Relationship of temporal lobe volumes to neuropsychological test performance in healthy children.

Ecological validity of neuropsychological assessment includes the ability of tests to predict real-world functioning and/or covary with brain structures. Studies have examined the relationship between adaptive skills and test performance, with less focus on the association between regional brain volumes and neurobehavioral function in healthy children. The present study examined the relationship between temporal lobe gray matter volumes and performance on two neuropsychological tests hypothesized to measure temporal lobe functioning (visual perception-VP; peabody picture vocabulary test, third…

The value of interictal diffusion-weighted imaging in lateralizing temporal lobe epilepsy.

BACKGROUND: A subgroup of patients with nonlesional temporal lobe epilepsy (TLE) has no evidence of hippocampal sclerosis on standard temporal lobe protocol MRI. OBJECTIVE: To investigate whether interictal diffusion-weighted imaging adds lateralizing information in patients with TLE with and without lateralizing conventional MRI. METHODS: We studied 22 patients (9 right, 13 left TLE) who had undergone temporal lobectomy and 18 control subjects. We measured hippocampal volumes on high- resolution coronal magnetization-prepared rapid gradient echo scans. Apparent diffusion coefficients (ADCs)…

Olfactory dysfunction in temporal lobe epilepsy: a case of ictus-related parosmia.

Olfactory abnormalities in temporal lobe epilepsy (TLE) usually involve either brief hallucinations prior to seizures or chronic impairments in odor discrimination and identification. We describe the case of a man (B.C.) with TLE with an unusual presentation, an ictus-related parosmia. B.C. reported distorted perception of odor quality and hedonics that could provoke nausea and gagging, typically at its most extreme in the week or so following a seizure. Measures of B.C.'s olfactory functioning were obtained at stages of the ictal cycle when parosmia symptoms were severe and when they had decr…

[Lesion of extrahippocampal cortices of left medial temporal lobe: a case report].

INTRODUCTION: The cortex of medial temporal lobe is a group of different allocortical fields which included the hippocampal formation (dentate gyrus, hippocampus proper, and subicular complex) and extrahippocampal cortices (entorhinal, perirhinal, and parahippocampal). It is widely accepted that both of them play an important role in memory process. In humans, several reports indicates that damage to the hippocampus alone would lead to a clinically significant amnesia; when the injuries are more extensive others cognitive disorders, as those related to visual sphere, make worse the clinic pict…

Temporal lobe ictal behavioral patterns in hippocampal sclerosis and other structural abnormalities.

Ictal behavioral characteristics may provide clues in determining the nature of the epileptic focus. We defined ictal behavioral characteristics in patients with intractable temporal lobe epilepsy (TLE) who underwent anterior temporal lobectomy (ATL) and lived seizure-free for 2 years of follow-up. Video/EEG data on 282 seizures observed in 48 patients who suffered from TLE and underwent ATL were analyzed. All patients were seizure-free after surgery. We divided the patients into two groups on the basis of the pathological examination. Two hundred and two seizures in 35 patients with hippocamp…

The neurobiology of temporal lobe epilepsy: too much information, not enough knowledge.

Although there are many types of epilepsy of both genetic and acquired forms, temporal lobe epilepsy (TLE) with hippocampal sclerosis is probably the single most common human epilepsy, and the one most intensely studied. Despite a wealth of descriptive data obtained from patient histories, imaging techniques, electroencephalographic recording, and histological studies, the epileptogenic process remains poorly understood. Progress toward understanding the etiology of an acquired neurological disorder is largely dependent on the degree to which experimental animal models reflect the human condit…

Temporal lobe hemorrhage in the full-term neonate presenting as apneic seizures.

We describe a 1-day-old neonate born after 39 weeks of gestation with the unusual presentation of apneic seizures. He manifested apnea durin … rmalization of the EEG several days later. Head ultrasound disclosed an echogenic mass in the right temporal lobe, corroborated as hemorrhage on brain magnetic resonance imaging. This report emphasizes the importance of EEG in the evaluation of full-term neonates with recurrent unexplained apnea.

Quantitative measurement of cortical surface features in localization-related temporal lobe epilepsy.

