traumatic false aneurysm and carotid-cavernous fistula: a complication of sphenoidotomy.

Severe head injury with resultant basilar skull fracture is the most common etiology for traumatic false aneurysm of the cavernous carotid artery and for traumatic carotid-cavernous fistula. We present a case of false aneurysm and fistula that occurred as a complication of sphenoidotomy, fortunately a rare etiology of these two serious problems. Because of the current resurgence of trans-sphenoidal surgery, it is of utmost importance that all neurosurgeons possess an accurate and detailed knowledge of the anatomy of the carotid artery and its intracavernous route.

Endoscopic Endonasal Transpituitary Gland Approach for Resection of Dorsum Sellae Meningioma - Technical Case Report.

BACKGROUND AND IMPORTANCE: Access to a dorsum sellae meningioma is difficult. A transcranial approach, such as a pterional, subtemporal, or transpetrosal method, often requires significant brain retraction and crossing of cranial nerves to access this region. We present here a successful purely endoscopic endonasal transpituitary gland approach for resection of a growing dorsum sellae meningioma. CLINICAL PRESENTATION: A 74-yr-old woman came to us with dizziness. Magnetic resonance imaging (MRI) demonstrated a mass on the dorsum sellae around the left posterior clinoid. Follow-up MRI examinat

Efficacy of ultra-short single agent regimen antibiotic chemo-prophylaxis in reducing the risk of meningitis in patients undergoing endoscopic endonasal transsphenoidal surgery.

OBJECTIVES: The study aims to evaluate the incidence of infectious complications (namely meningitis) within 30 days after endoscopic endonasal transspheinodal neurosurgery (EETS) in patients receiving an ultra-short peri-operative chemo-prophylaxis regimen with 2 doses of 1st generation cephalosporin or macrolide. PATIENTS AND METHODS: We retrospectively analyzed the clinical records of 145 patients who received an ultra-short chemoprophylaxis with two doses of an antibiotic, given 30 min before and 8h after EETS, over a 30-month time-frame. Ninety-seven patients (66.89%) received endovenous

Sphenoid sinus mucocele causing third nerve paralysis: CT and MR findings.

A 64-year-old woman presented with a 2 week history of ptosis and medial-gaze paralysis of her left eye. Computed tomography scanning of the paranasal sinuses revealed an expansile, homogeneous lesion with no contrast enhancement in the sphenoid sinus. The mass was hyperintense on all MR sequences and there was extension of the mass to the left cavernous sinus and optic canal. Operation revealed a large mucocele. Third nerve palsy disappeared 4 weeks after operation. Because the spread of mucoceles is variable, they may cause different symptoms. Radiological evaluation, especially computed tom

Pathogenesis and treatment of a giant occipital bone defect with meningoencephalocele in an NF1 child: case report and review of the literature.

Autosomal dominantly inherited neurofibromatosis type I (NF1) is a systemic disorder caused by a mutation of a gene on chromosome 17q11.2 and characterized by multiple café-au-lait spots, lentiginous macules, Lisch nodules of the iris, and tumors of the nervous system. Bony manifestations such as scoliosis, dysplasia of the greater sphenoidal wing, tibial pseudoarthrosis, short stature, and macrocephaly have been reported in approximately 50% of patients. However, calvarial bone defects are rare. After screening 324 articles, 23 cases (12 adult and 11 pediatric patients) of occipital bone defe

Combined cranio-facial fractures.

Clinically, fronto-maxillary injuries may constitute a diagnostic problem, as their severity need not correlate with the patient's general condition. To establish a definitive radiological diagnosis, both normal standard films and tomographs are required. These will help to identify fracture lines involving the base of the skull. Most serious among the complications which may be associated with fronto-maxillary injuries is the occurrence of cerebrospinal rhinorrhoea with potential ascending infection. Other complications include oculomotor dysfunction, obstruction of lacrimal drainage and nasa

Surgical anatomy of the pterion and its relationship to the middle meningeal artery in optimizing pterional craniotomies: a cadaveric perspective.

PURPOSE: The pterion is an anatomic landmark formed via junction of the temporal, sphenoid, parietal, and frontal bones on the lateral aspect of the cranium. Due to the proximity to the middle meningeal artery (MMA), pterion-based surgical approaches have a potential risk of arterial rupture, leading to subsequent epidural hematoma formation. This study characterized the localization of the pterion and its relationship to the MMA in the four main pterion configurations. METHODS: Ninety-one pterions were exposed, and suture patterns were uncovered to determine pterion classification. Dura mate

Nondestructive, epi-illumination surface microscopic characterization of surface discontinuity in bone: a new approach offers a descriptive vocabulary and new insights.

Perspectives are in the eye of the beholder and are expanded with increased scrutiny (e.g., magnification). The term "porosity' for macroscopically or radiologically identifiable discontinuity in bone is so variably applied as to be incomprehensible in its meanings. A closer examination of surface discontinuity seems appropriate. Histological and atomic level analysis of bone alterations misses the forest for the trees and the former destroys the very subject it attempts to characterize. Surface discontinuity was therefore evaluated at the three-dimensional submacroscopic level. An epifluoresc

Severe posterior epistaxis-endoscopic surgical anatomy.

OBJECTIVE: To describe the anatomy of the sphenopalatine foramen (SPF) region and possible anatomical variations. STUDY DESIGN: Prospective study accomplished from September, 2006, to January, 2007. METHODS: The sphenopalatine foramen (SPF) of 61 cadavers were carefully dissected. Presence of the ethmoidal crest, location of sphenopalatine and accessory foramens, and the number of arterial branches emerging through foramens were observed. Data were analyzed in relation to gender, racial group, and symmetry of the cadaver. Prediction of the presence of accessory foramen was evaluated. RESULT

[Primary osteosarcoma of the base of the skull in Garcin syndrome].

This contribution presents a case study of a 17-year-old female patient with unilateral failure of several brain nerves within a period of five months. Further findings included fleeting homolateral signs of the small brain and, during the terminal phase, discrete hemiparesis. Diagnosis was based on the tomograms of the skull base and computerized tomography, which revealed a tumour at the base of the skull. During the obduction this was found to be a primary osteosarcoma. According to the literature it is rare for this kind of tumour to lead to Garcin 's syndrome.

[The present state of frontal skull-base surgery (author's transl)].

Our experience in 125 cases of traumatic frontal skull-base and mid-face lesions is discussed. 88 patients were operated on immediately after injury whereas 37 were operated on when the presented with complications due to the trauma some time after the injury. Small aeroceles seen on the computer tomogram are a definite and accurate sign of a dural rupture. The indications for an exploration of the frontal skull-base should be rather liberal since most dura lesions are difficult to verify. The current surgical techniques to repair the posterior wall of the frontal sinus and the cribriform plat
