Asymmetrical vertebral column decancellation for the management of rigid congenital kyphoscoliosis.

BACKGROUND: Congenital kyphoscoliosis is a disease that often requires surgical treatment. Wedge osteotomies, such as pedicle subtraction osteotomy, are insufficient to correct this complicated rigid deformity. Vertebral column resection yields sufficient correction, but it is an exhaustively lengthy operation with a high risk of major complications. There are few effective and safe techniques for treating rigid congenital kyphoscoliosis. We aimed to investigate the technique of asymmetrical vertebral column decancellation (AVCD) for the treatment of rigid congenital kyphoscoliosis and evaluat

Use of a New Indocyanine Green Pooling Technique for Improved Visualization of Spinal Dural AV Fistula: A Single-Center Case Series.

BACKGROUND: Intraoperative indocyanine green video angiography (ICG-VAG) is a useful tool in cerebral vascular surgery. In spinal procedures such as dural arteriovenous (AV) fistula, use of ICG-VAG is limited due to lower perfusion pressure. Therefore, we developed a new pooling technique with adapted workflow to improve intraoperative visualization. METHODS: Patients operated on spinal dural AV fistulas using ICG-VAG were prospectively included. A new workflow for ICG-VAG was applied: 1) temporary clip placement at the suspected fistula point, 2) ICG administration during 100% oxygenation, 3

Novel posterior technique to temporize life-threatening hemorrhage after great vessel laceration during posterior osteotomy.

PURPOSE: Great vessel injury during posterior 3-column osteotomy is rare, but potentially fatal. Literature review revealed limited data guiding spine surgeons during this major catastrophe. In this study our aim was first, to present our case of mortality; second, to discuss a novel technique that can be performed to temporize hemorrhage in a life-threatening situation where an iatrogenic great vessel injury occurs and hemodynamic stability cannot be achieved through usual means of hemostasis; third, a cadaveric study to determine if this novel technique is feasible. METHODS: Three fresh cad

The comprehensive anatomical spinal osteotomy and anterior column realignment classification.

OBJECTIVESpinal osteotomies and anterior column realignment (ACR) are procedures that allow preservation or restoration of spine lordosis. Variations of these techniques enable different degrees of segmental, regional, and global sagittal realignment. The authors propose a comprehensive anatomical classification system for ACR and its variants based on the level of technical complexity and invasiveness. This serves as a common language and platform to standardize clinical and radiographic outcomes for the utilization of ACR.METHODSThe proposed classification is based on 6 anatomical grades of

The dual-isotope ProstaScint imaging procedure: clinical experience and staging results in 145 patients.

PURPOSE: Whole-body, regional planar, and SPECT imaging using the In-111-labeled monoclonal antibody capromab pendetide (In-111 MAb; ProstaScint) has been shown to increase the detection of early disease spread in patients with prostate cancer. However, recognition of metastatic tumor sites can be difficult, especially if the involved nodes are near blood vessels. We have developed an alternate approach to the identification of metastatic sites that is based on a single simultaneous In-111 MAb and Tc-99m RBC SPECT acquisition of the pelvis and abdomen on day 5 after injection. We have also dev

Differences in orthostatic blood pressure changes measured with an oscillometric blood pressure monitor and a mercury sphygmomanometer.

AIMS: To compare orthostatic blood pressure (BP) changes recorded with the SpaceLabs 90207 BP monitor (SL) and the standard mercury sphygmomanometer (HgS). METHODS: 85 hospital in-patients aged 60-90 years had nine BP measurements recorded by both instruments using the same arm sequential measurement technique in supine and standing position by two observers. Supine BP was taken as the final set of three supine measurements, ie, one made with the SL, and the mean of the two HgS readings immediately before and after the monitor reading. From the SL supine reading was subtracted the three stand

Strategies for prevention of rod fracture in adult spinal deformity: cobalt chrome rod, accessory rod technique, and lateral lumbar interbody fusion.

OBJECTIVE: Restoring the proper sagittal alignment in adult spinal deformity (ASD) can improve radiological and clinical outcomes, but pseudarthrosis including rod fracture (RF) is a common problematic complication. The purpose of this study was to analyze the methods for reducing the incidence of RF in deformity correction of ASD. METHODS: The authors retrospectively selected 178 consecutive patients (mean age 70.8 years) with lumbar degenerative kyphosis (LDK) who underwent deformity correction with a minimum 2-year follow-up. Patients were classified into the non-RF group (n = 131) and the

Surgical correction of a rigid sagittal and coronal plane cervical-thoracic deformity.

