Molecular diagnosis of Aspergillus fumigatus endocarditis.

A 66-year-old male with ischaemic cardiomyopathy and chronic lymphocytic leukemia developed signs of severe systemic inflammatory response syndrome. Serial blood cultures were negative and a SeptiFast test detected the presence of Aspergillus fumigatus DNA. Afterwards, detection of galactomannan and 1,3-β-D-glucan showed a positive result. Autopsy revealed the presence of branched fungal structures suggestive of Aspergillus.

[Comorbidities of sleep-disordered breathing in children].

Sleep-related respiratory disorders in children, especially childhood sleep apnea-hypopnea syndrome, are associated with a wide range of comorbidities affecting the central nervous system, cardiovascular and metabolic systems, and growth. The two most important factors contributing to the physiopathology of this disorder are intermittent hypoxia and sleep fragmentation, which seem to cause the systemic inflammatory response resulting in these end-organ consequences.

Statins in COVID-19: is there any foundation?

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is the causal agent of coronavirus disease 2019 (COVID-19). Acute respiratory distress syndrome is the main cause of death from COVID-19 and occurs due to an exaggerated inflammatory response that causes the release of pro-inflammatory cytokines such as interleukins and tumor necrosis factor-alpha (TNF-α). Statins are lipid lowering drugs with pleiotropic effects. They have shown benefit in the management of inflammatory and autoimmune diseases such as systemic lupus erythematosus, rheumatoid arthritis and multiple sclerosis. Further

Low plasma butyrylcholinesterase activity in dogs: prevalence, association with underlying disorders, laboratory variables, and survival.

BACKGROUND: Low blood butyrylcholinesterase (BChE) activity is associated with obesity, pancreatitis, inflammation, sepsis, and malignancy i … 53 of 207 (25.6%) dogs with repeat in-hospital measurements. Low BChE activity was associated with systemic inflammatory response syndrome (SIRS) and sepsis, but the predictive value was lower than that of C-reactive protein (CRP). Low admission BChE activity was associated with increased odds of non-survival, lower hematocrit and cholesterol concentrations and increased band neutrophi …

A rare case of unusual anti-tuberculosis drug reaction in disseminated tuberculosis with pre-existing Multisystem Inflammatory Syndrome in Children (MIS-C).

Children with Multisystem Inflammatory Syndrome in Children (MIS-C) often have weakened immune systems and more prone to secondary infections such as tuberculosis (TB). MIS-C is a rare case but serious complication of COVID-19. It commonly affects the gastrointestinal, cardiovascular, hematologic, and mucocutaneous systems. This study presents a case of MIS-C in a 15-year-old girl complicated by respiratory failure, increased intracranial pressure with status epilepticus, grade II TB meningitis, severe dengue, disseminated TB, anti-tuberculosis drug induced hepatotoxicity, and cutaneous drug e

Nucleated red blood cells in critically ill cats.

ObjectivesThis study investigated the presence of nucleated red blood cells (NRBCs) in the circulation as a prognostic factor in critically ill cats.MethodsCritically ill cats were prospectively included over 11 months if they fulfilled at least 3/4 systemic inflammatory response syndrome (SIRS) criteria or if their general condition was severely reduced. All cats underwent a physical examination and blood collection for haematological and clinical chemical parameters, including NRBCs at admission and during hospitalisation. Outcome was defined as survival to 28 days after discharge from hospi…

Rare disease mimicking multisystem inflammatory syndrome in children.

Multisystem inflammatory syndrome (MIS-C) is a rare but serious condition associated with SARS-CoV-2, the virus that causes COVID-19. We report a 16-year-old female who presented with seven days of fever, abdominal pain, headache, and fatigue. Physical examination revealed meningeal irritation. Laboratory findings showed pancytopenia, elevated inflammatory markers, and positive SARS-CoV-2 IgG with negative PCR. The patient had no history of COVID-19 vaccination or previous confirmed infection. Initially evaluated as MIS-C due to fever, multisystem involvement, and antibody positivity, she was

MIS-C, inherited metabolic diseases and methylmalonic acidemia: a case report and review of the literature.

BACKGROUND: Methylmalonic acidemia (MMA) secondary to mutase deficiency, mut0, is an inborn error of metabolism causing complete enzyme defect, allowing a high risk of irreversible complications, secondary to metabolic decompensation, induced by infections and the hyperinflammatory state. Multisystem Inflammatory Syndrome in Children (MIS-C) is a hyperinflammatory syndrome that manifests 14-60 days after the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection in patients aged < 21 years. Only a few cases of patients with inherited metabolic diseases (IMD) and MIS-C are descr

Serial measurements of SIRS and SEP scores to identify unique phenotypes of sepsis.

