Serum C reactive protein (CRP) and erythrocyte sedimentation rate (ESR) were measured on 469 occasions in 149 patients with ankylosing spondylitis who had also been investigated for faecal carriage of klebsiella/enterobacter species on the same occasions. Raised values of CRP and ESR correlated with each other (P less than 0.001) as well as with clinically assessed active disease episodes (P less than 0.001). Nine cases from among 64 patients with ankylosing spondylitis (AS) are described. In addition to bilateral sacroiliitis these cases had a peculiar type of spondylodiscitis characterised by quite diffuse and marked sclerosis of multiple vertebral bodies, with only minimal erosions of the adjacent vertebral plates while classical syndesmophytosis was absent. The antigen HLA-B27 was found only in 1 of these 9 cases. This type of spondylodiscitis could discriminate among all the patients with AS a subgroup with a peculiar clinical pattern and a probably distinctive pathological mechanism.

The results of surgical treatment of 39 patients with tuberculous spondylitis, complicated with bronchial fistula, are analysed. The fistula was liquidated in 35 patients. Late results followed up from 3 to 18 years were studied in 26 patients. Bronchial fistula is yet present in 4 patients, who were discharged with its presence.

Ninety-two patients who satisfied the criteria proposed by Kellgren for the diagnosis of ankylosing spondylitis were re-evaluated by clinical, radiological and laboratory parameters after an average length of illness of 18.7 years. Ankylosing spondylitis (AS) is rarely found in conjunction with connective tissue diseases. Moreover, reports on the coexistence of AS with systemic lupus erythematosus (SLE) are few. Here, we describe such a case of coexisting AS and SLE in a 58-year-old male patient. He presented with a 5-year history of pain in the lower spine. Immunological tests showed positive results for antinuclear and Sm antibodies. The human leukocyte antigen B27 was also positive. Here, we report a case where AS coexisted with SLE.
