Hip fracture or not? The reversed prevalence effect among non-experts' diagnosis.

Despite numerous investigations of the prevalence effect on medical image perception, little research has been done to examine the effect of expertise, and its possible interaction with prevalence. In this study, medical practitioners were instructed to detect the presence of hip fracture in 50 X-ray images with either high prevalence (Nsignal = 40) or low prevalence (Nsignal = 10). Results showed that compared to novices (e.g., pediatricians, dentists, neurologists), the manipulation of prevalence shifted participant's criteria in a different direction for experts who perform hip fracture dia

Advantages in diagnosis of giant cell arteritis by ultrasound.

During perceptual decision-making tasks, centroparietal electroencephalographic (EEG) potentials report an evidence accumulation-to-bound process that is time locked to trial onset. However, decisions in real-world environments are rarely confined to discrete trials; they instead unfold continuously, with accumulation of time-varying evidence being recency-weighted towards its immediate past. The neural mechanisms supporting recency-weighted continuous decision-making remain unclear. Here, we use a novel continuous task design to study how the centroparietal positivity (CPP) adapts to differen

Impact of non-invasive brain stimulation on transcallosal modulation in mild traumatic brain injury: a multimodal pilot investigation.

Background: We combined performance on working memory (WM) tasks with diffusion (dMRI) and functional (fMRI) magnetic resonance imaging in young adults who had suffered a concussion to better understand the inter-hemispheric effects of unilateral repetitive transcranial magnetic stimulation (rTMS). Methods: The article is presenting pilot data on 8 symptomatic patients with persistent post-concussion symptoms for over 6 months. They received 20 sessions of rTMS over the left dorsolateral prefrontal cortex. Fractional anisotropy and mean diffusivity of the corpus callosum (CC) and fMRI measurem

Shifted risk preferences in pathological gambling.

BACKGROUND: Pathological gambling (PG) is an impulse control disorder characterized by excessive monetary risk seeking in the face of negative consequences. We used tools from the field of behavioral economics to refine our description of risk-taking behavior in pathological gamblers. This theoretical framework allowed us to confront two hypotheses: (1) pathological gamblers distort winning probabilities more than controls; and (2) pathological gamblers merely overweight the whole probability range. Method Eighteen pathological gamblers and 20 matched healthy participants performed a decision-

Impaired auditory discrimination and auditory-motor integration in hyperfunctional voice disorders.

Hyperfunctional voice disorders (HVDs) are the most common class of voice disorders, consisting of diagnoses such as vocal fold nodules and muscle tension dysphonia. These speech production disorders result in effort, fatigue, pain, and even complete loss of voice. The mechanisms underlying HVDs are largely unknown. Here, the auditory-motor control of voice fundamental frequency (fo) was examined in 62 speakers with and 62 speakers without HVDs. Due to the high prevalence of HVDs in singers, and the known impacts of singing experience on auditory-motor function, groups were matched for singing

The Influence of Alertness on the Spatial Deployment of Visual Attention is Mediated by the Excitability of the Posterior Parietal Cortices.

With a reduced level of alertness, healthy individuals typically show a rightward shift when deploying visual attention in space. The impact of alertness on the neural networks governing visuospatial attention is, however, poorly understood. By using a transcranial magnetic stimulation twin-coil approach, the present study aimed at investigating the effects of an alertness manipulation on the excitability of the left and the right posterior parietal cortices (PPCs), crucial nodes of the visuospatial attentional network. Participants' visuospatial attentional deployment was assessed with a free

Collaborative roles of Temporoparietal Junction and Dorsolateral Prefrontal Cortex in Different Types of Behavioural Flexibility.

Behavioural flexibility is essential for everyday life. This involves shifting attention between different perspectives. Previous studies suggest that flexibility is mainly subserved by the dorsolateral prefrontal cortex (DLPFC). However, although rarely emphasized, the temporoparietal junction (TPJ) is frequently recruited during flexible behaviour. A crucial question is whether TPJ plays a role in different types of flexibility, compared to its limited role in perceptual flexibility. We hypothesized that TPJ activity during diverse flexibility tasks plays a common role in stimulus-driven att

Alternatives to positive airway pressure devices for obstructive sleep apnea: a phenotype-guided review.

INTRODUCTION: Continuous positive airway pressure (PAP) is the reference treatment for obstructive sleep apnea (OSA); however, limited adherence leaves many patients inadequately treated, creating a pressing need for effective alternatives matched to individual disease mechanisms. AREAS COVERED: This review examines the principal alternatives to PAP - custom oral appliances, upper airway and adherence-directed surgery, hypoglossal nerve stimulation, positional therapy, myofunctional and respiratory muscle training, daytime neuromuscular stimulation, neuromuscular pharmacotherapy, and weight-d

A review of mental health and psychosocial support needs in Central Africa with emphasis on service gaps and opportunities for action.

