[Knee extensor mechanism ruptures].

Knee extensor mechanism is composed of the quadriceps and its tendon, patella and patellar tendon. Rupture of either the quadriceps or patellar tendon, or of the patella itself, lead to a disruption of the knee extensor mechanism. Clinical examination reveals an inability to actively extend the knee. Standard radiographs of the knee show a suprapatellar swelling of the soft tissues in case of quadriceps tendon rupture, or a displaced patellar fracture, or a patella alta in case of patellar tendon rupture. Echography and MRI confirm the diagnosis and may reveal associated injuries. In case of k

Histologic analysis of ruptured quadriceps tendons.

Quadriceps tendon ruptures are uncommon injuries. Degenerative changes in the tendon are felt to be an important precondition for rupture. We retrospectively reviewed 45 quadriceps tendon ruptures in 42 patients. Quadriceps tendon ruptures occurred most often in the sixth and seventh decade of life. Men were affected six times as often as women. A tissue sample from the rupture-zone was obtained in 22 cases and histologic analysis was performed. Degenerative changes were present in only 14 (64%) of the 22 samples. We observed an increasing ratio of degenerative to nondegenerative tendons with

Incarcerated patellar tendon in Hoffa fracture: an unusual cause of irreducible knee dislocation.

Irreducible knee dislocation due to incarcerated soft tissue structures in associated fractures is extremely uncommon. We report a case of irreducible, closed dislocation of the knee secondary to incarcerated patellar tendon in a Hoffa fracture (unicondylar fracture of femur in the coronal plane) with associated communited fracture of patella and disruption of both cruciates. We treated this case initially with open reduction and internal fixation of fracture and repair of the patellar tendon. During the total followup period of eighteen months, the patient had restricted joint range of motion

Tibial fractures associated with crush injuries to the soft tissues of the dorsal foot in children.

We retrospectively studied 15 children with tibial fractures associated with crush injuries to the soft tissues of the dorsal foot. The fractures, including six open fractures, were united with no complications within an average of 11.1 weeks. Wound closure to treat crush injuries of the dorsal foot was achieved using split- or full-thickness skin grafts in most patients. The outcomes of these grafts were acceptable, and all skin coverage was successful and remained viable with no breakdown. Extensor tendon injuries of the foot sustained by eight patients could not be sutured or repaired due t…

[Excessive stress damage of the lower extremity caused by sports. The value of sonographic diagnosis in stress damage of the lower extremity].

150 sportsmen with lesions and diseases of the lower extremity served as basis for demonstrating the possibilities offered by diagnosis via sonography. Main indications were articular and paraarticular diseases of the large joints (hip and knee) to demonstrate effusions, ganglia, bursae and tendon ruptures, diseases and injuries of the musculature at the thigh and lower leg, as well as the Achilles tendon with its various changes such as rupture, achillodynia and peritendinitis. Muscular and tendon ruptures can be differentiated via sonography in respect of extension, type, age and localisatio

Extrinsic risk factors for tendinopathy: bridging the basic and clinical research divide.

PURPOSE: Tendon injuries frequently affect the Achilles, rotator cuff, and the tendons of the wrist extensors originating at the lateral epicondyle. Until recently, terms for tendon degeneration, disease, and injuries were debated between the basic research and clinical communities, creating barriers to advances in knowledge. Aside from acute traumatic rupture in healthy tendon, in the current 'omic age, most tendon disease, including ruptures in chronic disease, exists along a tendinopathy spectrum. While intrinsic factors such as sex, genetics, and aging are unmodifiable, extrinsic factors i…

The glucagon-like peptide 1 receptor agonist Exendin-4 induces tenogenesis in human mesenchymal stem cells.

Tendon injuries are common and account for up to 50% of musculoskeletal injuries in the United States. The poor healing nature of the tendon is attributed to poor vascularization and cellular composition. In the absence of FDA-approved growth factors for tendon repair, engineering strategies using bioactive factors, donor cells, and delivery matrices to promote tendon repair and regeneration are being explored. Growth factor alternatives in the form of small molecules, donor cells, and progenitors offer several advantages and enhance the tendon healing response. Small drug molecules and peptid…

Calcaneal Fracture-Dislocation: A Case Report.

Calcaneal fracture-dislocations are rare, high-energy injuries in which the sustentacular fragment typically remains stable, while the posterolateral fragment is displaced laterally. These injuries are frequently associated with additional fractures, ligamentous disruption, or tendon entrapment, and they present considerable diagnostic and surgical challenges. Accurate imaging and meticulous operative planning are essential to restoring hindfoot alignment and function. We report the case of a 45-year-old woman with a calcaneal fracture-dislocation, treated via a medial approach to enable accur

[Scanning Technique in Hip Ultrasonography].

This guide to ultrasound imaging of the hip describes the basic scanning planes and contains corresponding highresolution ultrasound images. The patient and probe positionings are explained in a simplified step-by-step manner using schematic drawings. This standard and simple examination technique seems to be an appropriate way to improve the learning curve of novice musculoskeletal sonographers. In this manuscript, an emphasis is put on hip effusion detection and on how to diagnose tendon and muscle injuries. Key words: tendons, hip tendinopathy, hip joint, athletic injuries, musculoskeletal,

Spontaneous rupture of the extensor pollicis brevis tendon in a baseball pitcher: a case report.

A 15-year-old pitcher on a boys' Little League baseball team suffered spontaneous rupture of the extensor pollicis brevis (EPB) tendon. Since the EPB tendon was, at surgery, found hypoplastic and non-functional, transfer of the extensor indicis proprius (EIP) tendon was carried out. After a 6-month period of rehabilitation and follow-up, the patient was able to resume playing baseball. Although rupture of the EPB is rare, transfer of the EIP tendon is one of the treatments of choice for such injuries.

Achieving balance in tendon repair.

PURPOSE: Achieving balance in tendon repair is essential for restoring optimal function after tendon injuries, particularly in the hand and upper limb. We review and discuss key steps of 'balanced' flexor and extensor tendon repair and rehabilitation.Flexor and extensor tendon repair:To achieve optimal outcomes, the surgeon needs to consider the balance between mechanical strength and tendon gliding. Strong multi-strand repairs provide security against gapping and rupture, but overly tight or running epitenon sutures may impair synovial and blood supply to epitenon. Pulley venting in zone 2 is…

Tendon: Principles of Healing and Repair.

Tendon stores, releases, and dissipates energy to efficiently transmit contractile forces from muscle to bone. Tendon injury is exceedingly common, with the spectrum ranging from chronic tendinopathy to acute tendon rupture. Tendon generally develops according to three main steps: collagen fibrillogenesis, linear growth, and lateral growth. In the setting of injury, it also repairs and regenerates in three overlapping steps (inflammation, proliferation, and remodeling) with tendon-specific durations. Acute injury to the flexor and extensor tendons of the hand are of particular clinical importa

Genome-wide association screens for Achilles tendon and ACL tears and tendinopathy.

Achilles tendinopathy or rupture and anterior cruciate ligament (ACL) rupture are substantial injuries affecting athletes, associated with delayed recovery or inability to return to competition. To identify genetic markers that might be used to predict risk for these injuries, we performed genome-wide association screens for these injuries using data from the Genetic Epidemiology Research on Adult Health and Aging (GERA) cohort consisting of 102,979 individuals. We did not find any single nucleotide polymorphisms (SNPs) associated with either of these injuries with a p-value that was genome-wi