Using scoring systems in discreet microbiologic cohorts in a serial fashion to identify unique phenotypes of sepsis remains unknown. Single-center, retrospective study that screened adults who triggered the hospital's SIRS (systemic inflammatory response syndrome) based sepsis alert into culture positive (Cx +) and culture negative (Cx-) groups. Subgroups were based on the location where the SIRS alert fired. SIRS scores and a novel score called SEP were calculated at t = 0 and at 3, 6, 12, and 24 h before and after t = 0. Primary outcome was a difference in SIRS/SEP scores in Cx + or Cx- grou…

Multisystem Inflammatory Syndrome in a Newborn (MIS-N): Clinical Evidence and Neurodevelopmental Outcome.

BACKGROUND: Although coronavirus disease-2019 (COVID-19) seems to be milder in children than in adults, children may exhibit severe multisystemic involvement, supported by growing evidence of this incidence in neonates. This case report aimed to demonstrate an inflammatory response syndrome in a full-term neonate born from a 35-old-year woman infected with severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2). CASE PRESENTATION: A full-term neonate girl with uneventful perinatal history was admitted with mild tachypnea at the first hour of birth and gradually worsened, resulting in sub

Post-COVID-19 Multisystem Inflammatory Syndrome-Related Cerebral Infarction in a Pediatric Patient Managed with Decompressive Craniectomy.

INTRODUCTION: Most people who are infected with the novel severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) are asymptomatic or present with mild upper respiratory symptoms. This is especially true in the pediatric population; however, rarely, a massive cytokine storm can develop, causing multisystem inflammatory syndrome associated with COVID (MIS-C). Furthermore, children may also suffer from acute ischemic strokes secondary to SARS-CoV-2 infection. CASE PRESENTATION: Here, we present a 2-year-old male who was admitted to the hospital with MIS-C and evidence of a previous SARS-Co

Misdiagnosis of multisystem inflammatory syndrome in children: A diagnostic challenge.

AIMS: As the COVID-19 pandemic continues, multisystem inflammatory syndrome in children (MIS-C) maintains its importance in the differential diagnosis of common febrile diseases. MIS-C should be promptly diagnosed because corticosteroid and/or intravenous immunoglobulin treatment can prevent severe clinical outcomes. In this study, we aimed to evaluate clinical presentation, diagnostic parameters and management of MIS-C and compare its clinical features to those of common febrile disease. METHODS: This study was conducted at a tertiary-level university hospital between December 2020 and Octob

Long-term humoral signatures following acute pediatric COVID-19 and Multisystem Inflammatory Syndrome in Children.

BACKGROUND: Although most children experience mild symptoms during acute SARS-CoV-2 infection, some develop the severe post-COVID-19 complication, Multisystem Inflammatory Syndrome in Children (MIS-C). While acute presentations of COVID-19 and MIS-C have been well immunophenotyped, little is known about the lasting immune profile in children after acute illness. METHODS: Children 2 months-20 years of age presenting with either acute COVID-19 (n = 9) or MIS-C (n = 12) were enrolled in a Pediatric COVID-19 Biorepository at a single medical center. We deeply profiled humoral immune responses and

Extracellular mitochondria drive CD8 T cell dysfunction in trauma by upregulating CD39.

RATIONALE: The increased mortality and morbidity seen in critically injured patients appears associated with systemic inflammatory response syndrome (SIRS) and immune dysfunction, which ultimately predisposes to infection. Mitochondria released by injury could generate danger molecules, for example, ATP, which in turn would be rapidly scavenged by ectonucleotidases, expressed on regulatory immune cells. OBJECTIVE: To determine the association between circulating mitochondria, purinergic signalling and immune dysfunction after trauma. METHODS: We tested the impact of hepatocyte-derived free m…

Measuring the Systemic Inflammatory Response to On- and Off-Pump Coronary Artery Bypass Graft (CABG) Surgeries Using the Tryptophan/Kynurenine Pathway.

Background: Cardiac surgeries induce many inflammatory responses with remarkable clinical implications. Tryptophan (Trp) is a precursor for serotonin, melatonin and kynurenine (Kyn). Plasma kynurenic acid (Kyna) and Kyn concentrations are thought to be related to the severity of inflammation. Plasma Trp/Kyn ratio is used to measure inflammatory cytokine activity. Methods: We performed the current longitudinal study in a tertiary care center and included 62 patients divided into two groups; group A (on-pump CABG patients) and group B (off-pump CABG patients). Plasma Trp and Kyn were measured usi

Prognostic value of systemic inflammatory response markers in cervical cancer.

We investigated the association between preoperative ratios of inflammatory markers and the prognosis in patients with invasive cervical cancer (CC). In this single-centre study, we retrospectively enrolled 163 CC patients who underwent radical hysterectomy between February 2008 and October 2018. Among the evaluated ratios, a high neutrophil-to-lymphocyte ratio (N/L) was significantly associated with deep stromal invasion and tumour size larger than 2 cm, whereas a high M/L was significantly related to advanced-stage CC (IB3-IIIC2), lymphatic metastasis (total) and pelvic lymph node metastasis