BACKGROUND: Rigid cervical-thoracic sagittal and coronal plane deformities are uncommon but profoundly debilitating conditions, impairing horizontal gaze and diminishing quality of life. Progressive kyphosis can result from many etiologies, including interbody cage subsidence following anterior cervical discectomy and fusion (ACDF), resulting in loss of anterior column support and eventual fusion in a malaligned position. Surgical correction of these complex deformities requires meticulous planning, and an approach tailored to the severity of the deformity. Posterior-based approaches, includin

Clinical utility of circulating tumor-derived endothelial cells in lung cancer detection and prognosis prediction.

Circulating tumor-derived endothelial cells (CTEC) have emerged as promising biomarkers reflecting tumor angiogenesis, disease progression and therapeutic response, although their clinical relevance in lung cancer remains underexplored. A total of 74 patients with lung cancer were enrolled in the present study, and both preoperative and postoperative peripheral blood samples were collected. A total of 20 healthy individuals were included as controls. CTEC (CD31+ aneuploid cells) and circulating tumor cells (CTC; CD31- aneuploid cells) were simultaneously detected using an improved subtraction

Single-position surgery with intraoperative digital subtraction angiography via a transpopliteal approach for complex spinal vascular lesions: illustrative cases.

BACKGROUND: Intraspinal vascular lesions, including dural arteriovenous fistulas (DAVFs) and arteriovenous malformations (AVMs), require accurate intraoperative localization and complete obliteration to prevent irreversible neurological deficits. Conventional transfemoral digital subtraction angiography (DSA) is often limited by patient repositioning during spinal surgery. This study describes a single-position technique integrating microsurgical resection with intraoperative DSA via a transpopliteal approach in the prone position. OBSERVATIONS: Three patients (1 AVM, 2 DAVFs) underwent micro

Diagnosis of Carotid Fibromuscular Dysplasia Using Transoral Ultrasound in the Intensive Care Unit.

Fibromuscular dysplasia (FMD) is a rare, non-atherosclerotic vascular disease affecting medium to large arteries, especially the renal and internal carotid arteries (ICAs). The string-of-beads appearance, indicative of alternating areas of stenosis and dilatation, is a key imaging feature typically observed in the distal ICAs. Diagnosing FMD in critically ill patients poses challenges due to the risks associated with traditional imaging methods such as computed tomography angiography (CTA), magnetic resonance angiography, and digital subtraction angiography. We report a case of a 74-year-old w

Arteriovenous Malformations (AVMs): Molecular Pathogenesis, Clinical Features, and Emerging Therapeutic Strategies.

Arteriovenous malformations (AVMs) are fast-flow vascular malformations formed by direct artery-to-vein shunts without an intervening capillary bed, which increases the risk of hemorrhage and organ-specific damage. A synthesis of recent advances shows that AVMs arise from interplay between germline susceptibility (ENG, ACVRL1, SMAD4, RASA1, EPHB4), somatic mosaicism (KRAS, MAP2K1, PIK3CA), perturbed signaling (TGF-β/BMP, Notch, VEGF, PI3K/AKT, RAS/MAPK), hemodynamic stress, and inflammation. Multimodal imaging-digital subtraction angiography (DSA), MRI/MRA with perfusion and susceptibility seq

Multi-mode information fusion navigation system for robot-assisted vascular interventional surgery.

BACKGROUND: Minimally invasive vascular intervention (MIVI) is a powerful technique for the treatment of cardiovascular diseases, such as abdominal aortic aneurysm (AAA), thoracic aortic aneurysm (TAA) and aortic dissection (AD). Navigation of traditional MIVI surgery mainly relies only on 2D digital subtraction angiography (DSA) images, which is hard to observe the 3D morphology of blood vessels and position the interventional instruments. The multi-mode information fusion navigation system (MIFNS) proposed in this paper combines preoperative CT images and intraoperative DSA images together t

Load-sharing biomechanics of lumbar fixation and fusion with pedicle subtraction osteotomy.

Pedicle subtraction osteotomy (PSO) is an invasive surgical technique allowing the restoration of a well-balanced sagittal profile, however, the risks of pseudarthrosis and instrumentation breakage are still high. Literature studied primary stability and posterior instrumentation loads, neglecting the load shared by the anterior column, which is fundamental to promote fusion early after surgery. The study aimed at quantifying the load-sharing occurring after PSO procedure across the ventral spinal structures and the posterior instrumentation, as affected by simple bilateral fixation alone, wit

Diploic arteriovenous fistulas with marked cortical venous reflux.

Diploic arteriovenous fistulas are rare arteriovenous shunts involving the skull, which often drain antegradely into the internal or external jugular veins. Diploic arteriovenous fistulas with marked cortical venous reflux are extremely rare. Here, we present the case of a patient with diploic arteriovenous fistulas with marked cortical venous reflux and a literature review. A 73-year-old woman presented with headache. Magnetic resonance angiography revealed abnormal signal intensity in the diploic layer of the left frontal bone. Digital subtraction angiography demonstrated a diploic arteriove