Differences in cortical surface features between healthy controls (n = 48) and patients with temporal lobe epilepsy (n = 46), ages 14-59, were characterized by means of advanced quantitative MRI processing techniques. Cortical surface features of interest included gyral and sulcal curvature, cortical depth, and total cortical surface area. Epilepsy patients and controls differed on measures of gyrification; the abnormalities generalized despite the focal nature of the primary epileptic process. Changes in cortical surface features were associated with increasing chronological age in both group…

MRI-negative PET-positive temporal lobe epilepsy: a distinct surgically remediable syndrome.

Most patients with non-lesional temporal lobe epilepsy (NLTLE) will have the findings of hippocampal sclerosis (HS) on a high resolution MRI. However, a significant minority of patients with NLTLE and electroclinically well-lateralized temporal lobe seizures have no evidence of HS on MRI. Many of these patients have concordant hypometabolism on fluorodeoxyglucose-PET ([18F]FDG-PET). The pathophysiological basis of this latter group remains uncertain. We aimed to determine whether NLTLE without HS on MRI represents a variant of or a different clinicopathological syndrome from that of NLTLE with…

Left hemisphere dysfunction affects dichotic listening in patients with temporal lobe epilepsy.

This study evaluated the relative effect of left hemisphere dysfunction and side of seizure onset on dichotic listening performance in patients with temporal lobe epilepsy and left hemisphere speech dominance. Seventeen patients were divided into groups based on side of seizure onset and based on scores on a composite measure revealing left hemisphere dysfunction. The group with left hemisphere dysfunction had more correct responses from the left ear, and a left ear advantage, on dichotic listening. The group with normal left hemisphere function showed the expected right ear advantage. Side of…

Prolonged febrile convulsions and mesial temporal lobe epilepsy in an identical twin.

The authors report a 30-year-old identical twin who had prolonged febrile convulsions (FC) at the age of 8 months, left mesial temporal lobe epilepsy beginning at the age of 2 years, and left mesial temporal lobe sclerosis (MTS). The unaffected twin had short FC and meningitis at the age of 4 years but remained seizure free. Thus, prolonged FC in children younger than 4 years may precipitate later development of MTS.

Morphological alteration of temporal lobe gray matter in dyslexia: an MRI study.

Functional imaging studies of developmental dyslexia have reported reduced task-related neural activity in the temporal and inferior parietal cortices. To examine the possible contribution of subtle anatomic deviations to these reductions, volumes were measured for the major lobes of the brain, the subcortical nuclei, cerebellum, and lateral ventricles on magnetic resonance imaging (MRI) scans from 16 right-handed dyslexic men, ages 18 to 40, and 14 matched controls, most of whom had previously undergone PET imaging. A specific decrease in tissue volume was localized to the temporal lobes and …

Brain MRI of hippocampal volumetry in patients with refractory temporal lobe epilepsy.

OBJECTIVE: To detect for hippocampal sclerosis presurgically in a group of patients with refractory temporal lobe epilepsy. METHODS: Eighty-four consecutive patients with refractory temporal lobe epilepsy were subjected to brain MRI of hippocampal volumetry. A ratio of smaller to larger hippocampal volume was determined and unilateral hippocampal atrophy (HA) was diagnosed if the ratio is less than 0.91. RESULTS: Unilateral HA was diagnosed in 42 patients (50%) and bilateral HA in 1 patient. Other MRI abnormalities were detected in 18 patients with or without coexisting HA. CONCLUSIONS: Hip…

Cognitive correlates of 1H MRSI-detected hippocampal abnormalities in temporal lobe epilepsy.

OBJECTIVES: To examine associations between 1H magnetic resonance spectroscopic imaging (1H MRSI)-detected hippocampal creatine to N-acetylaspartate (Cr/NAA) ratios and neuropsychological measures sensitive to mesial temporal lobe function. BACKGROUND: The measurement of 1H MRSI-detected hippocampal metabolites has proved effective in determining extent and lateralization of neuronal damage. However, relationships between 1H MRSI-detected hippocampal metabolic abnormalities and specific areas of cognitive functioning have received limited attention compared to other studies using MRI volumetr…