BACKGROUND: Mental health and psychosocial well-being remain critically underprioritized in Central Africa, a region characterized by persistent conflict, displacement, poverty, and fragile health systems. This narrative review synthesizes evidence on mental health and psychosocial support (MHPSS) needs across Central Africa, identifies gaps, and informs policy directions. METHODS: A systematic search of PubMed, Scopus, Web of Science, and Google Scholar was conducted (January 2010-December 2025) using keywords related to mental health, psychosocial support, and Central African countries. A landscape analysis of perinatal mental health delivery among stakeholders in Ecuador and Peru.

Perinatal mental health disorders are prevalent in Ecuador and Peru. Despite national health policies supporting maternal mental health care, service provision remains fragmented, relying on a mix of public, private, and nongovernmental actors. This study examined professional interest holders' perceptions of barriers to perinatal mental health care and the solutions they propose. We employed a mixed-methods approach. First, a systematic review of publicly available data was conducted to identify organizations engaged in maternal and mental health care in Ecuador and Peru. Following this, in-d

Integrator dynamics in the cortico-basal ganglia loop for flexible motor timing.

Flexible control of motor timing is crucial for behaviour1-4. Before volitional movement begins, the frontal cortex and striatum exhibit ramping spiking activity, with variable ramp slopes anticipating movement onsets5-12. This activity in the cortico-basal ganglia loop may function as an adjustable 'timer,' triggering actions at the desired timing. However, because the frontal cortex and striatum share similar ramping dynamics and are both necessary for timing behaviours, distinguishing their individual roles in this timer function remains challenging. Here, to address this, we conducted pert

Efficiency Recalibrates Social-Emotional Trade-Offs Behind Partner Choice in Direct Reciprocity through Intention-Specific Neural Bases.

Direct reciprocity, in which beneficiaries return favors to benefactors, is a cornerstone of human cooperation. Previous empirical work addresses partner control-how individuals decide whether and how much to reciprocate-whereas the equally critical dimension of partner choice, deciding whom to reciprocate when aided by multiple benefactors, remains understudied. This gap is addressed by testing two determinants: social-emotional motivations and reciprocal efficiency (the efficiency of reinforcing future relational capital). Combining an interpersonal task with fMRI multivariate neural express

A Sociological Framework for Improving Palliative Pain Care in Low-Resource Nations.

This paper applies institutional theory to palliative pain care in low resource nations (LRNs), suggesting that three interdependent pillars shape access: regulative (laws and rules), normative (professional standards and values), and cultural-cognitive (shared beliefs). Biomedical solutions alone are insufficient. Lasting progress requires coordinated legal reform, professional accountability, and culturally sensitive engagement. RECENT FINDINGS: Restrictive regulations, stigmatizing policy language, and administrative bottlenecks limit access to essential analgesics, while the inverse care l

Does gambling differ in people with a minority sexual orientation?

BACKGROUND: Gambling is common and there is growing concern about its public health implications. Little is known about how gambling differs in people with minority sexual identities. We sought to understand whether lesbian, gay, and bisexual (LGB) individuals differ from non-LGB individuals in terms of gambling and associated characteristics. METHODS: A total of 534 participants age 18 to 29 who gambled at least 5 times in the preceding year undertook clinical and neurocognitive evaluations. Those who identified as LGB were compared to heterosexuals on clinical and cognitive measures. RESUL

"We Need Other Human Beings in Order to be Human": Examining the Indigenous Philosophy of Umunthu and Strengthening Mental Health Interventions.

This paper examines how cultural, historical and contemporary perspectives of mental health continue to inform ways of understanding and responding to mental distress even under the biomedical gaze of the Movement for Global Mental Health (MGMH). Based on experiences in Malawi, the authors explore three prominent interventions (practical support, counselling and support groups) employed by village health workers within a mental health task-shifting initiative and reveal how the ancient philosophy of Umunthu with its values of interconnectedness, inclusion and inter-relationships informs and sh

Dyslexic children are sluggish in disengaging spatial attention.

Previous work has shown that inefficient attentional orienting is likely a causal factor for dyslexia; however, the nature of this attentional dysfunction remains unclear. The process of attentional orienting is characterized by an early facilitation effect, resulting from the successful engagement of attention, and a later inhibitory effect-frequently referred to as inhibition of return (IOR)-which encourages attentional disengagement and facilitates efficient visual sampling. The present study examined the time course of attentional orienting in dyslexic and typically developing children, by
