Geometric Model for the Postural Characterization in the Sagital Plane of Lumbar Raquis.

The individual's posture is the physical expression of his body. It is modified throughout life and it is determined by the particular anatomical characteristics that directly affect the biomechanics of the spine. The typing of the spinal curvature is important for the knowledge of body posture. The possibility of having a method for the systematic postural characterization of the spine is an essential objective resource in order to obtain normal or control patterns of the spinal morphology of the population. A widely accepted methodology of morphological characterization of the spine is a nec

The vertebral fracture cascade in osteoporosis: a review of aetiopathogenesis.

Once an initial vertebral fracture is sustained, the risk of subsequent vertebral fracture increases significantly. This phenomenon has been termed the "vertebral fracture cascade". Mechanisms underlying this fracture cascade are inadequately understood, creating uncertainty in the clinical environment regarding prevention of further fractures. The cascade cannot be explained by low bone mass alone, suggesting that factors independent of this parameter contribute to its aetiopathogenesis. This review explores physiologic properties that may help to explain the vertebral fracture cascade. Diffe

Etiology of lumbar lordosis and its pathophysiology: a review of the evolution of lumbar lordosis, and the mechanics and biology of lumbar degeneration.

The goal of this review is to discuss the mechanisms of postural degeneration, particularly the loss of lumbar lordosis commonly observed in the elderly in the context of evolution, mechanical, and biological studies of the human spine and to synthesize recent research findings to clinical management of postural malalignment. Lumbar lordosis is unique to the human spine and is necessary to facilitate our upright posture. However, decreased lumbar lordosis and increased thoracic kyphosis are hallmarks of an aging human spinal column. The unique upright posture and lordotic lumbar curvature of t

Congenital kyphosis in thoracic spine secondary to absence of two thoracic vertebral bodies.

Congenital thoracic kyphosis at D6-D7 vertebral body level has been encountered in an 18-month-old boy without neurological deficit. The constellation of the spine malformation complex such as incomplete development of the vertebral bodies associated with defective ossification of the thoracic pedicles causing effectively the development of spinal cord injury at the kyphotic level of D6-D7 were present. Congenital kyphosis in thoracic spine secondary to absence of two thoracic vertebral bodies has been reported in a few studies. It is the absence and asymmetry of growth plates that may contrib

Idiopathic scoliosis.

BACKGROUND: Scoliosis is a three-dimensional deviation of the spinal axis. The main diagnostic criterion is spinal curvature exceeding 10° on a plain anteroposterior X-ray image. Scoliosis is called idiopathic when no other underlying disease can be identified. METHODS: Selective literature review and recommendations of the relevant medical societies in Germany and abroad. RESULTS: Scoliosis in children of school age and above primarily occurs in girls. Its prevalence is 1% to 2% among adolescents, but more than 50% among persons over age 60. The therapeutic goal in children is to prevent pr

The spine and surgical treatment in osteogenesis imperfecta.

In the osteogenesis imperfecta patient, spinal deformity should be expected, particularly in the severely affected nonambulator with chest deformity. Curves develop early (age five to six) and generally progress rapidly. Early bracing, although somewhat effective, may well compress the soft osteoporotic rib cage without controlling the spinal curvature. The pulmonary compromise created by the scoliosis is compounded by the chest cage deformity secondary to bracing. In the patient with severe disease (thin bones and numerous fractures), posterior correction and fusion, with or without Harringto

Spinal kinematics during gait in healthy individuals across different age groups.

Most studies investigating trunk kinematics have not provided adequate quantification of spinal motion, resulting in a limited understanding of the healthy spine's biomechanical behavior during gait. This study aimed at assessing spinal motion during gait in adolescents, adults and older individuals. Fourteen adolescents (10-18years), 13 adults (19-35years) and 15 older individuals (≥65years) were included. Using a previously validated enhanced optical motion capture approach, sagittal and frontal plane spinal curvature angles and general trunk kinematics were measured during shod walking at a

Modification of the original Luque technique in the treatment of Duchenne's neuromuscular scoliosis.

Luque segmental instrumentation with Galveston technique for pelvic fixation is generally used in the correction of Duchenne's neuromuscular spinal deformities with pelvic obliquity. Particularly difficult is the control of the lumbopelvic junction. Instrumentation failures and only mediocre correction of pelvic obliquity are reported. To obtain better sacropelvic anchorage and to improve pelvic correction, this technique was modified with the introduction of sacral screws in each S-1 pedicle and a device for transverse traction between the caudal right-angle bends of the L-rods. From 1988 and

The role of genetics and molecular mechanisms in early onset scoliosis.

Early onset scoliosis (EOS), a spinal deformity that occurs before 10 years of age, imposes significant morbidity due to its rapid Cobb angle progression, three-dimensional spinal curvature, and potential respiratory compromise from thoracic cage distortion. This condition, classified into idiopathic, congenital, neuromuscular, and syndromic subtypes, exhibits high phenotypic heterogeneity and multisystem involvement. Current treatments like bracing and surgery focus on modulating curve progression and preserving growth, but the genetic and molecular overview is not clear. This review delineat

Spinal deformity induced by radiotherapy for solid tumours in childhood: a long-term follow up study.

A group of 44 individuals were re-evaluated at a median follow up time of 19 years after radiotherapy for childhood cancer involving some part of the vertebral column. The median age at diagnosis was 2.6 years. The diagnosis included Wilms' tumour (n = 24), neuroblastoma (n = 9) and other solid tumours (n = 11). Scoliosis with or without kyphosis was the most common spinal deformity and was found in 40/44 patients. The apex of the major curvature was on the lumbar vertebral body 1-3 in 23 cases. The kyphosis and lordosis were greater in the subjects with tumour other than Wilms' (P = 0.04 both

Secondary analysis of change in physical function after exercise intervention in older adults with hyperkyphosis and low physical function.

BACKGROUND: Hyperkyphosis is common in older adults and associated with low physical function and reduced health related quality of life (HrQol). Improved kyphosis has been previously established in kyphosis-targeted interventions in randomized controlled trials in older adults with hyperkyphosis; however, evidence for improved physical function is conflicting. Few studies have investigated change in physical function after a targeted kyphosis intervention in older adults with low physical function. The primary aim in this descriptive study was to explore change in physical function after a pr

Advances in the application of spinal anesthesia in patients with spinal structural abnormalities.

Spinal anesthesia is a fundamental and widely adopted technique in modern clinical practice. However, its application in patients with abnormal spinal anatomy remains contentious due to heightened risks of technical failure, neurological injury, and hemodynamic instability. This review synthesizes recent advances in the use of spinal anesthesia in this complex patient population, providing clinical insights and practical recommendations derived from a comprehensive analysis of over 40 studies retrieved from the PubMed and Web of Science databases. Findings suggest that subarachnoid block may o

Simultaneous Anterior and Posterior Release in Lateral Decubitus Position for Rigid Adult Spinal Deformity: A Technical Note and 2 Case Reports.

BACKGROUND: Correction surgery for rigid adult spinal deformity usually involves a complex 360° osteotomy, multiple intraoperative position changes, and staged surgery. Moreover, there is a lack of consensus regarding the surgical strategy for this pathology. We report the technical advantages of a simultaneous anterior and posterior release only in the lateral decubitus position to reduce surgical invasiveness in two case reports. CASE DESCRIPTION: A 76-year-old woman and an 80-year-old woman presented with significant spinal imbalance and segmental fusion in the anterior and posterior colum
