Dry eye disease in strabismus patients: Does eye deviation harm ocular surface?

INTRODUCTION: Dry eye disease is a multifactorial disease which affects the ocular surface system, and determines ocular discomfort symptoms and visual disturbance. Various types of ocular surgery, including strabismus one, represents a recognized cause for iatrogenic dry eye. However, it is not uncommon that strabismus patients, even not undergone to surgery, report symptoms of ocular discomfort and dryness. Hypothesis We hypothesize that two possible mechanisms may determine dry eye in strabismus patients by setting up a specific vicious circle: firstly, the increased exposure of the area of…

Staggered sub-Tenon's blocks: a novel anesthetic approach for bilateral strabismus surgery.

Bilateral strabismus surgery is usually performed under general anesthesia. However, sometimes general anesthesia is not in the best interest of patients presenting with advanced multiple comorbidities. These patients are best treated with local anesthesia instead. However, the bilateral nature of the surgery becomes an issue because bilateral akinetic needle blocks for ophthalmic surgery have not been described before. This article describes a novel approach for bilateral eye surgery in three patients who were at high risk from general anesthesia but needed to have the surgery performed in th…

Persistent pupillary membrane, strabismus, and optic nerve hypoplasia in Donnai-Barrow syndrome.

We report a case of Donnai-Barrow syndrome in a 2-year-old boy who presented with exotropia and prominent eyes since birth. Ocular findings included high myopia, persistent pupillary membrane, and optic nerve hypoplasia. Facial manifestations included broad nasal bridge, hypertelorism, and downward-slanting of palpebral fissures. The patient had associated omphalocele, sensorineural hearing loss, and developmental delay. Magnetic resonance imaging revealed agenesis of the corpus callosum. To our knowledge, this is the first reported association of persistent pupillary membrane, strabismus, and…

A VR-based Automated Strabismus Diagnosis System with Progressive Semi-Supervised Learning.

Strabismus is a prevalent ocular disorder that can impair visual development and cause psychological issues if not diagnosed early. Conventional clinical diagnosis primarily relies on the prism cover test (PCT), which is subjective, requires patient cooperation, and lacks standardization. Recent advances in virtual reality (VR) and deep learning offer promising solutions for automated and standardized diagnosis. However, practical deployment faces three key challenges: realistic VR simulation of clinical exams, addressing image degradation (reflections/occlusions) with limited annotated data, …

Oculocardiac Reflex During Strabismus Surgery: Conjunctival Incision versus Standardized Rectus Muscle Traction.

BACKGROUND: Extraocular muscle (EOM) tension during strabismus surgery produces trigemino-vagal bradycardia as a form of the oculocardiac reflex (OCR). Surface pain in the eye can cause bradycardia in premature infant eye examinations, so we evaluated the effect on the heart rate of different aspects of strabismus surgery. METHODS: Electrocardiograph heart rate (H.R.) was monitored during outpatient strabismus surgery without anticholinergic or local anesthetic. The impact of limbus-based conjunctival incision was compared to 200 gram, 10-second tension on rectus muscles. RESULTS: The 239 pa…

Automated strabismus detection and classification using deep learning analysis of facial images.

Strabismus, or eye misalignment, is a common condition affecting individuals of all ages. Early detection and accurate classification are essential for proper treatment and avoiding long-term complications. This research presents a new deep-learning-based approach for automatically identifying and classifying strabismus from facial images. The proposed methodology leverages Convolutional Neural Networks (CNNs) to achieve high accuracy in both binary (strabismus vs. normal) and multi-class (eight-class deviation angle for esotropia and exotropia) classification tasks. The dataset for binary cla…

A Case of Conjunctival Cyst Required Removal Six Months After Strabismus Surgery.

A conjunctival cyst is a rare yet significant complication following strabismus surgery. This report describes a nine-year-old girl who developed a conjunctival cyst after undergoing bilateral lateral rectus recession surgery for intermittent exotropia. Despite an uneventful surgery and standard postoperative care, she presented with a gradually enlarging subconjunctival mass in the left eye three months later. Initial conservative treatment with topical antibiotics and steroids proved ineffective, leading to surgical excision six months postoperatively. Histopathological examination confirmed…

Tendon elongation with bovine pericardium in strabismus surgery-indications beyond Graves' orbitopathy.

BACKGROUND: For some patients with complex ocular motility disorders, conventional strabismus surgery is insufficient. Surgery with tendon elongation allows correction of larger angles and maintains a sufficient arc of contact for rectus muscles. This study reports results for tendon elongation with bovine pericardium (Tutopatch®) in indications other than Graves' orbitopathy in which it is already widely used. METHODS: We reviewed the records of all patients who underwent surgery with Tutopatch® in our institution. Angles of squint and head postures were analyzed preoperatively, on the first…
